  • Research progress on the influencing factors of chemotherapy-related cognitive impairment in lung cancer patients

    Chemotherapy-related cognitive impairment (CRCI) is one of the treatment-related side effects in cancer patients, which can reduce patients’ participation in medical decision-making and treatment, seriously affecting their daily function and quality of life. This article reviews the definition, research status, and influencing factors of CRCI in lung cancer patients, in order to provide basis and ideas for the subsequent evaluation and management of CRCI in lung cancer patients, and promote the optimization and improvement of the overall rehabilitation process of lung cancer patients.

  • Bi-modality Image Classification Based on Independent Component Analysis

    We in the present research proposed a classification method that applied infomax independent component analysis (ICA) to respectively extract single modality features of structural magnetic resonance imaging (sMRI) and positron emission tomography (PET). And then we combined these two features by using a method of weight combination. We found that the present method was able to improve the accurate diagnosis of Alzheimer's disease (AD) and mild cognitive impairment (MCI). Compared AD to healthy controls (HC): the study achieved a classification accuracy of 93.75%, with a sensitivity of 100% and a specificity of 87.64%. Compared MCI to HC: classification accuracy was 89.35%, with a sensitivity of 81.85% and a specificity of 99.36%. The experimental results showed that the bi-modality method performed better than the individual modality in comparison to classification accuracy.

  • An Etiological Study of Cognitive Impairment in the Institutional Elderly: A Meta-analysis

    目的 全面了解空巢老人认知功能障碍危险因素。 方法 检索Ovid Medline、Ovid Embase、Cochrane 图书馆及中国生物医学文献数据库(光盘版)等数据库,检索时间为各数据库建库至2011年12月31日。文献纳入标准:年龄≥60岁或作者指明以老人为研究对象;居住状态包括老人无子女或未与子女同住。两位研究者独立地对符合纳入标准的试验进行质量评价和资料提取。 结果 4篇文献(共1 485例)分析了养老机构老人认知障碍的影响因素,合并分析显示年龄[WMD=2.10岁,95%CI(1.28,2.92)岁,P<0.000 01]、女性[OR=3.61,95%CI(1.63,8.01),P=0.002]、失能[SMD=0.78,95%CI (0.36,1.20),P=0.000 2]、躯体健康[SMD=0.06,95%CI (0.01,0.11),P=0.02]、未婚/寡居[OR=3.16,95%CI(1.43,6.96),P<0.000 01]、文盲[OR=2.94,95%CI(1.06,8.14),P=0.04]、社会支持差[OR=7.29,95%CI(2.27,23.39),P=0.000 8]、朋友关系差[OR=2.14,95%CI(1.35,3.39),P=0.001]、参加活动少[OR=2.11,95%CI(1.36,3.29),P=0.000 9]及远离子女[OR=5.73,95%CI(3.33,9.87),P<0.05]是其危险因素。 结论 养老机构老人认知功能障碍的危险因素包括老年、女性、文盲、未婚/寡居、社交差、与子女距离远、躯体健康水平差及抑郁等。结果仅来源于较少的研究和数量较小的对象,且均为横断面研究,需谨慎看待上述结论。

  • 癫痫对患者认知影响的研究现状及进展

    癫痫病因较复杂,与患者发病年龄、情绪、心理因素以及癫痫类型等密切相关。认知障碍会导致患者幼年时期学习能力下降,成年后严重影响其日常生活及工作,对患者及家属的身心造成了严重伤害,同时带来较重负担,极大影响了患者的心理状态。近年来,随着癫痫患者认知障碍的发病率逐渐升高,二者共病逐渐引起了广泛关注,癫痫共患认知障碍发病机制尚不明确,但其造成的严重影响值得临床重视。文章从癫痫引发认知损害的可能机制、癫痫发病形式、社会因素以及生理因素、癫痫对患者认知功能的影响等方面进行简要综述,旨在探索其发生认知功能损害的原因,为提高患者的生活质量提供参考依据。

  • Multi-channel Synchronization Analysis of Mild Cognitive Impairment in Type 2 Diabetes Patients

    The cognitive impairment of type 2 diabetes patients caused by long-term metabolic disorders has been the current focus of attention. In order to find the related electroencephalogram (EEG) characteristics to the mild cognitive impairment (MCI) of diabetes patients, this study analyses the EEG synchronization with the method of multi-channel synchronization analysis--S estimator based on phase synchronization. The results showed that the S estimator values in each frequency band of diabetes patients with MCI were almost lower than that of control group. Especially, the S estimator values decreased significantly in the delta and alpha band, which indicated the EEG synchronization decrease. The MoCA scores and S value had a significant positive correlation in alpha band.

