  • 电视胸腔镜辅助食管癌切除术

    目的 探讨电视胸腔镜辅助食管癌切除术的方法。方法 2000年12月-2001年5月我科用胸腔镜辅助施行食管癌切除术5例,均为食管中段癌,0期1例。Ⅰ期2例,Ⅱa期1例,Ⅱb期1例。结果 手术经过均顺利,无1例中转开胸手术,手术时间平均180min,术中出血量平均210ml,无严重术后并发症发生,无手术死亡,均治愈出院。结论 随着胸腔手术设备和器械的改进,手术操作技术的熟练,选择适当的患者采用电视胸腔镜辅助食管癌切除术是可行的,且手术效果良好。

  • Comparison of three-dimensional laparoscopic simulator with two-dimensional laparoscopic simulator in training of laparoscopic novices

    ObjectiveTo compare the three-dimensional (3D) laparoscopic simulator with two-dimensional (2D) laparoscopic simulator in training of laparoscopic novices.MethodsBetween January 2018 and December 2019, surgical residents from Chinese PLA General Hospital were enrolled, which were grouped into 3D and 2D group. After receiving training program, novices in both two groups subject to performance examination, including bean-picking module, exchange module, transfer module, needle-manipulating module, and suture module. Times and errors were compared between the two groups for each module.ResultsA total of 16 novices in 3D group and 15 novices in 2D group were enrolled, and baseline characteristics including age, gender, major hand, glass wearing, laparoscopic experience, and shooting game experience were well balanced between the two groups (P>0.05). There were comparable times and errors between the two groups in terms of bean-picking module and exchange module (P>0.05). The time of transfer module and needle-manipulating module was not significant between the two groups (P>0.05), but novices in 3D group performed more precise than those in 2D group (P<0.05). In suture module, 3D group had shorter time (P=0.02) and higher accuracy (P=0.03).Conclusion3D laparoscopic simulator can shorten novice performance time in complex procedures, improve accuracy, and facilitate laparoscopic training.

  • Investigation and analysis on psychological pressure of surgeon-in-chief to complete animal remote surgery using home-made Tumai surgical robot in combination with 5G network

    ObjectiveTo understand the psychological pressure when the surgeon-in-chief remotely completes animal surgery using home-made Tumai surgical robot in combination with 5G network (Abbreviated as“remote robotic surgery” ), and investigate and analyze the sources of psychological pressure, and then provide evidence guidance for the training of surgeon in performing remote robotic surgery. MethodsA modified perceptual stress scale was used to conduct a questionnaire survey. The 44 surgeons-in-chief with robot operation qualification from 10 medical units in the Gansu Province recruited by Gansu Provincial People’s Hospital were as observation subject, who participated in the psychological stress validation test of remote robotic surgery, from September 4, 2022 to October 10, 2022. The difference of psychological stress before and after the test was compared. The stressor of surgeon-in-chief was analyzed by Likert scale. The animals in this study were swines. ResultsA total of 132 valid questionnaires were obtained from 44 surgeons-in-chief. The surgical physician’s perceived stress score after the test was statistically higher than before the test [(47.50±9.06) points vs. (38.34±5.55) points, mean difference and its 95% confidence interval=9.61 (7.00, 12.27), t=7.42, P<0.001]. The analysis results of multiple linear regression showed that the number of robotic surgery performed by the surgeon-in-chief in the past had a negative impact on the psychological pressure of the surgeon-in-chief after the test (β=–0.292, P=0.042); At the same time, the subjective perception data of the surgeon-in-chief, such as unstability of signal transmission and unskilled equipment manipulation by the surgeon had positive impacts on the psychological pressure of the surgeon-in-chief after the test (β=1.987, P=0.026; β=3.184, P=0.010), and the tacit understanding between the surgeon-in-chief and the first assistant had a negative impact on the psychological pressure of the surgeon-in-chief after the test (β=–2.185, P=0.047). ConclusionsAccording to the data from this study, remote robotic surgery will increase the psychological pressure of the surgeon-in-chief. Previous experience in robotic surgery can reduce psychological stress after surgery. The main sources of psychological pressure are unstability of signal transmission, unskilled equipment manipulation by the surgeon, and the tacit understanding between the surgeon-in-chief and the first assistant.

