  • Application of exercise therapy in rehabilitation of maintenance hemodialysis patients

    Most patients with end-stage renal disease choose maintenance hemodialysis to prolong survival. The clinical application of exercise therapy has a definite effect on maintenance hemodialysis patients, and can effectively improve their quality of life and promote rehabilitation. Individualized exercise therapy under the guidance of medical professionals has positive effects on patients’ physical and mental rehabilitation. This paper mainly summarizes the status of exercise, factors affecting exercise, exercise therapy, exercise and rehabilitation of maintenance hemodialysis patients, and reviews the impact of exercise therapy on the physical and mental health of maintenance hemodialysis patients, in order to provide some references for clinical intervention and prognosis studies.

  • Efficacy of different exercise type on inflammatory cytokines in individuals with overweight or obesity: a network meta-analysis

    Objective To systematically review the efficacy of different exercises on inflammatory cytokines in individuals with overweight or obesity. Methods The CNKI, WanFang Data, VIP, PubMed, EBSCO, Cochrane Library, Web of Science and Embase databases were electronically searched to collect randomized controlled trials (RCTs) on the efficacy of exercise on inflammatory cytokines in individuals with overweight or obesity from January, 2000 to April, 2021. Two reviewers independently screened the literature, extracted data, and assessed the risk of bias of the included studies. The network meta-analysis was then performed using Stata 16.0 software. Results A total of 63 RCTs were included, 49 of which reported the changes in IL-6, 47 of which reported the changes in TNF-α, and 16 of which reported the changes in IL-10. The results of the network meta-analysis found that compared with those in the control group, aerobic exercise (AE) (SMD=−0.9, 95%CI −1.4 to −0.5, P<0.01) and high-intensity interval training (HIIT) (SMD=−1.3, 95%CI −2.3 to −0.3, P=0.011) significantly reduced IL-6. AE (SMD=−1.3, 95%CI −1.7 to −0.9, P<0.01), combined exercise (COM) (SMD=−0.7, 95%CI −1.3 to −0.1, P=0.02), and HIIT (SMD=−1.8, 95%CI −2.6 to −0.9, P<0.01) significantly reduced TNF-α; AE (SMD=0.8, 95%CI 0.1 to 1.5, P=0.03) significantly increased IL-10. The cumulative probability ranking results showed that HIIT was the most effective in reducing IL-6 and TNF-α and increasing IL-10, followed by AE and COM, and resistance training (RT) was the least effective. Conclusion Different exercise types have different effects on improving inflammation in individuals with overweight or obesity. HIIT can be suggested as the best exercise program to improve chronic inflammation in individuals with overweight or obesity. Due to the limited quantity and quality of the included studies, more high-quality studies are needed to verify the above conclusion.

  • Clinical pathway for exercise prescription in patients with hypertension, diabetes and dyslipidemia

    Exercise prescription is an effective tool for the prevention and control of hypertension, diabetes and dyslipidemia. However, a full set of exercise prescription is difficult to be implemented in China's primary medical institutions and community public health service centers. Therefore, under the support of the theoretical system of exercise prescription and the standard development norms, this clinical pathway of exercise prescription is developed according to the characteristics of national physical fitness and the status quo of primary healthcare institutions in China, aiming at simplifying the process of exercise prescription development, reducing the professional threshold, empowering primary healthcare, and providing a scientific and feasible solution for the promotion of exercise prescription in primary healthcare institutions.

  • Efficacy of exercise intervention in adolescents with mild to moderate idiopathic scoliosis: a meta-analysis

    ObjectiveTo systematically review the efficacy of exercise intervention in adolescents with mild to moderate idiopathic scoliosis (AIS). MethodsPubMed, Web of Science, The Cochrane Library, WanFang Data and CNKI databases were electronically searched to collect randomized controlled trials (RCTs) on the efficacy of exercise intervention in adolescents with mild to moderate AIS from inception to November 2021. Two reviewers independently screened the literature, extracted data and assessed the risk of bias of the included studies. Meta-analysis was then performed using Stata 16.0 software. ResultsA total of 11 RCTs involving 638 patients were included. The results of meta-analysis showed that exercise intervention significantly improved the Cobb angle of the spine in patients with AIS (MD=−1.65, 95%CI −2.63 to −0.68, P<0.01), reduced the angle of trunk rotation (ATR) (MD=−0.68, 95%CI −0.96 to −0.40, P<0.01), and increased their forced vital capacity (FVC) (MD=0.63, 95%CI 0.10 to 1.15, P=0.02). However, there was no statistically significant improvement in the forced expiratory volume in the first second (FEV1) or the ratio of FEV1/FVC between the two groups. ConclusionCurrent evidence shows that the exercise intervention could improve the Cobb angle of the spine, reduce ATR and enhance FVC. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusion.