  • Effects of Ultrasound-guided Stellate Ganglion Block on Cerebral Oxygen Metabolism and Postoperative Cognitive Dysfunction in the Elderly

    To observe the effects of ultrasound-guided stellate ganglion block (SGB) on cerebral oxygen metabolism and postoperative cognitive dysfunction (POCD) of elderly patients, we collected 80 elderly patients undergoing selective coronary artery bypass graft under cardiopulmonary bypass. The Mini Mental State Examination (MMSE) was applied to test the cognitive function. The SjvO2, Da-jvO2 and CEO2 were used for the analysis of the cerebral oxygen metabolism. We found that POCD was related to disequilibrium of cerebral oxygen metabolism. Ultrasound-guided SGB before surgery reduced the incidence of POCD because of the improvement of cerebral oxygen metabolism.

  • Research of the application status of event-related potentials in epilepsy

    Event-related potentials (ERPs) are potential activities extracted from electroencephalogram (EEG) that are associated with specific stimuli. They possess the advantages of objectivity, ease of operation, and real-time reflection of cognitive processing in the brain. ERPs have been extensively utilized in studying pathophysiological mechanisms related to Alzheimer’s Disease (AD), Parkinson’s Disease (PD), stroke, schizophrenia, and other conditions. Epilepsy is a common neurological disorder wherein ERPs can be employed to explore the neuroelectrophysiological causes underlying cognitive impairment, anxiety, and depression in patients with epilepsy while providing an objective assessment. This article reviews the application of ERPs in patients with epilepsy.

  • Cognitive impairment rate of the elderly in China: a meta-analysis

    ObjectivesTo systematically review the current status of cognitive impairment of the elderly in China.MethodsCNKI, VIP, CBM, WanFang Data, PubMed, EMbase and The Cochrane Library databases were electronically searched to collect studies on the current status of cognitive impairment of the elderly in China from January 1st, 2000 to March 12th, 2020. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies. Then, meta-analysis was performed by using Stata 14.1 software.ResultsA total of 126 studies involving 187 115 elderly were included. The results of meta-analysis showed that the cognitive impairment rate of the elderly in China was 22.0% (95%CI 20.4% to 23.6%). Subgroup analysis showed that the cognitive impairment rate was higher in females, seniors, low education level, residing in rural area, engaging in manual labor, no spouse, living alone, monthly income less than 1 000 yuan, and suffering from chronic diseases.ConclusionsCurrent evidence shows that the cognitive impairment rate of the elderly in China is 22%, which is relatively high in females, seniors, low education level, residing in rural area, engaging in manual labor, no spouse, living alone, low-income, and suffering from chronic diseases.

  • Interpretation of European Stroke Organization (ESO) and European Academy of Neurology (EAN) Joint Guideline on Post-stroke Cognitive Impairment

    Post-stroke cognitive impairment (PSCI) is the most common dysfunction after stroke, which seriously affects patients’ quality of life and survival time. To strengthen the management and prevention of PSCI, the European Stroke Organization and the European Academy of Neurology jointly developed the guidelines for PSCI in 2021. This paper introduces the background, compilation method and structure, management suggestions and expert consensus of PSCI, the next research direction, etc. Compared with the current prevention and treatment measures of PSCI in China, it aims to provide methodological reference for Chinese scholars to develope PSCI guidelines and reference evidence for clinical prevention and treatment of PSCI.

  • 癫痫和认知障碍双向关系的研究进展

    认知障碍在癫痫中很常见,常常继发于癫痫或由癫痫引起。目前普遍认同的观点是癫痫发作损害大脑并导致脑功能退化和行为改变。最近,癫痫与认知之间是否存在双向关系引起了人们的重视,分清什么是疾病和什么是症状最为重要。随着对癫痫病因学认识的逐步深入,癫痫发作、认知障碍和行为问题可能都是潜在病理状态下所表现出来的各种症状。癫痫和行为异常之间存在功能性关联,即癫痫活动可影响行为,行为异常又可改变癫痫活动。总之,以癫痫为中心看待其引起的行为问题的单向思维已经过时,这种观点甚至可能妨碍寻找和治疗潜在的病因。另外,对于癫痫合并症也应该引起临床医生的重视和治疗。
