ObjectiveTo explore the variation tendency of hospital infection and the antibacterial usage between 2011 and 2012,analyze the factors for hospitalization infection,and instruct on taking effective intervention measures,in order to reduce the rate of hospital infection,and thus to provide a scientific basis for the prevention and control of hospital infection. MethodsAll patients hospitalized in our hospital from 00:00 to 24:00 between September 21,2011 and August 16,2012 were investigated.By using the method of combined clinical investigation and case study,we asked the patients to fill the case questionnaires standardly,and analyzed the hospital infection cases on the survey day. ResultsA total of 1165 hospitalized patients were investigated from the year 2011 to 2012,and the actual check rate was 100%.A total of 46 cases of hospital infection occurred in 39 patients.The prevalence rate of hospital infection was 3.88% in 2011 and 2.80% in 2012.High rate hospital infections occurred in the Department of Neurosurgery which was 27.27% and Cardiothoracic surgery which was 16.67% in 2011.In 2012,the departments included ICU which was 28.57% and Internal Cardiovascular Medicine which was 9.09%.The main infection sites were respiratory tract,followed by urinary tract and superficial incisions.The usage rates of antibiotics were 63.07% and 40.56% respectively in the two years.There were significant difference in antibiotic use rate between the two years (P<0.01). ConclusionThe focus of future work is to strengthen the management of respiratory tract,urinary tract and surgical site infections,and to use antimicrobial drugs reasonably,in order to reduce hospital infection effectively.
ObjectiveTo explore the current situation of nosocomial infection, sample test and use of antibiotics in the in-patients, so as to provide scientific evidence for control and prevention of nosocomial infection. MethodsBedside investigation and medical records checking were adapted to conduct cross-sectional survey among all the in-patients in 2013 and 2014. ResultsOn July 16 th, 2013 (00:00-24:00) and September 10 th, 2014 (00:00-24:00), a total of 1 400 in-patients were investigated in whom 68 with nosocomial infection were diagnosed, with a prevalence rate of 4.86%. No statistical difference were found between the two year's prevalence rate (χ2=1.341, P=0.264). The prevalence rate of Intensive Care Unit, Department of Neurosurgery (including Department of Thoracic Surgery) and Department General Surgery ranked the first three places, and the most common infection position was respiratory tract (61.76%). The usage rate of antibiotics was 48.00% in the two years, and no statistical difference was observed in the usage rate between the two years (χ2=1.309, P=0.261). Therapeutic use (67.86%) and onefold use (90.33%) accounted for most of antibiotics use. Test rate of therapeutic antibiotics use was 49.17% and no statistical difference was observed in the test rates between the two years (χ2=2.023, P=0.170). ConcluslonsThe nosocomial infection rate was stably high in the two years. To prevent and control nosocomial infection effectively, medical personnels knowledge and skills on nosocomial infection should be improved, surveillance and guidance towards key departments should be strengthened and clinicians' diagnosis and treatment behaviors should further be regulated.
Objective To systematically review the prevalence of osteoporosis (OP) in Chinese postmenopausal females. Methods The CNKI, VIP, CBM, WanFang Data, PubMed, EMbase and The Cochrane Library databases were electronically searched to collect studies on the prevalence of OP in Chinese postmenopausal females from inception to March 17th, 2022. Two reviewers independently screened the literature, extracted data and evaluated the risk of bias of the included studies. Meta-analysis was then performed using Stata 14.0 software. Results A total of 54 studies involving 96 391 cases and 41 651 confirmed OP patients were included. The meta-analysis results showed that the prevalence of OP in Chinese postmenopausal females was 38.8% (95%CI 34.1% to 43.5%). Subgroup analysis showed that the prevalence of OP was 31.7% (95%CI 16.6% to 48.6%) in females who had been menopausal for less than 5 years, 47.4% (95%CI 32.2% to 62.5%) for 5-9 years, 52.7% (95%CI 37.1% to 68.3%) for 10-14 years, 77.5% (95%CI 69.5% to 85.4%) for 15-19 years, and 78.3% (95%CI 56.3% to 100.0%) for more than 20 years. The rate was 16.2% (95%CI 28.8% to 37.2%) in under 50 years age group, 28.8% (95%CI 34.4% to 47.9%) for 50- years group, 41.1% (95%CI 34.4% to 47.9%) for 60- years group, 55.3% (95%CI 45.8% to 64.8%) for 70- years group, 82.0% (95%CI 76.1% to 87.9%) for 80- years group. The prevalence in different regions was between 31.0% and 43.5%, which was lowest in the northeast (31.0%, 95%CI 26.3% to 51.4%) and highest in the northwest (43.5%, 95%CI 22.3% to 64.7%). According to the year of publication, the rate was 43.9% (95%CI 15.7% to 72.0%) from 1997 to 2009 and 38.2% (95%CI 33.8% to 42.5%) from 2010 to 2021. The prevalence of OP was 24.9% (95%CI 20.5% to 29.3%) in postmenopausal females with no birth or 1 birth, and 45.0% (95%CI 37.0% to 53.0%) in females with 2 or more births. Conclusion The prevalence of OP in Chinese postmenopausal females is relatively high. Due to the limited quantity and quality of the included studies, more high-quality studies are needed to verify the above conclusion.
