Objective To assess the efficiency of iodised salt on iodine deficiency disorders. Methods we searched MEDLINE (1996 to December 2009), EMbase (1974 to December 2009), The Cochrane Library (Issue 4, 2009), Current Controlled Trials, The National Research Register, CBM (1983 to December 2009) and CNKI (1994 to December 2009), and traced the literature of the reference. The included objects were the resident of the iodine deficiency area and the age, the race. The sex was not limited. The fortified salt and the iodized salt effects were compared and the research quality was evaluated. Meta-analyses were conducted by the RevMan5.0 software. Results Eight studies involving 1 503 people were included. The goiter prevalence of the fortified salt which combined iodine with iron was lower than that of iodised salt [MD=1.22, 95%CI (0.90, 1.54)]. So was the thyroid gland volume [OR=1.88, 95%CI(1.01, 3.47)]. But the fortified salt which combined iodine with VA did not show this. Conclusion The fortified salt and the iodized salt can obviously improve the iodine nutritional status of people. The fortified salt has enhanced the effect while the iodized salt prevents iodine deficiency disorders, but more high grade randomized controlled trials are needed to confirm this.
Objective To approach the inhibitory effect of Iodine-125 (125I) on moderately differentiated adenocarcinoma of colon by establishing the nude mice model bearing subcutaneous tumor of SW480 cell. Methods The moderately differentiated adenocarcinoma of colon cells (SW480) were implanted subcutaneously to the nude mice. The bearing tumor nude mice were randomly divided into study group (n=24) and control group (n=24) by using method of random sampling. One blank particle was implanted into the mouse of the control group, a 1.48×107 Bq dosage 125I particle was implanted into the mouse of the study group, then the growth of tumor was observed after implantation. Six bearing tumor nude mice were sacrificed and the tumors were obtained on day 7, 14, 21, and 28 after implantation, respectively. The expression of proliferating cell nuclear antigen (PCNA) was detected by immunohistochemistry SP method. The cell apoptosis was determined by TUNEL method. Results As the accumulation of radiation time, the volume of tumor in the study group was smaller than that in the control group on day 10 after implantation (Plt;0.05). The PCNA labeling index in the study group was lower than that in the control group on day 14 after implantation (Plt;0.05). The apoptotic index in the study group was higher than that in the control group on day 21 after implantation (Plt;0.05). Conclusion Persistent low dose 125I radiation could down-regulate the expression of PCNA, and induce the apoptosis of moderately differentiated adenocarcinoma of colon cell, which might be a mechanism of inhibiting the proliferation of moderately differentiated adenocarcinoma of colon.
Objective To explore the value of iodine overlay image technique based on dual-source CT dual-energy for the diagnosis in the primary hepatocellular carcinoma patients undergoing transcatheter arterial chemoembolization (TACE). Methods The imaging data of patients with primary hepatocellular carcinoma after TACE treatment from September 2015 to November 2018 were retrospectively analyzed. All patients completed dual-source CT dual-energy examination and digital subtraction angiography (DSA). The imaging data of the patients were analyzed by conventional linear blending images and iodine overlay image. The results of DSA were taken as the gold standard. The data indexes measured by the two imaging methods were compared. The consistency between the two imaging methods and DSA was determined by Kappa statistics. Results A total of 31 patients were included, and 57 nodes were found under DSA examination. The image quality evaluation (P<0.05), sensitivity (95.35% vs. 69.77%), specificity (92.86% vs. 64.29%), accuracy (94.74% vs. 68.42%), positive predictive value (97.62% vs. 85.71%) and negative predictive value (86.67% vs. 40.91%) of iodine overlay images were higher than those of conventional linear blending images. The consistency between conventional linear blending images and DSA was low (Kappa=0.286, P=0.023), and the consistency between iodine overlay images and DSA was excellent (Kappa=0.861, P<0.001). Conclusions Iodine overlay image could avoid the iodide artifact effectively and evaluate the abnormal enhancement quantitatively and qualitatively in patients undegoing TACE. This method could obtain more information to help diagnose in clinical and evaluate the postoperative efficacy of TACE accurately and objectively.
【 Abstract 】 Objective To investigate the clinical effects of targeting therapy with iodine-131 labeled monoclonal antibody for hepatocellular carcinoma (HCC). Methods The related published literatures were reviewed and summarized. Results The reasonable application of targeting therapy with iodine-131 labeled monoclonal antibody could improve the prognosis for patients with HCC especially for some primary HCC. It was used in various kinds of HCC patients with no severe side effects. ConclusionThe targeting therapy with iodine-131 labeled monoclonal antibody may be considered as a safe and effective method to treat HCC and an adjuvant therapy for liver surgery.
