  • Investigation of Application of Laparoscopic Hiatus Reconstruction with Crurosoft Patch in Elderly Patients with Gastroesophageal Reflux Disease

    ObjectiveTo investigate the clinical effects of laparoscopic hiatus reconstruction with Bard Crurosoft patch associated with Nissen fundoplication in elderly patients with gastroesophageal reflux disease (GERD). MethodsFrom July 2006 to July 2009, 22 consecutive elderly patients (≥65 years) with GERD underwent laparoscopic hiatus reconstruction associated with Nissen fundoplication, 10 of them underwent laparoscopic Crurosoft patch hiatus reconstruction (hiatus diameter≥5 cm in 2 patients, lt;5 cm in 8 patients) and 12 underwent laparoscopic simple sutured hiatus reconstruction (hiatus diameter≥5 cm in 2 patients, lt;5 cm in 10 patients). Intra and perioperative data including symptoms (heartburn, regurgitation, dysphagia, and respiratory complications), functional evaluations (esophagogastroscopy, manometric evaluations in lower esophageal segment, and 24 h pH-monitoring values) were compared and analyzed. ResultsPatients in 2 groups had similar preoperative values in demographics, symptom scores, functional evaluations, as well as operative data except for mean operative time. Three-month and 1-year follow-up after operation, the results of symptoms scores and functional evaluations of patients in 2 groups compared with preoperative values wear improved (Plt;0.05), but symptoms scores and functional evaluations of patients in patch group were evaluated to demonstrate more significant improvement than suture group (Plt;0.05). In suture group, the results of 3 months after operation were better than 1 year after operation, with statistically significant difference (Plt;0.05). Two patients underwent postoperative intrathoracic immigration of wrap in suture group, but this complication did not happen in patch group (Plt;0.05). ConclusionsLaparoscopic hiatus reconstruction with Bard Crurosoft patch associated with Nissen fundoplication is a safe and effective treatment for elderly patients(≥65 years) with GERD.

  • The Effect of Judgment of Brain Center Hernia on the Prognosis of Cerebral Hemorrhage Patients

    【摘要】 目的 分析脑中心疝对脑出血患者预后的影响,明确早期判定的重要性。 方法 回顾分析2005年2月-2010年8月所有因脑出血行手术治疗的173例患者的临床资料,其中并发小脑幕切迹疝48例,并发脑中心疝37例。 结果 173例患者治愈97例,轻残25例,中残30例,持续植物状态2例,死亡19例;死亡原因:脑功能衰竭11例,颅内感染2例,肺功能衰竭2例,肾功能衰竭3例,弥散性血管内凝血1例。脑中心疝患者病死率(27.03%)高于非脑中心疝患者(6.62%),差异有统计学意义(χ2=10.393,P=0.001)。脑中心疝分期与GOS分级呈负相关关系(rs=-0.827,P=0.000),分期越早,GOS分级越高;脑中心疝存活的27例患者日常生活、活动分级与脑中心疝分期呈正相关关系(rs=0.630,P=0.000),分期越早,ADL分级越低。 结论 脑中心疝的早期判定可以减少患者并发症的发生,降低病死率及伤残率。【Abstract】 Objective To Analyze the effect of brain center hernia on the prognosis of cerebral hemorrhage patients, and clarify the importance of early judgment of brain center hernia. Methods The recorded data of 173 patients undergoing surgery for cerebral hemorrhage in our hospital from February 2005 to August 2010 were retrospectively analyzed. Among them, there were 48 cases of combined transtentorial herniation and 37 cases of combined brain center hernia. Results Among the 173 patients, 97 were cured, 25 were slightly disabled, 30 were moderately disabled, 2 were in persistent vegetative state (PVS), and 19 died. In the 19 dead patients, 11 died of brain function failure, 2 of intracranial infection, 2 of lung failure, 3 of renal failure, and 1 of disseminated intravascular coagulation (DIC). The mortality of patients with brain center hernia (27.03%) was significantly higher than that of non-brain center hernia patients (6.62%) (χ2=10.393, P=0.001). The period of brain center hernia was negatively correlated with GOS′s stage (rs=-0.827, P=0.000), and the earlier the period, the higher the GOS stage. The brain center hernia period in the 27 survival patients was positively correlated with their ADL stage (rs=0.630, P=0.000), and the earlier the period, the lower the ADL stage. Conclusion Early judgement of brain center hernia can reduce patients′ complications, their mortality and disability rate.

  • Application of Ultrapro Mesh in Laparoscopic Repair for Inguinal Hernias

    目的 探讨Ultrapro轻量型网片在腹腔镜腹股沟疝修补术中的优点和应用价值。方法 回顾性总结分析我院2008年8月至2010年10月期间,腹腔镜腹股沟疝修补术中应用Ultrapro轻量型网片的28例患者的临床资料。结果 28例患者共行疝修补术36例次,其中实施经腹腹膜前腹腔镜疝修补术(TAPP) 9例次,腹腔镜全腹膜外疝修补术(TEP)27例次;1例TEP中转行TAPP。平均手术时间为(103.4±39.8) min,术中平均出血量为(7.7±4.9) ml,术后平均住院时间(4.8±1.4) d。术后修补区暂时性神经异常1例,阴囊积液2例,尿潴留1例。28例均随访(15±1.6)个月(1~25个月),期间未观察到网片感染、疝复发、慢性疼痛、异物感或睾丸萎缩等并发症。结论  Ultrapro轻量型网片在腹腔镜腹股沟疝修补术中应用方便、视野清晰、柔软、硬度适中、放置定位容易,术后患者并发症少、恢复快,具有较好的临床应用价值。

