  • Effect of early rehabilitation therapy on complications and prognosis in mechanically ventilated elderly patients with severe pneumonia

    Objective To explore the effect of early rehabilitation treatment on complications and prognosis of elderly patients with sever pneumonia undergoing mechanical ventilation. Methods The patients who meeting the inclusion criteria were randomly divided into an early rehabilitation group and a control group, with 35 cases in each group. On basis of same routine treatment, the early rehabilitation group was treated with early rehabilitation. The early rehabilitation methods included exercise therapy, electrical stimulation therapy, swallowing therapy, cough training and wheelchair-bed transfer training, etc. The patients received individual training methods according to their conditions. The difference of two groups were observed in the rates of ICU-acquired weakness (ICU-AW), ventilator-associate pneumonia (VAP), the incidence of delirium, the mechanical ventilation time, ICU-hospital time, total hospital time, 30-day hospital mortality, extubation fail rate and tracheotomy rate. Results Compare with the control group, the incidence of ICU-AW (14.28% vs. 37.14%), VAP (8.57% vs. 28.57%), and delirium (40.00% vs. 65.71%) in the early rehabilitation group were significantly reduced (all P<0.05). The duration of delirium [(3.50±1.31) dvs. (6.40±1.47) d], the ventilation time [(6.32±2.19) d vs. (9.40±4.43) d], ICU hospitalization time [(10.80±3.64) d vs. (15.31±3.85) d] and total hospitalization time [(22.52±7.56) d vs. (30.22±11.54) d] of the early rehabilitation group were significantly lower than the control group (all P<0.001). The tracheotomy rate and 30-day hospital mortality of the early rehabilitation group were significantly lower than the control group (25.71%vs. 51.42% and 28.57% vs. 54.28%, both P<0.05). There was no significant difference in extubation fail rate (5.71%vs. 11.42%, P>0.05). In the early rehabilitation group, there were no complications such as pipe prolapse, limb injury or serious arrhythmia. Conclusion Early rehabilitation can reduce the incidence of ICU-AW, VAP, delirium in elderly patients with severe pneumonia, help to shorten the mechanical ventilation time, ICU hospitalization time and total hospitalization time, reduce extubation failure rate and tracheotomy rate, so it is safe and effective, and worthy of being popularized and applied.

  • ALLOGRAFTING FOR MASSIVE BONE DEFECT: BONE AND SOFT TISSUE RECONSTRUCTION AND POSTOPERATIVE REHABILITATION

    OBJECTIVE: To evaluate the results of limb function and the methods of bone and soft tissue reconstruction of patients treated with allografting. METHODS: From May 1992 to January 1999, 90 patients suffered from bone malignant tumor were treated with allografting in different methods of internal fixations. The average follow-up was 37.5 months. The limb postoperative function, complications related to different surgical methods were compared according to Enneking evaluation system. RESULTS: Skin necrosis, infection, non-union, fracture of allograft were the main complications which affect patients’ limb postoperative functions. Of the 90 fresh-frozen allografting procedures, the final results of operation showed that hip joints and knee joints were better than the shoulder joints. More than 80% of the patients treated with interlocked intramedullary nail and allograft-prosthesis combination led to an over-all result that was excellent and good. Interlocked intermedullary nail was of recommended method of internal fixation. Early exercises of operative limbs could promote function recovery. CONCLUSION: Using of interlocked intramedullary nail and allograft-prosthesis combination are of recommended operation method and can be applied with better results, and early exercises of operative limbs will lead to better functions.