  • Application of taijiquan in exercise rehabilitation of patients with chronic kidney disease

    Chronic kidney diseases (CKD) with long duration and a variety of complications have caused great physical and psychological problems for patients, and the overall quality of life of patients is low. Taijiquan, as a traditional Chinese techniques project, is beneficial to improving cardiopulmonary function, enhancing lower limb muscle strength, and reducing cardiovascular and cerebrovascular risks. We summarized the latest progress in clinical research concerning taijiquan as exercise rehabilitation for patients with CKD, aiming to promote the clinical application of taijiquan and other traditional exercises in the rehabilitation process of CKD patients and improve the overall quality of life of CKD patients.

  • Effectiveness of multiple exercise modalities on intervention in obese children and adolescents with metabolic syndrome: a network meta-analysis

    Objective To systematically review the effects of various exercise modalities on obese children and adolescents with metabolic syndrome (MetS). MethodsChinese and English databases such as CNKI, WanFang Data, VIP, PubMed and Web of Science were selected to search for RCTs on the effects of exercise on obese children and adolescents with MetS, and the search period was from January 2000 to November 2024 And two researchers independently screened the literature, extracted data and evaluated the risk of bias of the included studies, and net meta−analysis was performed using Stata 17.0 and RevMan 5.4 software. Results A total of 15 RCT trials involving 968 obese children and adolescents with MetS were included. The results of reticulated meta−analysis showed that compared with the non−exercise intervention group, aerobic exercise was effective in improving the patients' body mass index (BMI) (SMD=−1.21, 95% CI −2.31 to −0.11, P=0.031), total cholesterol (TC) (SMD=−0.44, 95% CI −0.82 to −0.05, P=0.028), triglyceride (TG) (SMD=−1.10, 95% CI −1.98 to −0.22, P=0.014), fasting blood glucose (FBG) (SMD=−0.70, 95% CI −1.34 to −0.07, P=0.030), systolic blood pressure (SBP) (SMD=−1.10, 95% CI −1.83 to −0.38, P=0.003), diastolic blood pressure (DBP) (SMD=−0.93, 95% CI −1.49 to −0.37, P=0.001); Resistance exercise can effectively improve the HDL cholesterol (SMD=0.55, 95% CI 0.09 to 1.02, P=0.020) and SBP (SMD=−1.16, 95% CI −2.18 to −0.14, P=0.025); aerobic combined with resistance exercise can effectively improve waist circumference (WC) (SMD=−1.09, 95% CI −1.74 to −0.44, P=0.001), BMI (SMD=−1.22, 95% CI −2.32 to −0.12, P=0.030), HDL (SMD=0.56, 95% CI 0.13 to 1.00, P=0.011), and FBG (SMD=−0.57, 95% CI −1.13 to −0.02, P=0.044). The results of cumulative probability ranking showed that aerobic exercise was the most effective in improving TG, TC, FBG and DBP; resistance exercise was the most effective in improving SBP; and aerobic combined with resistance exercise was the most effective in improving WC, BMI and HDL. ConclusionDifferent exercise modes have different improvement effects on various body indexes in obese children and adolescents with MetS. Due to the limitation of the number and quality of included studies, more high−quality studies are needed to verify the above conclusions.

  • Effects of Exercise Therapy on Patients with Low Back Pain: A Meta-analysis

    ObjectiveTo systematically review the clinical efficacy of exercise therapy for patients with low back pain. MethodsWe electronically searched databases including PubMed, The Cochrane Library, CNKI, WanFang Data and VIP from 2000 to September 2014 to collect randomized controlled trial (RCTs) about exercise therapy versus other therapies in the treatment of low back pain. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. Then, meta-analysis was performed by using RevMan 5.3 software. ResultsA total of five RCTs involving 413 patients were finally included. Compared with the control group, exercise therapy could relieve pain (MD=-0.92, 95%CI -1.32 to -0.51, P<0.000 1), and improve function activity (MD=-1.21, 95%CI -1.43 to -0.99, P<0.01). ConclusionExercise therapy can improve low back pain and functional activity to a certain extent. Due to the limited quantity and quality of the included studies, larger scale, multicenter, high quality RCTs are needed to verify the aforementioned conclusion.