  • The Application of Plug Mesh TensionFree Hernioplasty of Abdominal External Hernia (Report of 58 Cases)

    目的探讨疝环充填式无张力疝修补术在腹外疝修补术中的价值。方法回顾性总结1999年5月至2002年10月我院应用疝环充填式无张力疝修补术治疗腹外疝患者58例,共62例次。其中合并高血压、心肺功能不全、前列腺肥大、糖尿病者占39.7%(23/58)。 结果切口均为一期愈合。术后3~7 d出院。随访1~29个月,平均18个月,随访率为89.7%(52/58),无一例复发。结论疝环充填式无张力疝修补术具有创伤小、符合解剖生理、痛苦少、恢复快及复发率低等优点。

  • Experience of Tension-Free Hernioplasty for Inguinal Hernia (Report of 126 Cases)

    目的 总结腹股沟疝无张力疝修补术的手术经验。方法 回顾性分析我院2002年2月至2009年1月期间126例腹股沟疝患者行无张力疝修补术的临床资料及手术方法。结果 本组126例患者无围手术期死亡者,并发症主要有局部肿胀、异物感、疼痛等,随访6个月~6年(平均38个月)有6例(4.8%)复发。结论 无张力疝修补术是治疗腹股沟疝较为理想的术式。

  • Design and Fabrication of a Device for Intraperitoneal Treatment of Pediatric Hernia

    For treatment of pediatric inguinal hernia, we fabricated a device, i.e. so called "filling type pediatric hernia sac", which treats the problem from the abdominal cavity, through the abdominal and is a self-adaptive closer, using synthetic material. The device includes filling rack, self-adaptive umbrella support bar, bottom piece, outside pulling line and device fixing lines. The filling rack is composed of 2 concentric circles of 3.0 cm diameter with peripherally fixed together and can be pulled into the shapes of a ball or an olive. The supporting bar is structured of 3 pieces with 0.5 cm wide, 4.0 cm long, cross-fixed on top of the filling rack. The bottom piece is in a circular structure with a diameter of 3.0 cm, and it is connected to the filling rack bottom. Adjust positioning stay outside the fixed on the top of the device are connected at one end, and the other end free through filling the top frame connected with the bottom slice of central fixation. By using this device, we treated 37 pediatric inguinal hernia cases with 38 side-inguinal hernia successfully. The mean duration of post-operation follow-ups was 14.6±5.89 months, without hernia recurrence, obvious scar and hard sections of inguinal region. This device could provide a convenient, safe and effective plugging technology for children's pediatric hernia.

  • ADVANCES IN CLINICAL APPLICATION OF HERNIOPLASTY BY HIGH MOLECULAR MATERIAL

    Objective To study the advances in clinical application of hernioplasty by high molecular material. Methods The literature in the recent years on the advances of hernioplasty by high molecular material was reviewed. Results At present time many operative techniques of hernioplasty by high molecular material have been developed. The representative techniques were ①Rives-Stoppa′s mesh inlay hernioplasty; ②Lichtenstein′s tesion-free herniorrhaphy; ③mesh plug hernioplasty; ④Gilbert′s sutureless hernioplasty; ⑤laparoscopic inguinal hernioplasty. The reparing high molecular material was divided into absorbable and unabsorbable material, the former included polyglycolic-acid and polyglaction, the later consists of polypropylene polyester and expanded polytetrafluoroethylene.Conclusion The clinical application of henioplasty by high molecular material is increasing. According to the hernia type and patient condition, excellent outcome will be achieved by the application of proper repairing method and repair material.

  • Application of Dexmedetomidine in Combined Spinal Epidural Anesthesia for Laparoscopic Total Extraperitoneal Hernia Repair

    ObjectiveTo observe the effects and security of dexmedetomidine in combined spinal epidural anesthesia (CSEA) for laparoscopic total extraperitoneal hernia repair (TEP). MethodsFrom January 2010 to January 2013, we selected 90 patients who were going to receive TEP surgery as our study subjects. The patients were divided into three groups:M1, M2 and M3 with 30 patients in each. The patients had left lateral position, and anesthesia was done between 3-4 lumbar epidural line. Injection of 0.5% bupivacaine 2 mL was carried out, and epidural catheter was 3-5 cm. Anesthesia plane was adjusted from the chest 4 or 6 vertebra to the sacral vertebra. The three groups of patients were treated with micro pump using dexmedetomidine given at a pre-charge of 0.5 μg/kg, and then group M1 was maintained by 0.3 μg/(kg·h), M2 by 0.5 μg/(kg·h), and M3 by 0.7 μg/(kg·h). The changes of mean arterial pressure (MAP), heart rate, respiration and pulse oximetry (SpO2) were observed at each time point, and bispectral index (BIS) monitor and Ramsay sedation score test were also conducted. ResultsThe changes of MAP, heart rate and respiration in group M1 were not obvious; the Ramsay score for group M1 was 2 to 3, and BIS value after pre-charge was 65-84. For group M2, MAP, heart rate and respiration had a slight decline; Ramsay score was 3-5 points, and BIS value was 60-79. In group M3, patients had a milder decline in their MAP and respiration; the heart rate declined obviously after receiving dexmedetomidine and one patient with severe decline of the heart rate alleviated after active treatment; Ramsay score was 5 to 6 points, and BIS value was between 55 and 75. There was little change in SpO2 in all the three groups, and the difference was not statistically significant (P>0.05). ConclusionContinuous injection of dexmedetomidine at 0.3-0.5 μg/(kg·h) in CSEA is an alternative way for anesthesia, which can effectively promote sedation and reduce pain and discomfort.