  • ERAS combined with preoperative oral nutrition supplement in patients with rectal cancer and its effect on postoperative stress response

    ObjectiveTo investigate the effect of enhanced recovery after surgery (ERAS) combined with preoperative oral nutrition supplement (EnsourceTM) in patients with rectal cancer and its effect on postoperative stress response.MethodsFrom January 2018 to August 2018, 80 patients with laparoscopic assisted radical resection of rectal cancer in our hospital were divided into two groups according to different perioperative management. Forty patients who had used enhanced recovery after surgery combine preoperative application of tumor total nutrition formula enteral nutrition solution were as observation group, while other 40 patients who only had used enhanced recovery after surgery in perioperative as control group. We observed the stress response (mean systolic blood pressure, mean diastolic blood pressure, mean heart rate, mean blood oxygen saturation, pain score and fasting blood glucose) at 24 hours after surgery and the time of the first anal exhaust, complication rate, hospitalization time, and hospitalization cost after surgery in both groups. We also compared the early postoperative (on the second day after operation) nutritional status of the two groups like the levels of serum total protein, albumin and proalbumin.ResultsThe mean systolic blood pressure, mean diastolic blood pressure, mean heart rate, mean blood oxygen saturation, pain score and fasting blood glucose of the observation group at 24 hours after surgery were significantly lower than those of the control group (P<0.05), while the total serum protein, albumin and prealbumin concentrations of the observation group were significantly higher than those of the control group on the second day after surgery (P<0.05). Postoperative hospitalization time and hospitalization cost in the observation group were also shorter or less than those in the control group (P<0.05). Compared with the control group, the first postoperative anal exhaust time was earlier and the incidence of postoperative complications was reduced in the observation group, but the difference was not statistically significant (P>0.05).ConclusionsERAS combined with preoperative application of tumor total nutrition formula enteral nutrition fluid (EnsourceTM) can reduce the postoperative stress response and the postoperative complication rate of patients with rectal cancer, and also improve the postoperative nutrition status of patients and promote the rapid recovery of patients after sugery.

  • Feasibility and safety of tension-free vaginal tape-obturator for female stress urinary incontinence under the daytime surgical mode based on the concept of enhanced recovery after surgery

    Objective To explore the feasibility and safety of tension-free vaginal tape-obturator for female stress urinary incontinence under the daytime surgical mode based on the concept of enhanced recovery after surgery. Methods The clinical data of female patients with stress urinary incontinence at the First Affiliated Hospital of Kunming Medical University between June 2019 and June 2023 were retrospectively analyzed. According to the perioperative management mode of patients, they were divided into daytime surgery group and routine surgery group. The basic, intraoperative, and postoperative conditions of two groups of patients were compared. Results Finally, 183 patients were included, including 91 in the routine surgery group and 92 in the daytime surgery group. All patients successfully completed the surgery. There was no statistically significant difference in age, preoperative comorbidities, surgeon in chief, or operation duration between the two groups of patients (P>0.05). The preoperative waiting time after hospitalization [(0.00±0.00) vs. (2.42±0.58) d], hospitalization expenses [(13815.10±2906.01) vs. (18095.21±3586.67) yuan], total surgical expenses [(3961.36±707.35) vs. (4440.19±1016.31) yuan], anesthesia expenses [(718.53±61.06) vs. (755.30±74.65) yuan], western medicine expenses [(818.07±259.30) vs. (1282.14±460.75) yuan], total hospitalization duration [(1.11±0.31) vs. (5.77±1.30) d], and postoperative hospitalization duration [(1.11±0.31) vs. (3.35±1.42) d] in the daytime surgery group were lower than those in the routine surgery group (P<0.05). There was no significant difference between the two groups in postoperative complications (respiratory complications, fever, nausea and vomiting, vaginal bleeding, urinary retention, peritonitis), satisfaction, postoperative pain or self perception of symptom improvement (P>0.05). Conclusion The daytime surgery for female stress urinary incontinence based on the concept of enhanced recovery after surgery is safe and feasible, which can shorten hospitalization duration and reduce hospitalization costs.

