Objective To investigate the characteristics of optical coherence tomography (OCT) images in idiopathic macular hole. Methods OCT、color photography and fundus fluorescein angiography were performed in 65 cases(70 eyes) of macular holes and which were then graded by connecting to their clinical characteristics. Results Among the 70 eyes the number of 1~4 stages of macular holes were 11,12,36 and 11 eyes respectively.In eyes of stage 1 OCT images showed flattening or disappearing of fovea and minimally reflective space within or beneath the neurosensory retina;stage 2 showed a fullthickness hole with an attached operculum and surrounding edema;stage 3 displayed a full-thickness hole with surrounding edema and stage 4 showed a full-thickness hole and a complete separation of the poterior hyaloid membrane from the retina.The dimeter of the macular holes in stage 2,3 and 4 were (241.75plusmn;107.08),(699.78plusmn;160.99), (631.36plusmn;243.46)mu;m,respectively. Conclusions OCT can display the characteristics of idiopathic macular holes and measure the diameters of holes quantitatively. (Chin J Ocul Fundus Dis, 1999, 15: 205-208)
Objective To evaluate and compare the prognosis of idiopathic macular holes (IMH) and traumatic macular holes (TMH) treated by pars plana vitrectomy (PPV).Methods The clinical data of 72 IMH eyes and 55 TMH eyes, which were treated by PPV between November 2001 and December 2007, were retrospectively reviewed. The visual outcomes and macular anatomic closure were evaluated, and their relationships with prognostic factors including the size of macular hole (MH), preoperative visual acuity (VA) and duration of disease were analyzed.Results The closure rate of IMH (100.0%) was significant higher than that of TMH (85.5%) (P=0.001). The postoperative VA of IMH and TMH were (0.25plusmn;0.02) and (0.21plusmn;0.21) respectively,both significantly increased compare to their preoperative VA (t=-6.841,-4.093; P=0.000). VAincreased IMH and TMH eyes had same VA (chi;2=3.651,P=0.07). PrePPV VAge;0.1 IMH eyes had better outcomes than PrePPV VA<0.1 IMH eyes (chi;2=12.04, P=0.001), while PrePPV VA had no effects on TMH outcomes (chi;2=0.371,P=0.486). IMH eyes with small holes had better outcomes (t=2.476,P=0.016), and TMH eyes with small holes had better closure (t=-4.042, P<0.001). The duration of disease had no significant influence on TMH visual (chi;2=0.704, P=0.401) and anatomic (chi;2=0.166, P=0.684) outcomes. Conclusions PPV is an effective treatment for MH. The closure rate of IMH is higher than that of TMH. The diameter of MH and preoperative VA are major factors for IMH outcomes, and the duration of disease and preoperative VA have no effects on postoperative VA in TMH.
The reattachment rate, macular hole (MH) closure rate, visual acuity improvement and re-detachment rate of MH retinal detachment (MHRD) of high myopia are not satisfactory owing to long axis oculi, posterior scleral staphyloma and macular atrophy. At present, minimally invasive vitrectomy surgery combined with the internal limiting membrane flap technique has become popular in the treatment of MHRD, as it can promote MH closure, and significantly improve the outcome of MHRD. However if this method can improve the postoperative visual function is still controversial. The advantage of this technique is that the loosened internal limiting membrane is applied to cover the MH surface to form a scaffold structure similar to the basement membrane. It can stimulate Müller cell gliosis more effectively, and promote tissue filling in the MH which results in MH closure. It can also promote retinal reattachment and reduce the likelihood of retinal re-detachment. This technique is expected to be a standard surgical method for the treatment of MHRD of high myopia in the future. The inserted internal limiting membrane flap technique is relatively easy to perform, induces stable flaps by simple procedures, and can be an essential complement procedure of the inverted internal limiting membrane flap technique. In order to reduce the recurrence rate in the future, it is necessary to further define the indications of different surgical methods and the predictive effects of MH healing mode on the success rate and visual function recovery.
