  • Thoughts and suggestions on the definition of hypoparathyroidism after thyroidectomy

    Hypothyroidism is one of the focuses of attention in the field of thyroid surgery, but postoperative hypoparathyroidism has always been an unavoidable complication for surgeons. Currently, there is still controversy over the definition and classification of postoperative hypoparathyroidism, and there are significant differences in the definition and incidence of permanent hypoparathyroidism among different studies. The author provides preliminary suggestions and ideas for the diagnosis and definition of hypoparathyroidism based on relevant guidelines, literature, and clinical experience, in order to provide more accurate diagnosis and treatment plans for postoperative hypoparathyroidism.

  • Research progress of recognition of parathyroid gland anatomy and its clinical application

    Objective To understand anatomy of parathyroid gland and explore its application value in protection of parathyroid gland function during thyroidectomy. Methods The literatures, which were associated with the parathyroid anatomy and hypoparathyroidism were collected. The origin, function, anatomical location, number, blood supply, lymphatic system of the parathyroid gland and its relationship with surrounding tissues of parathyroid gland and its clinical significance in the thyroidectomy, were reviewed. Results The position of the superior parathyroid gland was relatively constant, and the inferior parathyroid gland was more likely to be ectopic. The number of the parathyroid gland was uncertain. The mainstream view was that the arterial supply of the parathyroid glands was mainly ensured by the inferior thyroid artery, a few by anastomosis of the superior and inferior thyroid arteries, or by the superior thyroid artery. However, the alternative view was that the blood supply of the parathyroid gland was not mainly derived from the inferior thyroid artery. The parathyroid gland was not easily distinguished from the adipose tissue and lymph node. Whether there was an independent lymphatic system in the parathyroid gland was still controversial. In the thyroidectomy, the parathyroid gland and its blood supply were reserved or protected by distinguishing from the Zuckerkandl tubercle, recurrent laryngeal nerve, and parathyroid specific attachment fat, which were identified by utilizing of the nanocarbon, loupe magnification, etc.. Especially in the central lymph neck dissection, the main thyroid artery trunk and its important branches should be carefully dissected or retained through the gentle capsular dissection and the correct use of energy devices for vessel sealing. The parathyroid gland in situ was reserved according to the parathyroid type. If it was not possible to be preserved, the parathyroid autotransplantation was necessary during the thyroidectomy. Conclusions Understanding origin and location of parathyroid gland, it could provide a direction for searching parathyroid gland during thyroidectomy. Being familiar with blood supply of parathyroid gland makes it possible to protect blood vessel and preserve parathyroid gland. Gentle capsular dissection, rational use of energy device, and indocyanine green angiography seem to be more important. Number of parathyroid gland allows us to treat each parathyroid gland as the last one, if it is not preserved in situ , parathyroid gland need to be autografted to avoid hypoparathyroidism.

  • Application of parathyroid typing in evaluating the degree of difficulty of in situ preservation of parathyroid gland during thyroid surgery

    Objective To evaluate whether the classification of parathyroid can be used to evaluate how difficult it is that the parathyroid glands get preserved in situ during thyroid surgery. Methods Clinical date were retrospectively collected from the patients with thyroid nodules, who had undergone the initial thyroidectomy in the Department of Thyroid Surgery, West China Hospital of Sichuan University between January 2014 and June 2016. The number of parathyroid glands was counted according to the classification of parathyroid. It got comparative analysis that the rates of parathyroid glands in situ among the different types. Results A total of 996 patients were included in the study, and 3 269 pieces of parathyroid glands were identified. The mean number of parathyroid identification was 3.3 pieces. These parathyroid glands consisted of 77.5% (2 532/3 269) type A and 22.5% (737/3 269) type B. The rate of parathyroid glands in situ was 77.1% (1 951/2 532) in type A, and 80.7% (595/737) in type B, the difference was significant (P=0.03). And the rate of parathyroid glands in situ in type A1 was significantly higher than that in type A2 (80.5%vs 21.4%,P<0.001). The parathyroid of type A3 couldn’t get preserved in situ. The rate of superior parathyroid glands in situ in type B1 was higher than that in type A1 (97.5%vs 93.7,P<0.01). But the rate of inferior parathyroid glands in type B1 was closed to that in type A1 (62.2%vs 65.7%,P=0.23), and both the rates were significant less than that in type B2 (86.0%) and in type B3 (90.2%),PA1vs B2=0.001,PA1vs B3<0.001,PB1vs B2=0.004,PB1vs B3=0.001. Conclusion The classification of parathyroid can be used to evaluate effectively how difficult it is that the parathyroid glands get preserved in situ during thyroid surgery.