  • 加速康复外科理念在良性前列腺增生围手术期护理中的应用

    目的 探讨加速康复外科(enhanced recovery after surgery,ERAS)理念在良性前列腺增生患者围手术期护理中的效果。 方法 纳入 2013 年 6 月—2015 年 4 月实施前列腺电切手术患者 60 例,随机分为研究组和对照组各 30 例。研究组按照 ERAS 理念进行护理,对照组给予常规护理。对两组患者术后下床时间、术后排气时间、术后住院时间、尿管留置时间、住院费用,以及术后并发症进行观察比较。 结果 研究组术后下床时间、术后排气时间和术后住院时间分别为(20.3±5.7)h、(23.4±7.3)h 和(3.4±0.6)d,较对照组的(25.2±6.6)h、(27.8±6.6)h、(3.7±0.7)d 缩短,差异有统计学意义(P<0.05);尿管留置时间两组之间差异无统计学意义(P>0.05);对照组总体并发症发生率(40.0%)和住院费用[(1.3±0.3)万]高于研究组[16.7%,(1.1±0.2)万],差异有统计学意义(P<0.05)。 结论 将 ERAS 理念应用于良性前列腺增生围手术期护理,能促进患者康复,减轻患者痛苦,有利于缩短住院时间,降低并发症发生率和患者经济负担,值得临床推广。

  • Digital drainage system versus traditional drainage system for thoracic drainage after lung surgery: A case control study

    Objective To compare the digital drainage system and the traditional drainage system in the patients after lung surgery, and to evaluate the advantages of digital drainage system. Methods A retrospective analysis of consecutive 42 patients with lung surgery between September 2016 and May 2017 in Beijing University International Hospital was done. There were 30 males and 12 females with a median age of 34 years ranging 19-81 years. After the surgery 21 patients adopted Thopaz digital drainage device (a DDS group), and the other 21 patients adopted traditional drainage (a TDS group). Duration of air leakage and chest tube placement, length of hospital stay, thoracic drainage volume within 48 h and hospitalization expenses in the two groups were compared. Results The patients in the two groups were all successfully discharged. Compared with the TDS group, duration of air leakage and chest tube placement and length of hospital stay significantly shortened in the DDS group (35.6±16.3 h vs. 48.2±20.1 h, P=0.02; 50.0±16.1 h vs. 62.0±20.4 h, P=0.03; 5.9±2.3 d vs. 7.8±3.5 d, P=0.02), and thoracic drainage volume within 48 h and hospitalization expenses showed no significant statistical difference between the two groups. Conclusion Using digital drainage system after lung surgery can significantly shorten the duration of air leakage and the postoperative drainage, at the same time, without increasing the overall hospitalization expenses.

  • Bibliometric and visualized analysis of peripheral magnetic stimulation

    Objective To conduct a bibliometric analysis of research hotspots and development trends in the application of peripheral magnetic stimulation (PMS), systematically review the research evolution, and provide references for future studies. Methods We retrieved PMS-related literature from the Science Citation Index Expanded database in Web of Science Core Collection (January 1, 1989 - March 5, 2025). Using VOSviewer (v1.6.20) and CiteSpace (v6.1.R6), we performed annual publication volume analysis, country/institution/author collaboration network analysis, reference co-citation analysis, and keyword co-occurrence/clustering/burst analysis, with corresponding knowledge mapping visualizations. Results A total of 124 publications were included. Both publication counts and citation frequencies remained low in early years but showed steady annual growth after 2011. The publications originated from 21 countries/regions, with Germany being the most productive nation, Technical University of Munich the leading institution, and Schneider C (from the Research Center of Neuroscience and Rehabilitation Sciences, Laval University, Canada) the most prolific author. Keyword analysis revealed three major research clusters: neuromodulation technologies, physiological mechanisms, and clinical rehabilitation applications. Future research priorities included expanded clinical applications integrating neuroelectrophysiological indicators with rehabilitation assessments, and interdisciplinary optimization of stimulation parameters through engineering approaches. Conclusions PMS has gained widespread clinical application with growing scientific interest. Future studies should focus on current hotspots to advance precision and intelligent development of this technology.