Objective To reveal the relationship between the iodine nutrition and the change of spectrum of thyroiddiseases by analyzing the change of spectrum of thyroid diseases in different iodine environments before and after the implementation of universal salt iodization (USI). Methods To compare the urinary iodine concentration between the normal people (1 000 cases) and the patients with thyroid diseases (5 998 cases) by surgical operations who were from 4 cities of Guangxi Zhuang Autonomous Region, covering the iodine deficient areas and the iodine rich areas. Results After the USI was put into practice, the proportions of nodular goiter decreased, but the proportions of toxic nodular goiter, thyroid papillary carcinoma, and Hashimoto thyroiditis were higher than those before USI (P<0.05). The urinaryiodine concentrations of nodular goiter, Graves disease, toxic nodular goiter, thyroid papillary carcinoma, and Hashimotothyroiditis were higher than those before the measure was taken (P<0.05). The urinary iodine concentration of patients with thyroid was higher than that of normal people (P<0.05), and the urinary iodine concentration of patients with thyroidand normal people was higher than those before the USI (P<0.05). Conclusions The change of spectrum of thyroid diseases in Guangxi Zhuang Autonomous Region is obvious within 10 years after USI had been taken. The excessive intake of iodine may be one of the dangerous factors leading to toxic nodular goiter, thyroid papillary carcinoma, and Hashimoto thyroiditis.
Backgroud Iodine deficiency is the main cause for potentially preventable mental retardation in childhood and is still prevalent in large parts of the world. Objectives To assess the effects of iodised salt in comparison with other forms of iodine supplementation or placebo in the prevention of iodine deficiency disorders.Search strategy We searched the Cochrane Library, Medline, the Register of Chinese trials developed by the Chinese Cochrane Centre, and the Chinese Med Database. We performed handsearching of a number of journals (Chinese Journal of Control of Endemic Diseases, Chinese Journal of Epidemiology, Chinese Journal of Preventive Medicine, and Studies of Trace Elements and Health up to February 2001), and searched reference lists, databases of ongoing trials and the Internet. Date of latest search: November 2001. Selection Criteria We included prospective controlled studies of iodised salt versus other forms of iodine supplementation or placebo in people living in areas of iodine deficiency. Studies reported mainly goitre rates and urinary iodine excretion as outcome measures. Data collection amp; analysis As the studies identified were not sufficiently similar and not of sufficient quality, we did not do a meta-analysis but summarised the data in a narrative format. Main results We found six prospective controlled trials relating to our question. Four of these were described as randomised controlled trials, one was a prospective trial that did not specify allocation to comparison groups, and one was a large population-based study comparing different interventions. Comparison interventions included non-iodised salt, iodised water, iodised oil, iodation with potassium iodide versus potassium iodate. Numbers of participants in the trials ranged from 35 to 334; over 20 000 people were included in the population-based study. There was a tendency towards goitre reduction with iodised salt, although this was not significant in all studies. There was aim an improved iodine status in most studies (except in small children in one of the studies), although urinary iodine excretion did not always reach the levels recommended by the WHO. Reviewers’ conclusions The results suggest that iodised salt is an effective and safe means of improving iodine status. However, high quality controlled studies are needed to address questions of dosage and best means of iodine supplementation in different population groups.
Objective To observe the effect of cisplatin in bletilla hyacinthine particle chemotherapy combined with 125I brachytherapy on short-and long-term outcomes and the toxic and side effects in advanced gastric cancer. Methods One hundred seventy-six patients with stage Ⅱ or stage Ⅲ advanced gastric cancer underwent curative surgical resection were included in this study. They were randomly divided into brachytherapy and chemotherapy group (n=48), intraperitoneal chemotherapy group (n=32) and intravenous chemotherapy group (n=48), and other patients who abandoned radiotherapy and chemotherapy and signed informed consent form by themselves were considered as control group (n=48). The short-and long-term outcomes and the toxic and side effects were observed and the survival of all patients was analyzed by Kaplan-Meier method and Log-Rank test. Results For short-term outcomes, the total effective rate in 4 groups were 95.83%, 71.88%, 64.58% and 52.08% respectively, and the difference was significant (Plt;0.05). For long-term outcomes, the 3 -and 5-year mortality rate was 37.50% and 56.30%, and 5-year median survival time was (14±4.51) months (95%CI: 14.419-4.512) in brachytherapy and chemotherapy group patients. The 3- and 5-year mortality rate was 78.12%and 93.75%and 5year median survival time was (10.6±1.13) months (95%CI: 10.620-1.163) in intraperitoneal chemotherapy group patients. The 3-and 5-year mortality rate was 79.21%and 95.80%and 5-year median survival time was (11±3.10) months (95%CI: 11.130-3.162) in intravenous chemotherapy group patients. The 3-and 5-year mortality rate was 87.50%and 95.83% and 5-year median survival time was (9±2.30) months (95%CI: 10.024-1.180) in control group patients. Compared with the vein chemotherapy group, the short distance puts the chemotherapy group disgusting vomit, the marrow to suppress, the liver function harm, the kidney function harm formation rate to reduce obviously (Plt;0.05). Conclusion Cisplatin in bletilla hyacinthine particle chemotherapy combined with 125I brachytherapy can reduce the toxic and side effects of drugs and prolong survival time of patients with advanced gastric cancer.
