  • The methodological framework of surgical innovation: the introduction of IDEAL framework and recommendation

Surgical innovation is an important part of surgical research and practice. The evaluation of surgical innovation through the stages is similar to those for drug development, but with important differences. The Idea, Development, Exploration, Assessment, and Long-term follow-up (IDEAL) Framework and Recommendations represent a new paradigm for the evaluation of surgical intervention and devices which was developed in 2009. The IDEAL is a five-stage framework involving the nature stages of surgical innovation, together with recommendations for surgical research pathway. The Framework and Recommendations were updated and published in 2019, which added a pre-IDEAL stage if necessary. The updated IDEAL also underlines the purpose, key question and ethical issues for each stage. In the first paper of IDEAL Framework and Recommendations series, we conducted a comprehensive introduction of IDEAL (e.g. the development, updates and application of IDEAL) to promote the dissemination and application of IDEAL in China.

  • The analysis of invention patents in the field of artificial intelligent medical devices

The emergence of new-generation artificial intelligence technology has brought numerous innovations to the healthcare field, including telemedicine and intelligent care. However, the artificial intelligent medical device sector still faces significant challenges, such as data privacy protection and algorithm reliability. This study, based on invention patent analysis, revealed the technological innovation trends in the field of artificial intelligent medical devices from aspects such as patent application time trends, hot topics, regional distribution, and innovation players. The results showed that global invention patent applications had remained active, with technological innovations primarily focused on medical image processing, physiological signal processing, surgical robots, brain-computer interfaces, and intelligent physiological parameter monitoring technologies. The United States and China led the world in the number of invention patent applications. Major international medical device giants, such as Philips, Siemens, General Electric, and Medtronic, were at the forefront of global technological innovation, with significant advantages in patent application volumes and international market presence. Chinese universities and research institutes, such as Zhejiang University, Tianjin University, and the Shenzhen Institute of Advanced Technology, had demonstrated notable technological innovation, with a relatively high number of patent applications. However, their overseas market expansion remained limited. This study provides a comprehensive overview of the technological innovation trends in the artificial intelligent medical device field and offers valuable information support for industry development from an informatics perspective.

  • Analysis on the innovative ability of medical postgraduate students who won the national scholarship

Objective To analyze the innovative ability of medical postgraduate students who have won national scholarships, and discuss the cultivation of innovative ability of clinical medicine postgraduate students and related factors. Methods A survey was based on the 2016 and 2017 postgraduate students majoring in clinical medicine at West China School of Medicine of Sichuan University. According to whether they had won a national scholarship, they were divided into a national scholarship group and a control group. The innovation ability and cognitive differences of the two groups of postgraduate students were compared, and related factors were explored. Results A total of 1 173 valid questionnaires were responsed, with a response rate of 80.23%. There was no statistically significant difference in gender and major between the two groups of postgraduate students (P>0.05). For master’s degree postgraduate students, the proportion of published papers in international medical journals (80.95% vs. 29.63%), published high impact factor (impact factor≥6) journal papers (7.84% vs. 6.16%), applied for research patents (12.70% vs. 3.51%) and innovation and entrepreneurship competition awards (20.63% vs. 3.93%) in the national scholarship group were higher than the control group, the differences were statistically significant (P<0.05). For doctor’s degree postgraduate students, the proportion of published papers in international medical journals (96.00% vs. 69.66%), published high impact factor journal papers (22.22% vs. 8.89%), applied for research patents (17.33% vs. 8.36%), and innovation and entrepreneurship competition awards (17.33% vs. 1.86%) in the national scholarship group were higher than the control group, the differences were statistically significant (P<0.05). Regarding the cognition of the composition of innovation ability, the differences between the national scholarship group and the control group were not statistically significant (P>0.05). Regardless of master’s degree or doctor’s degree, more than 40% of the postgraduate students in the national scholarship group and the control group believed that the most important factor in the personal influence of innovation ability was the knowledge factor, and more than 45% of the postgraduate students believed that the most important factors affecting the cultivation of innovative ability was their own factors or tutors’ factors. Conclusions The innovative ability of clinical medicine postgraduate students needs to be improved. The level of tutors’ scientific research ability, tutors’ guidance, high-quality scientific research team, and a good academic atmosphere play an important role in the cultivation of innovation ability. Optimizing the team of tutors, forming a scientific research steering group, building a high-level innovation platform, aiming at publishing high-level papers, strengthening domestic and foreign exchanges and cooperation, and motivating students to participate in innovation and entrepreneurship competitions are powerful measures to cultivate innovative medical talents.

  • Role of participation in academic student associations in enhancing scientific innovation capability and comprehensive quality among undergraduate medical students

Objective To investigate the role of participation in academic student associations in enhancing the scientific innovation capability and comprehensive quality of undergraduate medical students. MethodsThis study was conducted from November to December 2022. Undergraduate medical students majoring in clinical medicine (five-year program) in grade 2017 and 2018 at West China School of Medicine of Sichuan University were included in the study. These students were divided into two groups based on their participation in a five-star academic student associations of West China School of Medicine: the exposure group and the control group. The study compared the average scores of compulsory courses during the first four years of undergraduate study, competency multi-station assessment scores, comprehensive quality assessment scores, and scientific innovation capability between the two groups. Subgroup and stratification analyses were also conducted based on grade level and average scores of compulsory courses. Results A total of 433 students were surveyed. Among them, there were 348 students in the control group and 85 students in the exposure group. The average scores of compulsory courses, competency multi-station assessment scores, comprehensive quality assessment scores, and scientific innovation capability of students in the exposure group were higher than those of students in the control group (P<0.05). Similar results were observed in subgroup analysis by grade level. The sub group analysis of the average scores of compulsory courses showed that in tertileⅠ(≥ 85 and ≤ 100 points) and tertileⅡ (≥ 80 and<85 points), the competency multi-station assessment scores, comprehensive quality assessment scores, and scientific innovation capability of the exposure group were higher than those of the control group (P<0.05). However, in tertile Ⅲ (>0 and<80 points), the exposure group showed better scores in competency multi-station assessment scores and scientific innovation capability than the control group (P<0.05). Conclusion Participation in academic student associations can effectively improve the scientific innovation capability and comprehensive quality of students with medium and above grades.

  • SWOT model analysis and innovative strategies for high quality development of pediatric day-care surgery in comprehensive hospitals

The developing status of pediatric day-care surgery is comprehensively analyzed based on SWOT model from the following four dimensions: strength (S), weakness (W), opportunities (O) and threats (T). Day-care surgery strategy is in line with the characteristics of pediatric diseases, which can improve medical efficiency and reduce medical costs. However, it is limited by both the particularity of pediatric patients and the complexity of postoperative management. In terms of external environment at present, there are many opportunities for its development of day-care surgery such as national policy support, medical technology innovation, and regional medical center construction. Nevertheless, some threats cannot yet be ignored such as medical safety risks, unfair resource allocation, traditional habit of medical treatment and medical insurance payment policies. In order to promote the high-quality development of pediatric day-care surgery in the future, it is recommended to innovate medical service models, improve medical quality management systems, train medical qualified personnel, and enhance national policy support.