  • Action guideline on promoting the high-quality development of rehabilitation services in public hospitals

    According to a guideline on promoting the high-quality development of public hospitals issued by the General Office of the State Council and a guideline on accelerating the development of medical rehabilitation, a high-quality and efficient rehabilitation system should be established, and efforts should be made to promote the high-quality development of rehabilitation services in public hospitals. We propose an action guideline for promoting the high-quality development of rehabilitation services in public hospitals, which is based on a Delphi method questionnaire on the opinions of front-line clinical rehabilitation experts, and integrated with relevant literature and evidences. This guideline introduces 31 specific action plans from 7 dimensions, including standardization of basic rehabilitation settings, standardization of clinical rehabilitation techniques, improvement of the tertiary rehabilitation system, standardization of emergency rehabilitation, standardization of rehabilitation education, standardization of rehabilitation management, and promotion of scientific and technological innovation in rehabilitation medicine.

  • Multi-modal synergistic quantitative analysis and rehabilitation assessment of lower limbs for exoskeleton

    In response to the problem that the traditional lower limb rehabilitation scale assessment method is time-consuming and difficult to use in exoskeleton rehabilitation training, this paper proposes a quantitative assessment method for lower limb walking ability based on lower limb exoskeleton robot training with multimodal synergistic information fusion. The method significantly improves the efficiency and reliability of the rehabilitation assessment process by introducing quantitative synergistic indicators fusing electrophysiological and kinematic level information. First, electromyographic and kinematic data of the lower extremity were collected from subjects trained to walk wearing an exoskeleton. Then, based on muscle synergy theory, a synergistic quantification algorithm was used to construct synergistic index features of electromyography and kinematics. Finally, the electrophysiological and kinematic level information was fused to build a modal feature fusion model and output the lower limb motor function score. The experimental results showed that the correlation coefficients of the constructed synergistic features of electromyography and kinematics with the clinical scale were 0.799 and 0.825, respectively. The results of the fused synergistic features in the K-nearest neighbor (KNN) model yielded higher correlation coefficients (r = 0.921, P < 0.01). This method can modify the rehabilitation training mode of the exoskeleton robot according to the assessment results, which provides a basis for the synchronized assessment-training mode of “human in the loop” and provides a potential method for remote rehabilitation training and assessment of the lower extremity.

  • Guideline for the rehabilitation treatment of osteoporosis (2024 edition)

    Osteoporosis (OP) is one of the most serious health problems, causing a huge economic burden to patients, families and society. OP rehabilitation treatment plays an important role in pain-relieving, reducing the risk of fracture, improving the ability of daily activities, and promoting the healing of osteoporosis fractures, and has been increasingly valued and recommended by domestic and foreign guidelines. This guideline was updated based on the Chinese expert consensus on rehabilitation intervention for primary osteoporosis 2019 editions, and was initiated by Chinese Society of Physical Medicine and Rehabilitation professor HE Chengqi, Chairman of the Chinese Medical Association Physical Medicine and Rehabilitation Branch. This guideline development was guided by domestic and international guideline development methods and principles, and selected through clinical issue screening and deconstruction, and two rounds of Delphi questionnaire consultation. The International Classification of Functioning, Disability and Health (ICF) was used as the theoretical framework, and the grading of recommendations assessment, development and evaluation (GRADE) was used based on the best available evidence. The ICF is a theoretical framework based on the best available evidence, and uses the GRADE method to grade the quality of evidence and recommendations, and reports them according to the Reporting Items for Practice Guidelines in Healthcare (RIGHT), taking into account patients’ preferences and values and the needs of Chinese clinical practice, resulting in 22 recommendations in 7 aspects of rehabilitation assessment, physical agents therapy, occupational therapy, rehabilitation brace, cognitive behavior and psychological therapy, traditional Chinese medicine therapy and health education to systematically standardize the rehabilitation of osteoporosis.