Objective To investigate the visual function of idiopathic macular hole before and after macular surgery. Methods Vitreous-retinal surgery was performed in 19 patients(19 eyes) with idiopthic macular hole.Among them,3 eyes belonged to stage 2,10 eyes stage 3 and 6 eyes were stage 4. The visual function including international standard visual acuity,laser retinal visual acuity,photopic ERG intensity responses- and Humphrey 30deg;-2 visual field were tested and compared before and after the surgery. Results (1)International standard visual acuity was from 0.01 to 0.1 preoperatively,and from 0.04 to 0.4 post operatively,the difference was not significant(P>0.0 5).(2)Laser retinal visual acuity was from 0.12 to 0.4 preoperatively,and from 0.2 to 0.63 postoperatively,the difference was significant (P<0.05).(3)In photopic ERG intensity responses:the Log K value of bwave decreased after surgery (P<0.05),but no obvious change found in Rmax and n Value after operation.(4)Humphrey visual field examination revealed the thresholds for 0-10deg; increased after surgery(P<0.05),but not obvious for 15-30deg;. Purpose To measure and compare the difference of multifocal electroretinogram in normal subjects and patients with age-related macular degeneration. Methods Seventeen cases(20 eyes)of normal subjects,7 cases(10 eyes)of dry form AMD(age-related macular degeneration),8 cases(8 eyes)of wet-form AMD and 11 cases(11 eyes)of idiopathic macular hole(IMH)were tested with VERIS SciencesTM 4.0 made by EDI company of America.The latencies and average response densities of 6 ring retinal regions in normal subjects were compared with those in various types of age-related maculopathies. Results The N1 and P1 wave latencies of all 6 rings in wet-form AMD and the N1 wave latencies of 3~6 rings in dry-form AMD were delayed statistically.The N1 and P1 wave average response densities of 1~4 rings in and the P1 wave average response densities of 1-6 rings wet-form AMD and the N 1 wave average response densities of 1~5 and the P1 wave average response densties of 1-6 rings in dry-form AMD were decreased statistically.The N1 and P1 wave average response densities of 1~2 and the P1 wave average esponse densities of 1~3 rings were decrease statistically in IMH. Conclusion Multifocal electroretinogram can be used to quantitate the visual function of the affected location in age-related macular degeneration. (Chin J Ocul Fundus Dis,2000,16:224-226)
Objective To observe the therapeutic effect of combined surgery of anterior and posterior segment and silicon oil tamponade on macular hole retinal detachment in eyes with high myopia.Methods The clinical data of 48 high myopia patients (48 eyes) with macular hole retinal detachment were retrospectively analyzed. Retinal detachment was mainly at the posterior pole; macular hole was confirmed by noncontact Hruby lens and optical coherence tomography (OCT). Phacoemulsification combined with pars plana vitrectomy and silicon oil tamponade were performed to all patients, of which 41 had undergone internal limiting membrane peeling, and 23 had intraocular lens implanting. The oil had been removed 3.5-48.0 months after the first surgery and OCT had been performed before the removal. The followup period after the removal of the silicon oil was more than 1 year.Results The edge of the macular hole could not be seen under the noncontact Hruby lens 1 week after the surgery in all but 5 patients, and the visual acuity improved. The silicon oil had been removed in all of the 48 patients; the OCT scan before the removal showed that the closed macular holes can be in U shape (8 eyes), V shape (6 eyes) or W shape (23 eyes). About 1338 months after the oil removal, retinal detachment recurred in 2 patients with the Wshaped holes. At the end of the followup period, 16 patients (33.3%) had U or Vshaped macular holes, and 32 patients (66.7%) had Wshaped macular holes. The rate of retinal reattachment was 100%.Conclusion Combined surgery of anterior and posterior segment and silicon oil tamponade is effective on macular hole retinal detachment in eyes with high myopia.
Recently, the new term "lamellar hole-associated epiretinal proliferation" was introduced to describe a specific epiretinal proliferation. Different from conventional epimacular retinal membrane, lamellar hole-associated epiretinal proliferation (LHEP) can be found around the edge of lamellar macular holes and part of full thickness macular holes. It is defined as a thick homogenous layer of yellowish material without any contractive properties on the epiretinal surface with medium reflectivity on optical coherence tomography images. Immunocytochemical analysis showed the presence of glial cells, fibroblasts, hyalocytes and collagen type Ⅱ. Electron microscopy revealed fibroblasts and hyalocytes as predominant cell types, densely packed in cell agglomerations. LHEP is a secondary event in lamellar macular hole formation and may represent a repair process after large and deep retinal defect. Further studies on its clinical features and clinical significance are still required.
