  • PylorusPreserving Pancreatoduodenectomy (Report of 12 Cases)

    目的探讨保留幽门的胰十二指肠切除术(PPPD)的手术适应证、捆绑式胰肠吻合术的特点及胃排空障碍的治疗。方法12例均行PPPD,消化道重建按Child法,胰肠吻合均采用捆绑式胰肠吻合。结果除1例术后第21天并发切口裂开、肺栓塞死亡外,其余11例均顺利出院,平均住院时间24 d,随访1年,1例术后8个月死亡,余均健在。结论PPPD是治疗胰头癌及壶腹周围癌的一种具有广阔应用前景的术式,捆绑式胰肠吻合有望作为胰肠吻合的标准术式。

  • A Meta-Analysis of Pancreaticojejunostomy on Postoperative Outcomes Following Pan-creaticoduodenectomy

    ObjectiveTo evaluate the effects of duct-to-mucosa pancreaticojejunostomy (dmPJ) and invagination pancreaticojejunostomy (iPJ) during pancreaticoduodenectomy (PD) on postoperative outcomes. MethodsPubmed, The Cochrane Library, Embase, Wanfang and CNKI database were searched to identify randomized controlled trials (RCTs) evaluating different type of pancreaticojejunostomy during PD. The literatures were screened according to inclusion and exclusion criteria. Quality assessment was conducted according to Jadad scoring system. ResultsNine RCTs were included, 1 032 patients were recruited, including 510 patients in dmPJ group and 522 patients in iPJ group. Meta-analysis indicated that there were no significant differences between two groups in terms of the incidence of pancreatic fistula in total (OR=0.95, P=0.78), clinical relevant pancreatic fistula (OR=0.78, P=0.71), overall morbidity (OR=0.93, P=0.60), perioperative mortality (OR=0.86, P=0.71), reoperation rate (OR=1.18, P=0.59), and length of hospital stay (WMD=-1.11, P=0.19). ConclusionDmPJ and iPJ are comparable in terms of pancreatic fistula and other complications.

  • 单层连续环形胰肠吻合对胰十二指肠切除术后胰瘘的影响

    目的 总结单层连续环形胰肠吻合对胰十二指肠切除术后胰瘘发生的影响。 方法 回顾性分析 2015 年 6 月至 2017 年 6 月期间在笔者所在医院行胰十二指肠切除术的 55 例患者的临床资料。 结果 所有患者均顺利完成单层连续环形胰肠吻合,无围手术期死亡。手术时间为 195~305 min、(231.46±18.69)min;术中出血量为 20~550 mL、(186.30±33.99)mL;胰肠吻合时间为 7~11 min、(9.31±2.43)min;住院时间为 14~37 d、(24.02±2.06)d。术后发生胰瘘 7 例,其中生化瘘 4 例,B 级胰瘘 3 例;发生腹腔感染 4 例,肺炎 5 例,胆汁漏 1 例,切口感染 2 例,无术后腹腔内出血发生。本组无开腹再手术患者,所有患者均顺利出院。术后所有患者均获访 90 d,随访期间均存活,发生胃排空障碍 1 例,碱性反流性胃炎 2 例。 结论 单层连续环形胰肠吻合的操作简单及安全,有一定的临床可行性。

  • Diagnosis and Treatment for Iatrogenic Injury of Cholodocho-Pancreatico-Duodenal Junction

    Objective To summarize the experiences in diagnosis and treatment for iatrogenic injury of cholodocho-pancreatico-duodenal junction (CPDJ). Method The clinical data of 9 patients with CPDJ injury were analyzed retro-spectively. Results Double lumen tube drainage was emplaced in all the operations. Nutrition support and inhibitor ofpancreatic secretion were applied in all the patients. ①CPDJ injuries were observed intraoperatively for 6 out of 9 patientswith CPDJ injury. Two patients with simple perforation of posterior wall of common bile duct inferior segment and 2patients with simple duodenal perforation who were treated by perforation repair, common bile duct T tube drainage, andperitoneal drainage. Two cases of bile duct or duodenal injury with pancreatic injury were treated by choledochojejunostomyand BillrothⅡgastrojejunostomy. These 6 patients with CPDJ injury were cured and none of case was death. ②Injuries of CPDJ were observed postoperatively for 3 out of 9 patients with CPDJ injury. Total bile diversion or total bile diversion with duodenal diverticulum were performed in 2 cases of bile duct or duodenal injury with pancreatic injury, respectively. The one with total bile diversion only was died of abdominal infection, duodenal fistula, wound dehiscence, abdominal and upper gastrointestinal tract bleeding, and multiple organ failure;the one with total bile diversion with duodenal diverticulum and the one with sequent PTCD and choledochojejunostomy for biliary obstruction caused by operation ofgigantic duodenal interstitialoma were cured. Conclusions CPDJ injury usually occurs in biliary duct exploration, endoscopic sphincterectomy or duodenal operation. It is easily misdiagnosed and serious consequences may happen from this. Timely and correct treatment should be taken according to the specific injury observed during or after the operation.