ObjectiveTo learn the current situation and feature of nosocomial infection (NI), so as to provide reference for making the prevention measures. MethodsPrevalence rate of nosocomial infection in patients hospitalized on December 18, 2012 was investigated by the combination of bed-side examination and medical record checking. ResultsA total of 1 083 patients were surveyed, and the prevalence rate of nosocomial infection was 6.00%. Intensive Care Unit had the highest NI prevalence (28.57%), followed by Hematology Department (20.00%) and Neurosurgery Department (20.00%). Lower respiratory tract (38.46%) was the main infection site, followed by superficial surgical incision (13.85%). The usage rate of antimicrobial agents was 30.56%, and 40 strains of pathogens were isolated, in which Klebsiella pneumonia (15.0%), Pseudomonas aeruginosa (12.5%) and Escherichia coli (10.0%) were the main ones. ConclusionTarget monitoring management of key departments and key sites should be strengthened, and the management of antimicrobial agents should be stressed and invasive procedures should be decreased.
ObjectivesTo systematically review the prevalence of methicillin-resistant staphylococcus aureus (MRSA) in healthy Chinese population.MethodsPubMed, EMbase, WanFang Data and CNKI databases were electronically searched to collect cross-sectional studies of the prevalence of MRSA in China from inception to December 16th, 2018. Two reviewers independently screened literature, extracted data and assessed the risk of bias of included studies, and then, meta-analysis was performed by using Stata 12.0 software.ResultsA total of 25 cross-sectional studies were included. The results of meta-analysis showed that: the pooled prevalence of MRSA in healthy population was 13.9% (95%CI 9.6% to 18.2%). The results of subgroup analysis showed that: the prevalence of MRSA in children was 16% (95%CI 8% to 24%), and that in adults (non-children) was 13% (95%CI 9% to 16%). The prevalence of MRSA in individuals with occupational livestock exposure was 28% (95%CI 5% to 51%), in medical staff it was 16% (95%CI 8% to 25%), in medical students it was 12% (95%CI 3% to 20%) and in community residents it was 5% (95%CI 2% to 8%).ConclusionsThe overall prevalence of MRSA in healthy Chinese population is approximately 13.9%. Effective prevention and control measures are required to reduce the spread of MRSA.
Objective To understand the prevalence of overweight and obesity and its influence factors in Zhuhai inhabitants. Methods Applying multi-stage cluster random sampling in the three administrative areas of Zhuhai including Xiangzhou, Doumen and Jinwan, A questionnaire-based survey was performed in conjunction of the measurement of height and weight among 961 inhabitants aged 15-69 years. In addition, a multivariate unconditional logistic regression model was employed to analyze the influence factors of overweight and obesity. Results The prevalence and standardized rates of overweight and obesity in the study population were 18.1%, 17.6%, 6.6%, 6.5%, respectively. The standardized rates of overweight and obesity in male and female were 18.4%, 5.8%, 16.5%, 7.2%, respectively. Age, drinking, smoking and regional difference were identified as the 4 risk factors of overweight and obesity, their OR values being 1.028, 1.683, 0.677, 1.404, Plt;0.05, respectively. Conclusion The prevalence of overweight and obesity in Zhuhai’s inhabitants was over the average level of Chinese residents, and overweight and obesity has become a major risk factor influencing the health of Zhuhai’s inhabitants. In view of the influence factors of overweight and obesity, timely and effective prevention and control measures should be taken.