  • Progress of fluorescence imaging in the study of parathyroid blood supply

    ObjectiveTo understand the methods of judging the blood supply of parathyroid during thyroidectomy at home and abroad in recent years. MethodThe literature on parathyroid blood supply was collected, the research progress was reviewed, and the advantages and disadvantages of related methods were analyzed. ResultsIn recent years, near-infrared fluorescence, laser speckle contrast imaging and other technologies had been applied. They showed better advantages as compared with naked eye observation. The research on parathyroid blood supply at home and abroad was still in its infancy, and more clinical samples and related equipment optimization were still needed. ConclusionFluorescence imaging technology has a certain auxiliary role in the judgment of intraoperative parathyroid blood supply and can reduce the incidence of hypoparathyroidism to a certain extent.

  • Diagnosis and Treatment of Asymptomatic Primary Hyperparathyroidism Duo to Parathyroid Adenoma(A Retrospective Analysis of 24 Cases)

    目的 探讨甲状旁腺腺瘤所致无症状原发性甲状旁腺功能亢进症(PHPT)的诊断和治疗。方法 回顾性分析中国医科大学附属第一医院1990年1月至2013年4月期间收治的24例甲状旁腺腺瘤所致无症状PHPT患者的临床资料。结果 24例甲状旁腺腺瘤所致无症状PHPT患者中男9例,女15例;年龄(47.92±12.96)岁。均无骨关节疼痛、病理性骨折、泌尿系结石或胃肠道症状等典型PHPT的临床表现,均为体检和并存或误诊为甲状腺疾病时发现。24例患者术前血钙升高18例,正常6例;血磷降低7例,正常17例;16例术前行血甲状旁腺激素检测患者中11例升高,正常5例;22例患者术前测定血碱性磷酸酶升高9例,正常13例。血钙与甲状旁腺激素均正常者4例,其中1例为体检时超声发现甲状旁腺病变,3例为手术治疗甲状腺疾病术中探查发现。24例患者术前均行超声检查,15例行甲状旁腺ECT检查,14例行甲状旁腺增强CT检查,阳性率分别为66.67% (16/24)、93.33% (14/15)及78.57% (11/14)。15例术前定性定位检查获得明确诊断,5例定性或定位检查高度怀疑甲状旁腺病变,4例术前误诊为甲状腺病变。均行甲状旁腺腺瘤切除,其中甲状旁腺腺瘤位于左侧上位2例、左侧下位9例、右侧上位2例、右侧下位11例。肿瘤最大径为(2.22±0.88) cm。24例患者中13例合并甲状腺病变。本组患者术后有8例出现手足、口周麻木,给予静脉或口服补钙后缓解,其余患者均无自觉症状。术后血钙及甲状旁腺激素均有不同程度下降,术后2周均降至正常范围。结论 甲状旁腺腺瘤所致无症状PHPT临床症状不典型,血清钙和甲状旁腺激素同步升高即可诊断,具有手术指征的患者应积极手术治疗。

  • Clinical Analysis of Non-Functioning Parathyroid Cyst of Six Cases

    目的探讨非功能性甲状旁腺囊肿的临床特点,总结其诊治经验。 方法回顾性分析6例非功能性甲状旁腺囊肿临床资料,结合该病的临床症状、辅助检查、随诊情况,讨论其诊断、治疗与预后。 结果6例患者均行手术治疗,术后病理确诊为非功能性甲状旁腺囊肿,术后均获得随访,随访时间6个月~5年,平均42个月,无复发。 结论非功能性甲状旁腺囊肿术前诊断困难,确诊依靠病理检查,手术切除为主要治疗手段。

