目的:为临床合理应用抗生素提供依据。方法:采用VITEK 32及GNS--120药敏卡、GPS -107药敏卡进行细菌的鉴定及药敏实验。结果:320 株病原菌中,革兰氏阳性菌占28.75 %,革兰氏阴性菌占71.25 %,其中大肠埃希菌、铜绿假单胞菌、鲍曼复合醋酸钙不动杆菌、阴沟肠杆菌、金黄色葡萄球菌是临床上主要致病菌。结论:临床应科学合理选用抗生素,尽量减少和延缓耐药菌的发生及发展。
Objective To investigate drug usage and costs of angina inpatients in Karamay Central Hospital in 2014 and to provide evidence and reference for evidence-based pharmacy study of single disease in circulation system. Methods The information of drug use and expenditure of angina inpatients were collected from the hospital information system (HIS). We analyzed the data including frequency, proportion and cumulative proportion by Excel 2007 software. Results We included 351 angina inpatients in Karamay Central Hospital in 2014 whose average age was 69.44±10.65 years old. Fifty-one kinds of drugs were used for angina therapy. The expenditure of drugs was 122.46 yuan per person. The symptomatic treatment drugs included 11 kinds of drugs. The top 3 usage drugs were isosorbide denigrates, aspirin, nitroglycerin. The secondary prevention medicines included 35 kinds of drugs, and were prescribed for 1 953 times; the anti-hypertensive drugs were most prescribed. The Chinese patent medicine were prescribed for 69 times. The compound Danshen dripping pill was the most prescribed drug. In total expenditure, the cost of clopidogrel, atorvastatin, irofiban ranked top three. Rivaroxaban, tirofiban, low molecular weight heparin were the top 3 drugs for per-time expenditure. Conclusion The top 3 used drugs are isosorbide denigrates, aspirin, nitroglycerin for angina inpatients in Karamay Central Hospital in 2014, while the top 3 total expenditure drugs are clopidogrel, atorvastatin and irofiban.
目的 分析都江堰市人民医院药品不良反应(ADR)的发生情况及引发ADR的相关因素,为临床合理用药及正确评价ADR的发生提供参考。 方法 对医院2009年1月-2011年12月收集上报至全国ADR监测网络的168例ADR报告,进行回顾性分类与统计分析。 结果 ADR报告例数最多的为临床科室,男女病例数比为0.87∶1,静脉给药途径引发的ADR最多(占77.98%);抗感染药物的ADR发生率最高(占61.90%);皮肤及其附件损害最常见占(27.98%)。 结论 抗感染药物和中药注射剂是ADR监测的重点药物,应加强ADR监测及相关知识的宣传,提高合理用药水平,减少药源性疾病的发生。
ObjectiveTo investigate and analyze the antibiotic use in Sichuan Province. MethodsAntibiotic use of inpatients was investigated in 23 hospitals on the day of June 25, 2011. ResultsThe percentage of antibiotic use was 42.4% in the total 27 144 inpatients, the range of which was between 24.3% and 63.3% for each hospital. And 63.8% of the antibiotic use was for therapeutic purpose, and 72.2% used only one antibiotic drug. A total of 36.9% of the patients receiving antibiotic treatment for therapeutic use sent samples for pathogenic detection. With the increase of the size of the hospital (judged by the number of beds), antimicrobial drug utilization decreased with statistical significance (χ2=411.834, P=0.000). With the improvement of regional economic level (judged by per-capita GDP of each region), the hospital antimicrobial usage decreased with statistical significance (χ2=500.093, P=0.000). The three types of antimicrobial drug most frequently used were other β-lactam antibiotics, penicillin and third generation cephalosporin. ConclusionAntibiotic use is reasonable in Sichuan Province as a whole. However, it is necessary to reduce irrational use of antimicrobial agents continuously, and to strengthen the management of clinical antibiotic usage among inpatients.
ObjectiveTo investigate the status of antibiotic drug use in the outpatient department based on the special rectification activities of antibiotic drugs, identify the reasons for irrational use of antibiotic agents and propose the solutions. MethodsPrescription of antibiotic drugs were analyzed by a retrospective comparative analysis during April to June 2011, August to October 2011 and August to October 2012. ResultsPrescriptions of antibiotic drugs were 435 (36.25%), 367 (30.58%) and 223 (18.58%) during April to June 2011, August to October 2011 and August to October 2012, respectively, and the unreasonable prescriptions were 35 (2.92%), 27 (2.25%), and 14 (1.25%) respectively. ConclusionThere is a decreased tendency of prescription of antibiotic drugs in the outpatient department of our hospital because of the special rectification activities of antibiotic drugs. It is in accordance with antibiotic use rate of <20% target of "antibiotics in 2012 special rectification program" in hospital outpatient service. But there are some unreasonable phenomena in the use of antibiotics, and to promote the reasonable use of antibiotics, we should strengthen management.

