  • BCGpolysaccharide Nucleic Acid Combined with Runzao Antipruritic Capsule in Treating Chronic Urticaria

    目的:观察卡介菌多糖核酸联合润燥止痒胶囊治疗慢性荨麻疹的疗效。方法: 将2008年4月~2009年4月门诊就诊的86例慢性荨麻疹患者随机分成两组,治疗组44例采用卡介菌多糖核酸2 mL肌注,隔日1次,18次为1疗程;同时予口服润燥止痒胶囊4粒,3次/d,连续治疗36天;对照组42例单独口服润燥止痒胶囊,方法疗程同治疗组。结果: 治疗组有效率为90.91%,对照组为7143%。两组比较差异有显著性(Plt;005)。结论: 卡介菌多糖核酸联合润燥止痒胶囊治疗慢性荨麻疹疗效确切。

  • Acids-Suppressive Drugs and Ventilator-Associated Pneumoni

    为预防应激性溃疡和消化道出血, 质子泵抑制剂( PPI) 、组胺H2 受体拮抗剂( H2 RA) 以及硫糖铝在ICU中的应用非常普遍。既往的研究认为以PPI 或H2RA 为代表的抑酸剂( ASD) 可致胃液pH 值增高, 为细菌在胃腔内定植创造了条件, 并进而增加医院获得性肺炎( HAP) , 尤其是呼吸机相关性肺炎( VAP) 发生的风险。我们通过文献复习发现,ASD 与VAP 的关联性迄今仍无明确的证据, 已有的临床研究结论互为矛盾; 另一方面, 既往研究多集中于硫糖铝与H2RA 的比较, 有关PPI 和H2RA 导致VAP 的风险比较研究仍然缺乏, PPI 是否比H2 RA 更易发生VAP 也缺乏研究可资证明。为此我们将有关内容综述如下, 以供同道参考。

  • APPLICATION OF ISOIONIC MICROTRAUMA ARTHROSCOPE ON TREATMENT OF KNEE ARTHROPATHY

    Objective To investigate the clinical applicationand curative effect of isoionic microtrauma arthroscope on treatment of knee arthropathy. Methods From May 2003 to November 2004, 52 cases of knee joint injury were cured by using isoionic microtrauma arthroscope, including 30 cases ofknee osteoarthritis, 10 cases of meniscus injury, 5 cases of kneecap dislocation, 5 cases of laxity of anterior cruciate ligation and 2 cases of rheumatoid arthritis. In accordance with Lysholm criterion for knee joint function, the scores were 35.5±4.9 before operation. Results All of these patients were followed up for 2-17 months. The scores of knee joint function was 86.4±5.3 after operation, and there was significant difference (Plt;0.001). Conclusion Isoionic microtrauma arthroscope is characterized by low-temperature hemoagglutination, crimpling,boiling, cutting and hemostasia, which makes kneejoint arthroscope operation easier-to-do, miner histological scathe and lighter side effect; so it is favourable for functional recovery and its curative effect is satisfactory.

  • Clinical features of ventilator associated pneumonia caused by Stenotrophomonas maltophilia in intensive care unit

    Objective To investigate the characteristics of ventilator associated pneumonia (VAP)caused by Stenotrophomonas maltophilia(Sm)in ICU。Methods The clinical data of 39 patients with VAP caused by Sm,from Jan 2001 to Dec 2006,were retrospectively investigated.Results In 15 kinds of antibiotics sensitivity test,all cases showed 100% resistance to 12 kinds of antibiotics except sulfamethoxazole/trimethoprim。ticarcillin/clavulanic acid and ciprofloxacin with sensitivity rate of 46.2% , 30.8% and 12.8% .respectively.92.30% of Sm VAP were CO—infected with other microorganisms and 79.5% of VAP were late-onset.The use of broad-spectrum antibiotics.especially carbapenem.and prolonged mechanical ventilation more than 7 days were risk factors for Sm VAP.Morbidity of Sm VAP was 87.2% .Conclusions Sm VAP has an important role in ICU infections with high morbidity and CO-infection rate.It should be alerted to the possibility of Sm VAP in the case of when prolonged ventilation (gt;7 days)or carbapenem is used.

