  • The Role of Hydrogen Sulfide in Prophase of Acute Peritoneal Cavity Infection of Rat

    Objective To study the role of hydrogen sulfide (H2S) in prophase of acute peritoneal cavity infection. Methods NaHS was taken as a donor of H2S. Seventy-two Sprague-Dawley rats were divided into 4 groups randomly:control group, cecal ligation and puncture (CLP) and treated with natural saline group,CLP and treated with NAHS group, and CLP and treated with DL-propargylglycine (PAG, an inhibitor of H2S formation) group. Selected 6 rats at 2h, 6h, and 12h after treatment in each group. The contents of TNF-αand H2S in serum and the content of MPO in intestinal tissue were measured, respectively. The histopathological change of ileum tissues were observed at 6 h after treatment in each group. Results The H2S could alleviate CLP-induced inflammation obviously, decrease the content of TNF-α in serum when inflammation,and attenuate the infiltration of neutrophilic granulocyte in small intestine. Conclusion The H2S has anti-inflammation effect in prophase of acute peritoneal cavity infection.

  • Standard Operating Procedure for Drug Labeling and Dynamic Medication Management of Double-blinded Randomized Controlled Trial

    Dynamic medication management can help to improve efficacy and save consumption in such trials when an investigational medication is expensive or in short supply or has a fairly short shelf-life in a long-lasting clinical trial, or, if protocol changes during study period. We presented the standard operating procedure of dynamic medication management and drug labeling through an instance of double-blind randomized controlled trial of Chinese Medicine for hepatopathy. Compared to conventional drug labeling, dynamic medication management and drug labeling is more flexibility to deal with problems during the study period, such as protocol update, medication waste, and outdated medication and so on. Appropriate method and reasonable procedure can ensure the precise implementation for drug labeling and medication management of double-blinded randomized controlled trials.

  • Standardization and surgical modification of sleeve gastrectomy with jejunojejunal bypass

    ObjectiveTo analyze why sleeve gastrectomy (SG) with jejunojejunal bypass (SG-JJB), despite being the second most common bariatric procedure in China, has not been recommended in national and international guidelines nor endorsed by expert consensus; to investigate the primary obstacles to its standardization and widespread adoption; and to propose strategies leveraging China’s extensive clinical experience to refine the technique, establish standardized protocols, and address existing challenges, thereby defining its future role in metabolic surgery. MethodsBy systematically reviewing the evolution, current evidence profile, and distinctive features of SG-JJB compared to other SG-Plus procedures, this study aimed to identify constraints hindering its adoption. Concurrently, considering the characteristics of domestic healthcare resources, we explored the feasibility of procedural refinements, key steps for standardization, and solutions to potential challenges, thereby facilitating the optimization and standardization of SG-JJB. ResultsThe three key constraints hindering SG-JJB development were: risks of blind loop syndrome, uncertainty regarding optimal bypass limb length, and limited evidence on long-term efficacy. To address these issues, this study proposed leveraging China’s clinical and multi-center collaboration strengths to: conduct high-quality studies defining the impact of bypass length on outcomes, establish unified diagnostic and monitoring protocols for blind loop syndrome, and systematically collect longitudinal data to evaluate long-term efficacy, thereby informing evidence-based surgical standardization. ConclusionsSG-JJB holds significant potential in Chinese bariatric-metabolic practice, yet its standardization faces persistent challenges. Addressing concerns about blind loop syndrome, defining optimal bypass limb length, and accumulating robust long-term efficacy data are pivotal for advancing SG-JJB standardization and adoption. Leveraging domestic clinical resources through multi-center collaborations, high-quality research, and evidence-based protocol development is the essential pathway to overcoming these barriers, achieving standardized implementation, and securing recognition in authoritative guidelines.

  • Early Effect Following the Resection of IlealCecum and Right HemicolonAn Experimental Study

    Objective To explore the early effect following resection of ilealcecum and right hemicolon. MethodsTwenty piglets were randomly divided into 2 groups, 10 in control group and experimental group respectively. The ilealcecum and right hemicolon for 50 cm each were resected in the experimental group. The two groups were fed the same food before and after the operation. Some indexes of two groups were monitored. Results①In the two groups, the body length change wasn’t significant (Pgt;0.05); but the body weight had descended markly in experimental group, compared with control group, since 3 weeks after operation (Plt;0.05). ②In experimental group, the ileal bacteria were increased from 104-107/ml before operation to 109-1011/ml at 6 weeks after operation (Plt;0.05). ③The enterohepatic circulation of biliac acid was affected during the early phase after resection. ④In experimental group, the stool became diluted within 6 weeks after operation. ⑤In the two groups, the Vitamin B12 was waved slightly within 6 weeks after operation (Pgt;0.05). The megaloblastic anemia was not appeared. ⑥The function of liver and the absorption of Na+, Cl-,K+ did not change significantly (Pgt;0.05). Conclusion The ileocecum and right hemicolon are very important for digestion and absorption, it should be retained or the ileocecal valve should be reconstructed if possible.

  • Magnetic resonance diffusion tensor imaging for optic nerves and optic radiation in blind patients

      Objective To observe the characteristics of magnetic resonance diffusion tensor imaging(MR-DTI)for optic nerves and optic radiation in blind patients.Methods The optic nerves and optic radiation of 20 blind patients(blind group)and 20 controls(control group) were scanned by MR-DTI. Fractional anisotropy (FA) and directional encoded color (DEC) maps were acquired through postprocessing with the aid of volumeone 1.72 software. The signal intensity of optic nerves and optic radiation were then observed. The FA, mean diffusivity (MD), lambda;∥ and lambda;perp; value of bilateral optic nerves and optic radiation in two groups were measured in the DEC maps.Results While the high signal intensity was found in bilateral optic nerves in FA and DEC maps in control group,the signal decreased markedly in the blind group. The FA and lambda;∥ value of optic nerves in the blind group were declined obviously compared to that in the control group. The difference was statistically significant (t=16.294, 14.660;P=0.000). The MD and lambda;perp; value of optic nerves in the blind group were increased obviously compared to that in the control group, the difference was also statistically significant (t=8.096, 8.538; P=0.000). The high signal intensity was found in bilateral optic radiation in FA and DEC maps in both the blind and control groups. There were no statistic differences in FA and MD value in bilateral optic radiation between the blind and control groups (Left:t=1.456,1.811;P=0.152,0.076. Right:t=0.779,0.073;P=0.440,0.942). Conclusion A low signal intensity of bilateral optic nerves and a high signal intensity of bilateral optic radiation were found in blind patients.
