摘要:目的: 探讨临床护理安全规范化管理的有效方法和效果。 方法 :成立病房护理安全管理小组;完善护理安全管理制度,培养质量管理意识;改善重点环节工作流程,强化质量监控;构建护理安全文化氛围。 结果 : 患者满意度明显提高,用药错误、管道脱落、压疮、投诉等发生率明显降低(〖WTBX〗P lt;0005)。 结论 : 规范化的护理安全管理提高了护理质量,保障了患者的安全,有效降低了护理风险的发生。Abstract: Objective: To discuss a effective way and effect of standardardized management of clinical nursing safety.Methods :Setted up nursing safety management team; Improved the nursing safety management system and trained awareness of quality management; Improved workflow of key links,and strengthened the quality control; Built a nursing safety culture. Results : Patients satisfaction improved obviously, and the medication errors、 pipe off、pressure sores、the incidence of complaints such as decreased obviously(P lt;0005).Conclusion : Standardized management of nursing safety improved the nursing quality, protected patients safety, and effectively reduced the risk of the occurrence of nursing.
2006年7月我院开展了护士规范化培训。通过培训,学员掌握了丰富的理论知识,规范化和程序化的护理操作技能,综合素质得到全面提高。培训结业后学员胜任临床护士职务,独自完成护理工作,为病员提供全面护理,感悟到了护理的奉献精神;拥有较强的教学意识,积极参与临床教学活动,培养护理人才,体会到了护理的专业性;主动投入护理科研工作,促进护理学科发展,满足自我实现的需求,升华了自己的人生观和价值观。
ObjectiveTo summary the standard treatment for early gastric cancer. MethodsThe current early gastric cancer treatment guidelines around the world were analyzed and the standardized treatment patterns for early gastric cancer were concluded. ResultsThe accurate preoperative evaluation for early gastric cancer is the basis of standardized treatment which can be divided into staging evaluation and histological evaluation.The staging evaluation is focused on the gastric wall invasion and lymph node involvement of the tumor while the histologic evaluation emphasize the histological type and grading of the tumor.According to the precise evaluation for early gastric cancer, endoscopic surgery, laparoscopic surgery, open surgery, and multimodal therapy can be applied individually to the patients.Different treatment methods have their indications, but the indications of the therapies in different guidelines are suggested with slight differences. ConclusionIn clinical practice, the choice of treatment should be made with comprehensive consideration of diagnosis and individual characteristics of patients to achieve the most benefit on prognosis.
ObjectiveTo explore a standardized model for cardiothoracic surgery resident training program based on a combined clinical and research oriented team approach. Methods We conducted this study in a nationally ranked Class AAA hospital among the eleven residents who were accepted into the program. Throughout their training periods, clinical surgical skills were taught by one-on-one mentoring by individually assigned doctoral degree advisors, other attending surgeons and doctoral degree candidates in the program. To foster their scientific curiosity and research skills, regular didactic lectures in basic science and statistical skills by the doctoral thesis advisors, supplemented by monthly journal clubs during which trainees were required to present and discuss a previously assigned topics based on recent clinical cases within the department, thus combining acquisition of clinical skills and theory/research at the same setting. ResultsEleven physicians were selected for this training path. With respect to clinical surgical skills, 4 residents were judged to be sufficient at the end of the first cycle. The other seven residents at the end of the second cycle. One trainee won the third prize in the Medical Skill Competition in the hospital. As for scientific research skills, the team produced 11 scientific articles, one of which was accepted by the annual national conference in cardiothoracic surgery and the first author was invited to present the article. The team submitted four research projects, one won funding at the provincial competition and three within the medical school. ConclusionOur proposed standardized model of cardiothoracic surgery resident training based on a combined clinical and research oriented approach appears to be practical with moderate success. Adaptation of this model by other cardiothoracic surgery training programs in the nation will further attest to its value and functionality.
目的 依托四川大学华西医院的临床资源,建立了一个规范化、标准化的肿瘤组织标本库,对临床医学研究提供了珍贵的资源。 方法 于2009年6月创建四川大学华西医院肿瘤组织标本库、进行质量控制、规范化管理及总结实践经验。 结果 从创建肿瘤组织标本库至2012年2月,共收集各类肿瘤(胃、食管、肝、肺、肠、乳腺、胰腺、甲状腺、纵隔、肾脏、喉、脑)组织标本44 609例,包括肿瘤组织、肿瘤旁组织(距离肿瘤组织2 cm的组织)和切缘远端非肿瘤组织(距离肿瘤组织>5 cm的组织)。对肿瘤组织标本进行有效和科学的管理,为科研提供了便利和高质量的资源。 结论 建立了一个国际标准化、规范化、信息完善的肿瘤组织标本库,可为肿瘤的发生、发展、转移及生物标志物的研究提供高质量的标本。
目的 探讨急性重症胰腺炎不同时期的治疗方式,观察治疗效果以及治疗前后的临床表现。 方法 回顾性分析2006年1月-2009年1月收治的34例急性重症胰腺炎患者的临床资料,患者给予内科规范化治疗,并对比治疗前、后的APACHEⅡ评分、主要的实验室检查指标以及CT检查表现。 结果 31例治愈,2例中途转外科治疗,1例死亡。所有患者治疗后APACHEⅡ评分较治疗前减小(Plt;0.01),治疗后血清淀粉酶和白细胞计数较治疗前明显降低(Plt;0.05),而治疗后短期内CT检查表现无明显变化。 结论 内科规范化治疗能有效地治疗急性重症胰腺炎,适当的肠内外营养和预防性使用抗生素可有效地减少急性重症胰腺炎并发症的发生。APACHE-Ⅱ评分可作为判断急性重症胰腺炎患者预后的指标。
Objective To explore the mental health status and the relevant influencing factors of the resident standardized trainees, and to provide reference for the psychological intervention. Methods All the resident standardized trainees in a first class of the third grade hospital in Sichuan from July 2012 to August 2015 were investigated by the questionnaire including symptom checklist 90, demographic characteristics and work condition. Results The detection rate of psychological problem among resident standardized trainees was 24.7% which was higher than the general population. The analysis of logistic regression showed that the training grade, identity, work time and working achievement were the main factors related to psychological problems. Conclusions The psychological problems of resident standardized trainees were prominent because they are in a transformation stage from medical students to clinical doctors. The related department should pay more attention and take measures to improve the resident standardized trainees’ mental health.
As the intensity of clinical and research work is high, teaching is gradually paid less attention to and the quality of education cannot be ensured. In this context, a full-time teaching position is set up in West China Hospital which is taken responsibility by qualified clinicians, to improve the teaching quality by strict management and omni-directional teaching. We introduce the setting and running of the full-time teaching position in West China Hospital in this paper.