  • Oncologic and short-term outcomes of robotic versus laparoscopic total mesorectal excision for rectal cancer: a cohort study

    ObjectiveTo compare oncologic and short-term outcomes between the robotic and laparoscopic total mesorectal excision for rectal cancer. Methods This is a retrospective cohort study using a prospectively collected database. Patients’ records were obtained from Gansu Provincial Hospital between July 2015 and October 2017. Eighty patients underwent robotic-assisted total mesorectal excision (R-TME group) and one hundred and sixteen with the same histopathological stage of the tumor underwent an laparoscopic total mesorectal excision (L-TME group). Both operations were performed by the same surgeon. Results The time to the first passage of flatus [(3.28±1.64) d vs. (6.01±2.77) d, P<0.001], the time to the first postoperative oral fluid intake [(4.46±1.62) d vs. (6.28±2.74) d, P<0.001) and the length of hospital stay [(11.20±5.80)d vs. (14.72±6.90) d, P=0.023] of the R-TME group was about 3 days faster than the L-TME group. The incidence of postoperative urinary retention (2.50% vs 7.76%, P=0.016) was significantly lower in the R-TME group than the L-TME group. However, the intraoperative blood loss of the R-TME group was more than the L-TME group [(175.06±110.77) mL vs. (123.91±99.61) mL, P=0.031, ). The operative time, number of lymph nodes harvested and distal margin were similar intergroup(P>0.05). The total cost was higher in the R-TME than in the L-TME group [(85 623.91±13 310.50) CNY vs. (67 356.79±17 107.68) CNY, P=0.084), however, this difference was statistically insignificant. ConclusionsCompared with the L-TME, the R-TME has the same oncologic outcomes and rapid postoperative short-term recovery. However, the long-term outcome of the R-TME remains to be further observed.

  • 心脏搏动下经右心室微创封堵婴幼儿肌部室间隔缺损

    目的探讨心脏搏动下经右心室微创封堵婴幼儿肌部室间隔缺损的临床效果。 方法回顾性分析2010年1月至2013年1月兰州大学第一医院心血管外科39例肌部室间隔缺损患儿行心脏搏动下经右心室微创封堵肌部室间隔缺损的临床资料,其中男26例、女13例,年龄10个月至3岁,体重6~15kg。应用经胸超声心动图进行术后随访,观察封堵器的位置,有无移位、有无残余分流、塑型情况及毗邻瓣膜是否开闭等。观察各瓣膜反流情况、各瓣膜口血流及房室腔的变化,心脏功能、肺动脉高压恢复情况等。 结果39例患儿中36例(92.3%)植入封堵器成功封堵,有3例患儿(7.7%)经食管彩色超声心动图(TEE)诊断不适宜行封堵治疗,其中1例由于右心室流出道有异常肌束,2例由于边缘过短不适宜行封堵手术,而改为体外循环下心内直视手术。36例患儿随访6个月以上,经胸超声心动图显示无封堵器脱落、移位,无溶血和房室传导阻滞,人工瓣膜表面光滑,无异物。心脏明显缩小,心功能正常,无神经系统并发症发生。其中3例术后存在轻微的残余分流,随访6个月均消失。 结论心脏搏动下经右心室微创封堵肌部室间隔缺损是肌部室间隔缺损一种较优的治疗选择,在经食管彩色超声心动图引导下手术是镶嵌治疗成功的关键,超声科医生与外科医生的合作是手术成功的基础。

    The interaction between medical instrument and target tissue during the surgery occurs in instrument-tissue interface. The reliability research on the interface is directly related to the safety and effectiveness of medical instrument in the clinical application. This paper illustrates the necessity of reliability research on instrument-tissue interface. Two main contents are synthetically reviewed the present paper: (1) reliability research on medical instruments; (2) biological tissue properties and its mechanical response.