The iodine-125 (125I) seeds interstitial implantation has the advantages of minimal invasiveness, high local control rate, and few complications; it has attracted worldwide attention. With the application of 3D printing technology in medicine, individualized 3D templates are gradually applied to clinical practice. Individualized 3D templates combining with CT-guided 125I seeds implantation are easy to operate and can not only effectively ensure the consistency and accuracy of preoperative and postoperative dose, but also minimize complications to achieve optimal efficacy. This paper reviews the application of CT-guided implantation of 125I seeds for malignant tumors assisted by individualized 3D template, and further demonstrates the safety and effectiveness of clinical application in 125I seeds implantation to provide a reliable basis for the standardization of 125I seeds implantation.
Objective To observe the therapeutic efficacy and complications of plaque radiotherapy (PRT) combined with transpupillary thermotherapy (TTT) on choroidal melanoma (CM). Methods Thirty unilateral CM patients (30 eyes, including 15 males and 15 females) were treated by PRT and TTT. The visual acuity ranged from 0.1 to 0.8 with an average of 0.3plusmn;0.2. The largest base diameter of tumor ranged from 6.8 mm to 17.9 mm with an average of (11.3plusmn;2.8) mm;The tumor height ranged from 3.9 mm to 10.6 mm with an average of (7.2plusmn;2.4) mm. The criteria of controlled local tumor: based on B-scan ultrasound measurement, the tumor was considered as ldquo;growingrdquo; if tumor height increased 2 mm or tumor largest base diameter increased 250 mu;m, otherwise the tumor was considered ldquo;controlledrdquo;. The followup ranged from 15 to 57 months with an average (33.01plusmn;9.81) months. The local tumor control rate, enucleation rate and visual acuity, complications after treatment were observed.Results The tumor largest base diameter after treatment ranged from 4.6 mm to 17.0 mm with an average (9.79plusmn;3.35) mm, which had statistically significant difference(t=2.195,F=0.49;P=0.032) with that before treatment. The tumor height after treatment ranged from 2.7 mm to 11.9 mm with an average (5.19plusmn;2.57) mm, which had statistically significant difference(t=2.069,F=0.018;P=0.0435) with that before treatment. At the end of follow up, the tumor largest diameter and height increased in two eyes respectively compared with those before treatment. Local tumor control rate was 86.7%. Three eyeballs were enucleated after treatment,the enucleation rate was 10.0%. The visual acuity remained unchanged in 12 eyes,improved in one eye and decreased in 17 eyes. Treatment complications included radiation retinopathy in 12 eyes (40.0%), secondary retinal detachment in three eyes (10.0%), secondary glaucoma in one eye (3.3%), cataract in four eyes (13.3%) and dry eye syndrome in five eyes (16.7%). Conclusion PRT combined with TTT is an effective therapy for choroidal melanoma with less complications.
Objective To investigate the effect of iodine-125 on inhibiting breast cancer growth and to explore its possible mechanism. Methods The animal model of the MCF-7 tumor was established firstly through injection of the cells into nude mice. The animals were divided into two groups before the implantation of the iodine-125 granule into the tumor mass: control group (n=40, to implant no-load seeds, non-iodine-125 radioelement) and experimental group 〔n=40, to implant iodine-125 seeds (1.48×107 Bq) when the length of tumor was 8-10 mm〕. The width and length of tumor, in order to calculate the volume, were measured every three days to observe tumor growth curve and to calculate the rate of the tumor inhibition. When the length of tumor was 15-20 mm in the control group, 30 nude mice were killed in every group to detect the weight of tumor and histopathological changes. Other ten nude mice of each group were remained to be observed the national life span and survival rate for 90 days. Results Within 90 days, the average survival time in control group and experimental group were significantly different (56.2 d vs. 74.8 d, P<0.05). In control group the growth curve was continuously elevated, while experimental group showed a low flat curve. With iodine-125 treatment, the tumor growth decreased in experimental group with tumor inhibition rate 55.21%. The average tumor weight in control group and experimental group was (3.26±0.39) g and (1.46±0.17) g (t′=22.8962, P<0.05). As compared with control group, under light microscope, the number of cancer cells was less, nuclear debris increased, and cancer structure was not obvious in experimental group. Conclusion This study suggests that iodine-125 seed may inhibit the growth of breast cancer, which may be involved in direct radiation breakdown of tumor cells or induction of apoptosis and inhabitation of tumor angiogenesis.