  • Comparative study on anastomotic fistula of modified triple-layer duct-to-mucosa pancreaticojejunostomy and end-to-end invagination pancreaticojejunostomy following pancreaticoduodenectomy

    Objective To compare anastomotic fistula of modified triple-layer duct-to-mucosa pancreaticojejunostomy and end-to-end invagination pancreaticojejunostomy following pancreaticoduodenectomy. Methods The clinical data of 147 patients underwent pancreaticoduodenectomy from January 2015 to June 2017 in the West China Hospital of Sichuan University were retrospectively analyzed. The modified triple-layer duct-to-mucosa pancreaticojejunostomy were used in 101 cases (MTL group) and end-to-end invagination pancreaticojejunostomy were used in 46 cases (IPJ group). The differences of intraoperative and postoperative statuses were compared between the two groups. Results The baseline data of these two groups had no significant differences (P>0.05). Except for the average time of the pancreaticoenterostomy of the MTL group was significantly longer than that of the IPJ group (P<0.05), the intraoperative blood loss, the first postoperative exhaust time, postoperative hospitalization time, reoperation rate, death rate, and rates of complications such as the pancreatic fistula, biliary fistula, anastomotic bleeding, gastric emptying disorder, and intraperitoneal infection had no significant differences between these two groups (P>0.05). Conclusions Both modified triple-layer duct-to-mucosa pancreaticojejunostomy and end-to-end invagination pancreaticojejunostomy following pancreaticoduodenectomy are safe and effective. An individualized selection should be adopted according to specific situation of patient.

  • Modified Method of Binding Pancreaticoenterostomy ( Report of 8 Cases)

    目的 探讨捆绑式胰肠吻合术的改良方法。方法 对8例胰十二指肠切除术患者行捆绑式胰肠吻合术时采用带血管蒂的大网膜包绕空肠浆肌鞘,以达捆绑之目的。8例中行桥袢空肠造瘘5例,胆总管造口T管引流3例,并观察其术后5 d内每天平均引流液量。结果 8例患者均治愈出院,无胰瘘、胆瘘发生; 5 d内每天平均引流液量除第1天胆总管造口低于空肠造瘘外,其余4 d每天平均引流液量均明显高于空肠造瘘; 1例空肠造瘘者术后第8天继发胰腺残端出血,经电灼后止血。结论 带血管蒂的大网膜包绕空肠浆肌鞘完全可以代替粗线环绕空肠结扎,并使整个空肠浆肌鞘与胰腺紧贴,更利于胰肠间愈合; 空肠造瘘或胆总管造口对桥袢空肠均有减压引流的作用,但T管引流更简单、有效、省时。

  • Practice of Modified Triple-Layer Duct-to-Mucosa Pancreaticojejunostomy with Resection of Jejunal Serosa During Pancreaticoduodenectomy

    ObjectiveTo evaluate the postoperative complications after pancreaticoduodenectomy with modified triple-layer(MTL) duct-to-mucosa pancreaticojejunostomy and with resection of jejunal serosa, analyse the risk factors of pancreatic fistula, and compare effects with two-layer(TL) duct-to-mucosa pancreaticojejunostomy. MethodsData on 184 consecutive patients who underwent the two methods of pancreaticojejunostomy during standard PD between January 1, 2010 and January 31, 2013 were collected retrospectively. The risk factors of pancreatic fistula were investigated by using univariate and multivariate analyses. ResultsA total of 88 patients received TL and 96 underwent MTL. Rate of pancreatic fistula for the entire cohort was 8.2%(15/184). There were 11 fistulas(12.5%) in the TL group and four fistulas(4.2%) in the MTL group(P=0.039). Body mass index, pancreatic texture, pancreatic duct diameter, and methods of pancreaticojejunostomy had significant effects on the formation of pancreatic fistula on univariate analysis. Multivariate analysis showed that pancreatic duct diameter less than 3 mm and TL were the significant risk factors of pancreatic fistula. ConclusionsMTL technique effectively reduced the pancreatic fistula rate after PD in comparison with TL, especially in patients with pancreatic duct diameter less than 3 mm.

  • Research Progress in Application and Complications of Pancreaticogastrostomy

    ObjectiveTo summarize the application and the complications of pancreaticogastrostomy (PG) after pancreaticoduodenectomy(PD). MethodThe domestic and international publications involving the theory, methods, and clinical application of PG were retrieved and reviewed. ResultsPG was gradually concerned on the choice of the method of the digestive tract reconstruction after PD, in view of its advantages in theory and operation. The literatures about PG were increased in recent years. But the discussion of decreasing complications of PG after PD had yet to be unified. ConclusionsPG is one of the important operations of digestive tract reconstruction after PD. The factors of operator and patient should be comprehensively considered in the choice of PG.