  • Influence of Left Atrial Contraction on Lone Atrial Fibrillation Recurrence after Minimally Invasive Radiofrequency Ablation

    ObjectiveTo investigate influence of left atrial contraction on lone atrial fibrillation recurrence after minimally invasive radiofrequency ablation. MethodsClinical data of 57 patients with lone atrial fibrillation underwent minimally invasive radiofrequency ablation in Department of Cardiothoracic Surgery, Xinhua Hospital, Medical School of Shanghai Jiaotong University from September 2010 to December 2011 were retrospectively analyzed. According to the absence of mitral A velocity, patients were divided into Group A (absence of mitral A velocity, 20 patients with their age of 56.32±17.18 years, including 5 females) and Group B (mitral A velocity exists, 37 patients with their age of 60.33±11.22 years, including 17 females). Minimally invasive radiofrequency ablation via thoracoscope were performed in all patients. Preoperative and postoperative left atrial diameter (LAD), left ventricular ejection fraction (LVEF) and mitral A velocity, as well as clinical and follow-up data were recorded and compared. ResultsPreoperative clinical characters were not statistically different between two groups (P > 0.05). All the patients were followed up for 24.3±8.8 months (range, 12-26 months). Rate of postoperative atrial fibrillation recurrence in group A was significantly higher than that in group B (20.0% vs. 2.7%, P < 0.05). LAD and LVEF of 57 patients at 6 months after surgery were significantly higher than preoperative LAD and LVEF (P < 0.05), but there was no statistic difference between two groups (P > 0.05). ConclusionDamage of left atrial contraction was related to lone atrial fibrillation recurrence after minimally invasive radiofrequency ablation. Absence of mitral A velocity could be a crucial predictor of postoperative lone atrial fibrillation recurrence.

  • Application of Digital Technology Assisted Minimally Invasive Surgery in Diagnosis and Treatment of Hepatolithiasis

    ObjectiveTo study the clinical value of digital technology assisted minimally invasive surgery in diagnosis and treatment of hepatolithiasis. MethodsThe image data of 64-slice spiral CT scanning were obtained from five patients of complicated hepatolithiasis and introduced into medical image three-dimensional visualization system (MI-3DVS) for three-dimensional reconstruction. On the basis of the data of three-dimensional reconstruction, minimally invasive surgical planning of preoperation was made to obtain reasonable hepatectomy and cholangiojejunostomy, and then preoperative emulational surgery was carried out to minimize the extent of tissue damage and provide guidance to actual operation. ResultsLiver, biliary system, stone, blood vessel, and epigastric visceral organ were successfully reconstructed by MI-3DVS, which showed clearly size, number, shape, and space distribution of stone, and location, degree, length, and space distribution of biliary stricture, and anatomical relationship of ducts and vessels. The results of three-dimensional reconstruction were successfully confirmed by actual operation, which was in accordance with emulational surgery. There was no operative complication. No retained stone in internal and external bile duct was found by Ttube or other supporting tube cholangiography on one month after operation. ConclusionThree-dimensional digitizing reconstruction and individual emulational surgery have important significance in diagnosis and treatment of complicated hepatolithiasis by minimally invasive technique.

  • Evolution of Minimally Invasive Surgery Accompanied by Controversies on What is “MiniAggression”

    近年来,在国内重要学术会议上无不谈及微创外科(技术)的问题,其中最具代表性的是中国工程院于2001年10月在北京举办的工程科技论坛“微创外科新概念”和2002年10月“中国微创外科杂志”创刊一周年大会对微创概念及相关领域技术的研讨。虽然对Minimally Invasive Procedure, Minimally Invasive Surgery, Minimal Access Surgery的中文译名和微创外科(技术)是否可以形成一个新的学科或分支学科存在着争议[1~6],但这样的研讨却促进了我国微创外科的快速发展。现就微创概念和微创外科技术在重要领域的进展作一简单介绍。