  • Research on the correlation between ventilation pressure and tidal volume in assisted mechanical ventilation with facemask during anesthesia induction

    Objective To investigate the curve correlation between ventilation pressure and tidal volume in assisted mechanical ventilation with facemask during anesthesia induction. Methods Between January and August 2015, 120 patients, American Society of Anesthesiology Ⅰ-Ⅱ, undergoing selective gynecological surgery were randomly divided into four groups: groups P5, P10, P15 and P20, with 30 patients in each group. Mask ventilation pressure for the four groups were respectively 5, 10, 15 and 20 cm H2O (1 cm H2O=0.098 kPa). Patients were ventilated by preset ventilation pressure and frequency based on different groups after loss of consciousness. Mean ventilation volume (mean value of three tidal volumes) and end-tidal carbon dioxide pressure (PetCO2) were recorded for analysis. Results There was no significant difference among the four groups in patient’s general condition (P>0.05). The tidal volume of assisted mechanical ventilation increased with ventilation pressure degrees, and the differences among the four groups were significant (P<0.05). After curve regression analysis, tidal volume and ventilation pressure showed a positive linear correlation when ventilation pressure was set at 5-20 cm H2O, and the correlation equation was: tidal volume = 33.612×ventilation pressure-53.155. PetCO2 in P5 group was lower than those in the other three groups (P<0.05), while there were no significant differences among groups P10, P15 and P20 (P>0.05). Conclusion When ventilation pressure is set at 5-20 cm H2O in assisted mechanical ventilation with facemask during anesthesia induction, tidal volume and ventilation pressure show a positive linear correlation.

  • Recent clinical research advances of reduced-port laparoscopic surgery for rectal cancer

    Objective To understand status of technical realization, present development, faced problems, and application prospects of reduced-port laparoscopic surgery for rectal cancer, and to analyze safety and feasibility so as to provide theoretical and practical basis for clinical application and promotion. Method By searching the databases such as Medline, Embase, and Wanfang, etc., the relevant literatures about reduced-port laparoscopic surgery for rectal cancer were collected and reviewed. Results At present, the most common reduced-port laparoscopic surgery was the 1-port laparoscopic surgery, 2-port laparoscopic surgery, and 3-port laparoscopic surgery. The 1-port laparoscopic surgery had the effects of minimal invasiveness and cosmesis, but it was difficult to perform. The 2-port laparoscopic surgery for rectal cancer preserved as far as possible the effect of minimal invasiveness, the difficulty of procedure was reduced greatly, which was easy to be learnt and promoted. The experience of the 3-port laparoscopic surgery for rectal cancer contributed to the technical development of the 1-port laparoscopic surgery, with no need for the assisted incision for intraoperative specimen. The reduced-port laparoscopic surgery for rectal cancer was technically feasible and safe, which possessed the equal or better short-term outcomes as compared with the conventional 5-port laparoscopic or open surgery beside the radical resection for rectal cancer. However, the stringent technique for the laparoscopic surgery was necessary and it needed to overcome the learning curve. Conclusions Reduced-port laparoscopic surgery has some obvious advantages in minimal invasiveness, cosmesis, and enhanced recovery. More large-sample, multi-center, randomized controlled trials are eager to further confirm safety, effectiveness, and feasibility of reduced-port laparoscopic surgery for rectal cancer.

  • Clinical histopathological and misdiagnostic analysis of 359 cases of intraocular tumors

    Objective To probe the clinical character,the histopathological classification and misdiagnoses of intraocular tumors. Methods The clinical and pathological data of 359 patients with intraocular tumor diagnosed clinically between 1980~2000 were retrospectively analyzed. Results There were 300 cases of malignant tumor and 23 cases of benign tumor respectively. Non-oncologic malady and benign tumor misdiagnosed as tumor or malignant tumor were 40 cases. The two leading malignant tumors were retinoblastoma and melanoma. Conclusion The clinical and pathological analysis of intraocular tumor is beneficial to the correct clinical diagnosis and treatment. (Chin J Ocul Fundus Dis,2002,18:28-30)

  • Attachments for Mandibular Implant-supported Overdenture: A Systematic Review

    Objective To evaluate clinical outcomes, patient satisfaction, and maintenance after treatment from mandibular implant-supported overdentures with different attachment types. Methods We searched six electronic databases: the Cochrane Central Register of Controlled Trials (CENTRAL, The Cochrane Library, Issue 4, 2005), Current Controlled Trials, MEDLINE (1966 to Sept. 2005), EMBASE (1984 to Sept. 2005), and Chinese biomedical database disk (1978 to Sept. 2005). Eleven Chinese professional journals in oral health were also handsearched from their first published issues. Three authors screened and selected the studies, appraised their methodological quality and extracted data from the studies. The results were presented narratively by meta-analysis. Results After strict screening, 12 trials involving 282 patients were included. Two of the trials were included in a meta-analysis with 27 patients in bar-clip group, 29 patients in ball-spring group, and the other trials were described thoroughly. The findings seemed to indicate that the bar-clip group had the highest retention but more oral mucosa complications, while the ball-spring group had good retention and less oral mucosa complications but needed more aftercare treatments, and the magnetic group had less retention but better peri-implant outcomes. Conclusions There is inadequate evidence to prove which is the best choice for mandibular implant-supported overdentures among bar-clip, ball-spring and magnetic attachments. More controlled clinical trials are required to guide clinicians on the choice of the type of attachment in mandibular implant-supported overdenture.