We analyzed the data including frequency, proportion and cumulative proportion by Excel 2007 software. Results We included 522 ischemic stroke inpatients in Karamay Central Hospital in 2014, the average age was 67.58±13.27 years old. Forty three kinds of drugs were used for ischemic stroke therapy. The total expenditure of drugs was 10.94 million and 1 807.99 yuan per person. The most used drugs were for symptomatic treatment which included 18 kinds of drugs. The using frequency of aspirin, edaravone, oxircetan were the highest. The secondary prevention medicines included 23 kinds of drugs. The anti-hypertensive, lipid-lowering and hypoglycemic agents were prescribed mostly. Traditional Chinese medicines were prescribed 234 times, and the injection Ligustrazine with glucose was the most used drug. Conclusion The mainly drugs for treatment of ischemia stroke inpatients in Karamay Central Hospital in 2014 are aspirin, edaravone and oxircetan; while the expenditure of clopidogrel and oxiracetam were the highest.
ObjectiveTo analyze the clinical distribution and changes of antimicrobial resistance profiles of Staphylococcus aureus (SA), as well as to provide the basis for the prevention and treatment of infection. MethodsThe clinical data and the antimicrobial resistance profiles of SA were collected from Jan, 2008 to Dec, 2014 in West China Hospital of Sichuan University. The WHONET 5.5 software was used to analyze the resistance data. ResultsA total of 5 698 SA isolates were included within 7 years. Of all strains, 2 721 (47.8%) were isolated from secretion, 1 638 (28.75%) were from respiratory tract specimens, 451 (7.9%) were from pus, and 362 (6.4%) were from blood. 811 (49.5%) SA isolates from respiratory tract specimens were Methicillin-resistant Staphylococcus aureus (MRSA), which was higher than those from secretion, pus and blood. 1052 (18.5%) SA strains were isolated from the dermatological department, 604 (10.6%) were from the orthopedics department, 472 (8.3%) were from the intensive care unit (ICU), 471 (8.3%) were from the department of burn, and the detection rate of MRSA from ICU (341, 72.2%) was the highest. During last 7 years, the total separation rate of SA was 8.2%, among them 1 858 (32.6%) MRSA were isolated, and the detection rate was 32.6%. The resistant rate of SA to erythromycin, clindamycin, tetracycline, gentamicin, rifampin, ciprofloxacin, levofloxacin and moxifloxacin had a statistically significant decrease from 2008 to 2014, while the resistant rate of SA to trimethoprim/sulfamethoxazole had increased. No vancomycin, linezolid, teicoplanin or tigecycline resistant strain was detected. The resistance rates of MRSA to common antibiotics such as penicillin G, erythromycin, clindamycin, tetracycline, gentamicin, rifampin and fluoroquinolones were higher than those of MSSA, while the resistance rate of MRSA to trimethoprim/sulfamethoxazole was lower than MSSA. ConclusionCompared with the monitoring data in China, the drug resistance of SA in West China Hospital is well controlled. However, experience-directed antibiotic treatment of MRSA infection is still limited. MRSA infection remains a serious problem in critically ill patients. The rational use of antibiotics and application of effective infection control measures are important to decrease the MRSA infection.
Objective To investigate the awareness of rational use of drug (RUD) on medical workers in hospitals of different ranks, the establishment and execution of relevant strategies in hospitals of different ranks, and the main factors affecting RUD on medical workers, so as to provide references for the generalization of RUD. Methods A certain number of different ranks of hospitals in Chengdu and Zigong were randomly selected by the convenience sample method. Medical workers in each hospital were selected through face-to-face interviews with a self-designed questionnaire. Monte Carlo Chi-square analysis was applied on the hospital ranks and formulation of RUD guidelines, and logistic regression analysis was performed on the awareness of RUD knowledge of the respondents. Results Among 700 questionnaires distributed, 672 were retrieved (response rate 96.0%). The numbers of respondents who were familiar with the Pharmaceutical Affairs Councils (PACs) as well as the Pharmacy and Therapeutics Committees (PTCs) were 217 (32.29%) and 83 (12.35%), respectively. The number of respondents who understood the concept of essential medicine was 502 (74.71%). A total of 441 (61.16%) medical workers had been trained on RUD, 199 (29.61%) medical workers denied there were PTCs in their hospitals, and 60.71% of the medical workers mentioned that the use of drug was monitored and controlled with the feedback in their hospitals. The result of logistic regression analysis indicated that the professional title (P=0.038), awareness of essential medicine (Plt;0.001) and participation of RUD training (P=0.008) were the factors influencing the awareness of RUD. Conclusion Both management and education should be executed at the same time to elevate the level of rational use of drug. In order to increase the level of RUD we should complete monitoring institution and improving RUD awareness of workers. Additionally enhancing the rationality of medication should be performed by establishing guidelines of treatment or medication. Improving personal knowledge, scientific information and medication custom should be performed by standardizing regulations, strengthening management, emphasizing education and promoting communication between doctors and patients.