  • Xenotransplantation of Microencapsulated Rabbit Parathyroid Tissue for Treatment of Hypoparathyroidism In Rats

    【Abstract】ObjectiveTo investigate the effect of xenotransplantation of microencapsulated rabbit parathyroid tissue in different sites in rats for the treatment of hypoparathyroidism. MethodsThe parathyroid glands from Wistar rats were removed to make them aparathyroid. Ultimately, sixteen rats were included because their serum calcium values were continuously below 1.6 mmol/L. We also encapsulated the cultured rabbit parathyroid tissue with alginateBaCl2 microcapsule. According to the transplantation sites, rats were randomly divided into two groups: renal adipose microcapsule group and peritoneal microcapsule group, eight in each group. Encapsulated rabbit parathyroid tissues were then transplanted accordingly to different microcapsule groups. The calcium serum contents were examined on 5,15,25,35,45,55 and 65 d respectively after transplantation and the grafts were observed through electron microscope on the 65 d in particular. ResultsThe calcium contents after transplantation in renal adipose microcapsule group restored to normal and the observation outcomes of grafts showed that they survived well. The calcium contents of posttransplantation in peritoneal group also restored to normal with an exception that it dropped to a level lower than 1.6 mmol/L on the 65 d. Electron microscope also showed that there were necrotic tissues in the center and only a few cells survived on the edge of the grafts. Within peritoneal microcapsule group, the values were significantly lower than others taken at different phases. ConclusionMicroencapsulated rabbit parathyroid tissue that was xenotransplanted into rats can survive and function without administration of immunodepressant. There are significant differences of calcium contents at varying phases between two transplantation sites, which demonstrate that renal adipose may be an optimal site for microcapsule xenotransplantation.

  • 甲状腺/甲状旁腺术后早期低钙血症诱发的清醒状态下喉痉挛2例报道

    目的总结甲状腺/甲状旁腺术后早期低钙血症诱发的清醒状态下喉痉挛的原因及防治措施。方法对2例甲状腺/甲状旁腺术后早期低钙血症诱发喉痉挛患者的临床资料进行回顾性分析。结果1例甲状腺右叶乳头状癌患者行甲状腺全切除 + 颈部右侧淋巴结清扫 + 右侧中央区淋巴结清扫,1例三发性甲状旁腺功能亢进症、左下甲状旁腺增生患者行颈部探查 + 左侧甲状旁腺全切除 + 术中喉返神经探查监测术。2例患者术后早期血清钙水平较术前迅速下降,患者清醒状态下出现喉痉挛。2例患者均予快速补充钙剂后,呼吸困难的临床症状迅速缓解。结论甲状腺/甲状旁腺切除术后可出现低钙血症,如血清钙水平迅速下降,甚至在清醒状态下诱发喉痉挛,及时纠正低钙血症后喉痉挛可迅速缓解,此类患者术后需要监测血清钙水平并预防性补钙。

  • Local administration of parathyroid hormone and parathyroid hormone-related peptides for bone tissue engineering

    Parathyroid hormone (PTH) exerts multiple effects such as regulating bone remodeling, promoting angiogenesis, etc., and it is an active factor with great application potential for bone repair. In recent years, with the development of scaffold material loading strategies and parathyroid hormone-related peptides (PTHrPs), in situ loading of PTH or PTHrPs on scaffold materials to promote bone defect healing gradually becomes possible. Based on the current status and challenges of intermittent PTH (iPTH) for bone tissue engineering, the review summarizes the in-situ application strategies of PTH and the construction of PTHrPs as well as current problems and further directions in this field, with a view to propel the clinical application of scaffold materials loaded with PTH or PTHrPs in situ.