  • Citespace-based visualization analysis of the imaging research progress of gastric cancer

    ObjectiveTo realize the current status and hot issues in the field of gastric cancer imaging research, and to provide references for radiologists and gastrointestinal surgeons to grasp the overall overview of gastric cancer imaging.MethodWe downloaded the relevant literatures of gastric cancer imaging published in 2010–2020 in the Web of Science database, and used Citespace 5.7.R3 software for related visual analysis.ResultsA total of 726 articles in the field of gastric cancer imaging research were retrieved and screened. The number of gastric cancer imaging studies published from 2010 to 2020 was on the rise, with the top three countries published in China, Japan, and the United States. The clusters of co-cited literature on gastric cancer imaging: confocal laser microendoscopy, gastric cancer, gastric mucosal intestinal metaplasia, preoperative T staging, convolutional neural network, deep learning, advanced gastric cancer, in vivo differentiation, early stage gastric cancer, surgical treatment, perigastric lymph nodes, nearest neighbor algorithm, and so on. Hot words in recent research fields included: risk factors, characteristics, deep learning, chemotherapy, and neural networks.ConclusionThe imaging diagnosis of early gastric cancer and the application of artificial intelligence are the research hotspots in this field.

  • Application of channel screw combined with skin-stretching technique in treatment of pelvic fracture with severe Morel-Lavallée lesion

    ObjectiveTo investigate the effectiveness of channel screw combined with skin-stretching technique in treatment of pelvic fracture with severe Morel-Lavallée lesion.MethodsBetween January 2017 and May 2020, 11 patients with pelvic fractures complicated with severe Morel-Lavallée lesions were treated with channel screw combined with skin-stretching technique. There were 8 males and 3 females, with an average age of 39.2 years (range, 28-58 years). Causes of injury included traffic accident in 9 cases and falling from hight in 2 cases. The time from injury to hospital admission ranged from 1 to 6 days (mean, 3.7 days). According to the Tile classification, the pelvic fractures were rated as type C1 in 7 cases, type C2 in 3 cases, and type C3 in 1 case. The sites of Morel-Lavallée lesions were buttocks in 8 cases, hips in 2 cases, and thigh in 1 case. There was 1 case of rectal injury, 1 case of splenic rupture, 3 cases of craniocerebral injury, 2 cases of chest injury, 4 cases of spinal fracture, and 6 cases of sacral nerve injury. X-ray films were performed to observe reduction and healing of fracture. The quality of fracture reduction was evaluated by Matta standard at 1 week after operation. Wound healing and complications were observed. Majeed scoring was used to evaluate hip function and Gibbons sacral nerve injury grading was used to evaluate neurological function recovery.ResultsThe patients underwent 4-7 operations, with an average of 5.8 operations. The time of first operation (pelvic fracture fixation, debridement, and skin distractor installation) was 115-275 minutes, with an average of 186.5 minutes. The amount of intraoperative blood loss was 30-80 mL, with an average of 45.5 mL. All patients were followed up 6-12 months, with an average of 8.3 months. The quality of fracture reduction according to the Matta standard was excellent in 7 cases, good in 2 cases, and fair in 2 cases, with an excellent and good rate of 81.8%. All fractures healed at 3-6 months after operation, with an average of 4.5 months. At last follow-up, the hip functions were excellent in 7 cases and good in 4 cases according to Majeed scoring, with an excellent and good rate of 100%. Among the 6 patients complicated with sacral nerve injury, the Gibbons sacral nerve injury grading reached gradeⅠ for 4 cases, gradeⅡ for 1 case, and grade Ⅲ for 1 case. The wounds of 9 cases were sutured directly after skin-stretching treatment for 25-32 days, with an average of 28 days. The dermal edge necrosis occurred in 1 case, and the defect was repaired by free skin grafting. The skin superficial dry necrosis of the wound occurred in 1 case, and healed by dressing change.ConclusionIn the treatment of pelvic fracture complicated with severe Morel-Lavallée lesion, the channel screw and skin-stretching technique showed their own advantages, which can obtain early fracture fixation and good wound healing.