Cardiac rehabilitation is a comprehensive and long-term plan including medical assessment, exercise prescription, correction of cardiovascular risk factors, education, counseling, and behavioral intervention. Evidence-based medical evidence confirms that cardiac rehabilitation plays an important role in the three level prevention of cardiovascular disease. In this paper, we searched the literature in recent 10 years to explain the current situation and future research direction of cardiac rehabilitation, and explore the best mode of cardiac rehabilitation.

  • Research Progress on Exercise Prescription: A Systematic Review

    Objective In order to improve the standing on the studies of Exercise Prescription (EP), a systematic review was conducted to provide reference and theory for further studies. Methods A broad computerized literature search of Medlin via Ovid, PubMed in all fields, Sport Discus, CINAHL, CAM, EMbase, EBM Cochrane Database of SR, and Web of Science was carried out till June 2008. Firstly, four categorical searches were conducted with the following keywords for searches: Exercise Prescription, Physical Fitness, Exercise Therapy, Fitness Assessment, Exercise Test and RCTs, and the searched results were checked by reviewers and duplicate results were removed. Then all titles were manually searched for potential inclusion in the review. Articles retrieved, review articles were examined for further relevant references. Results There were 318 relevant articles, however, the RCTs were only 7, and systematic review and meta-analysis were only one, respectively. Considering the importance and reference values there were 93 articles were included. 1970s was the developing age of EP; 1980s more specific EP were provided for many purposes; 1990s with the techniques developing of physical fitness, exercise therapy, fitness assessment and exercise test, EP for treating and rehabilitating more chronics and civilizing diseases were developed; 2000s the RCTs and CCTs were applied in the EP studies. Conclusions The developing of EP has close relationship with the developing of physical fitness, exercise therapy, fitness assessment, and exercise test. Although the RCTs researches in this field were limited, most of them were positive to support the advantage of EP. How to highly improve the advantages and greatly decrease the risk of EP is still valuable to study in the future.

  • Efficacy of exercise therapy in the treatment of chronic low back pain patients: a network meta-analysis

    ObjectiveTo systematically review the efficacy of exercise therapy for patients with chronic low back pain (CLBP) by network meta-analysis (NMA).MethodsThe PubMed, EBSCO, EMbase, The Cochrane Library, Web of Science, CNKI, WanFang Data, VIP and CBM databases were electronically searched to collect randomized controlled trials (RCT) on exercise for patients with CLBP from inception to May, 2020. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies. Then, NMA was performed by Stata 15.1 software.ResultsA total of 79 RCTs involving 5 782 CLBP patients were included. The effect of exercise therapy on pain in patients with CLBP were in the following rankings: yoga (SMD=−1.25, 95%CI −1.87 to −0.64, P<0.000 1), health Qigong/Taichi (SMD=−1.12, 95%CI −1.87 to −0.64, P=0.002), sling exercise (SMD=−1.07, 95%CI −1.64 to −0.50, P<0.000 1), Mackenzie therapy (SMD=−1.05, 95%CI −1.68 to −0.42, P=0.001), pilates (SMD=−0.96, 95%CI −1.74 to −1.78, P=0.016), multimodal training (SMD=−0.80, 95%CI −1.33 to −0.27, P=0.003) and stabilisation/motor control (SMD=−0.62, 95%CI −1.03 to −0.21, P=0.003). The effect of exercise therapy on function in patients with CLBP were in the following rankings: Mackenzie therapy (SMD=−0.62, 95%CI −1.03 to −0.21, P=0.003), and yoga (SMD=−0.88, 95%CI −1.51 to −0.25, P=0.007). Clusterank results showed that Mackenzie therapy, yoga, pilates, sling exercise and multimodal training were similar in improving pain and physical function in patients with CLBP.ConclusionsThe current study shows that yoga, Mackenzie therapy, pilates, sling exercise and multimodal training constitute the optimal group for improving CLBP symptoms. Health Qigong/Taichi is second only to yoga in improving pain in patients with CLBP, which has great promotional value.