ObjectiveTo systematically review the prevalence and risk factors of the chronic post-cesarean section pain (CPCSP). MethodsPubMed, EMbase, The Cochrane Library, CINAHL, PsycInfo, CBM, WanFang Data, VIP, and CNKI databases were electronically searched to collect studies on the prevalence and risk factors of CPCSP from inception to August 2021. Two reviewers independently screened literature, extracted data, and assessed the risk of bias of included studies. Meta-analysis was then performed using Stata 15.1 software. ResultsA total of 43 studies involving 12 435 participants were included. The results of meta-analysis showed that the prevalence of CPCSP for 2 to 5 months, 6 to 11 months, and at least 12 months were 19% (95%CI 15% to 23%), 13% (95%CI 9% to 17%), and 8% (95%CI 6% to 10%), respectively. The risk factors included preoperative pain present elsewhere, postoperative severe acute pain, low abdominal transverse incision, non-intrathecal administration of morphine, preoperative anxiety, postpartum depression, etc. ConclusionsThe current evidence shows that the overall prevalence of CPCSP is high. Preoperative pain presents elsewhere, postoperative severe acute pain, low abdominal transverse incision, non-intrathecal administration of morphine, preoperative anxiety and postpartum depression may increase the risk of CPCSP.
Objective To investigate the clinical effectiveness and adverse event of preventive medicine for severe acute respiratory syndrome (SARS), and provide clinical data for designing prospective clinical trial. Method Retrospective study on medical staffs, that were exposed to SARS patients, was conducted in two main SARS designated hospitals to obtain information such as SARS exposure risk and preventive measures (medical and others). According to the type of preventive medicine, they were assigned to earthworm’s nucleases and protease (ENP) group, interferon group and blank control group respectively. Exposure risk, suspected sub-clinical infection rate and adverse event rate were compared between the three groups. Results Non-medical preventive measures used in each group were consistent, but the exposure intensity to risk factors between groups was statistically different, which biased the evaluation of clinical effectiveness of preventive medicine. The rate of suspected sub-clinical infection in earthworm’s nucleases and protease (ENP) group, interferon group and control group were 4.5%, 4.5%, and 9.9% (Pgt;0.05), respectively; and adverse event rate were 19.6%, 13.6% (Pgt;0.05), and 0%, respectively. Conclusions Suspected sub2clinical infection rate in ENP group, interferon (INF) group were lower than that in control group, which indicated that these two medicines might be effective in preventing SARS. Adverse event rate in ENP group was similar to that of interferon group, and the symptoms were mild in both groups, which was in accordance with the result of in vitro experiments. ENP spray is a kind of biological preparation; further purification may reduce its adverse event rate. However, because there had excessive confounding factors, especially because of the unequal of exposure risk between three groups, the results of this study can only provide insights to design prospective clinical trial in the future.
ObjectiveTo investigate the prevalence of nosocomial infection in a hospital and to provide a basis for hospital infection control. MethodsUsing bedside investigation and medical records analysis, we surveyed all hospitalized patients from 00:00 to 24:00 on July 19th, 2013. ResultsThe real investigation was carried out on 1815 patients out of all the 1828 patients with a real investigation rate of 99.29%. There were 55 cases of nosocomial infection (55 case-times), and both the nosocomial point infection rate and case-time infection rate were 3.03%. The top three departments with the highest rate were Intensive Care Unit (37.50%), Neurosurgery Department One (13.73%) and Neurosurgery Department Two (12.00%). Most infections occurred on the lower respiratory tract, which accounted for 45.45%. Nosocomial infection pathogenic detection rate was 38.18% (21/55):6 cases of Staphylococcus aureus (28.57%), 5 of Pseudomonas aeruginosa (23.81%), 3 of Klebsiella pneumoniae (14.29%), and 2 cases of Acinetobacter baumanii (9.52%). The rate of antimicrobial drug use was 24.08%, in which drug treatment accounted for 75.29%. Gender, surgery, urinary catheter, vascular catheter, tracheostomy, ventilator application, hemodialysis, and use of antibiotics were all influencial factors for occurrence of nosocomial infection. ConclusionNosocomial infection prevalence survey can help fully understand the status of hospital infection, help to carry out targeted surveillance, and better guidance for hospital to prevent and control nosocomial infection.
Objective To investigate the morbidity of ocular fundus diseases in Tibetan above 40 in Maqin county, Qinghai Province. Methods A total of 2511 Tibetan above 40 in Maqin county, Qinghai Province were seelected by cluster sampling method. The sampling survey of ocular fundus diseases was performed on the individuals with indirect ophthalmoscope, and the results were compared with the domestic existing epidemiological data. Results Two hundred and thirty five (9.35%) cases were found to have abnormal ocular fundus, and the blindness rate due to the abnormal ocular fundus was 20.5% (80/391 eyes) which was lower than the blindness rate due to cataract (52.5%). Low vision and blindness led by ocular fundus diseases were found in 155 eyes (65.96%),including 74(47.74%) with age-related macular degeneration (AMD) and 23 (9.79%) with retinoangiopathy. Conclusions The morbidity of ocular fundus disorder is higher in high plateau than that in backland; AMD and retinoangiopathy are the main diseases leading to blindness.