Objective To evaluate the safety and efficacy of brilliant blue G (BBG) assisted internal limiting membrane (ILM) peeling on pathological myopic macular holes with retinal detachment.Methods This is a prospective and noncontrolled study. Twenty-seven high myopia patients (27 eyes) with macular holes and retinal detachment were enrolled. Routine examination was performed, including the best corrected visual acuity (BCVA), intraocular pressure, slit lamp microscope with +90 D pre-set lens, A- or B-ultrasound,optical coherence tomography (OCT) and visual field. All patients received vitrectomy with BBG-assisted ILM peeling and C3F8 gas tamponade. The 5 followup visits were at the first day, the seventh day, the first month, the third month and the sixth month after surgery. The BCVA, intraocular pressure, visual field, macular hole and retinal reattachment were comparatively analyzed.Results The ILM of all patients were peeled completely by BBG staining. There were no major complications such as corneal edema, anterior chamber reaction, elevated intraocular pressure, visual field defects. At the first month after surgery, macular hole closed and retina reattached in 26 eyes (96.3%), the macular hole did not close and retina redetached in one eye (25.9%). At the sixth month after surgery, BCVA of 25 eyes (92.6%) increased, two eyes (7.4%) didnprime;t change, the difference was statistically significant (t=6.08,Plt;0.05).Conclusions BBG can fully stain ILM without any side effects. Vitrectomy with BBG-assisted ILM peeling is a safe and effective treatment for pathological myopic macular holes with retinal detachment.
Idiopathic macular hole after the internal limiting membrane (ILM) is removed during surgery, the intraoperative optical coherence tomography can be used to observe the presence of debris tissue (RF) protruding into the vitreous cavity at the edge of the hole. Current studies suggest that RF may be caused by epiretinal proliferation and vitreomacular traction, but it is still controversial, and the influence of postoperative anatomical and functional recovery is not clear. Common points can still be found, some of the studies suggest that RF is not conducive to postoperative anatomical and functional recovery during the operation, ILM fragments remain on RF tissues after ILM peeling and re-staining. However, in some studies suggest that RF is beneficial to postoperative anatomical and functional recovery, and ILM fragments on RF are removed. This suggests that whether ILM is removed on RF lead to a certain influence on the postoperative efficacy. There are few researches on RF at present, so it is necessary to understand RF from its essence and assist judgment through histological analysis.
Objective To compare the outcome of pars plana vitrectomy (PPV) with triamcinolone (TA) assistance and internal limiting membrane (ILM) peeling for the treatment of moderate and extreme highly myopic macular hole retinal detachment (MHRD). Methods Forty-one highly myopic MHRD patients (41 eyes) who underwent PPV with TA assistance and ILM peeling were enrolled in this study. These eyes were divided into two groups according to different anatomic features: group A (24 eyes) had a consistent moderate long axial lengths (<29 mm), quot;mildquot; retinal pigment epithelium (RPE) and chorioretinal atrophy, and posterior staphyloma (level 0 - 1 and depth le;2 mm); while group B (17 eyes) had a consistent extreme long axial lengths (ge;29 mm), quot;severequot; RPE and chorioretinal atrophy, and posterior staphyloma (level 2 - 3 and depth>2 mm). All the patients underwent C3F8 tamponade at the end of PPV. The anatomic reattachment of the retina, macular hole closure, and visual acuity were observed at 12 months after surgery. Results The rates of retinal reattachment and macular hole closure were 91.67% and 58.33% in group A, 64.71% and 17.65 % in group B in the first time of surgery. The differences of rates of retinal reattachment (P=0.049) and macular hole closure (chi;2=6.787, P=0.009) between two groups were statistically significant. The rates of retinal reattachment and macular hole closure were 95.83% and 58.33% in group A, 88.23% and 29.53% in group B in the second time of surgery. The difference of retinal reattachment rate between two groups was not statistically significant (P=0.560). The difference of macular hole closure rate between two groups was statistically significant (chi;2=4.894, P=0.027). Twelve months after surgery, the vision acuity improved in 14 eyes, unchanged in nine eyes, and decreased in one eye in group A; the vision acuity improved in six eyes, unchanged in eight eyes, and decreased in three eyes in group B. The differences of vision result between two groups was not statistically significant (chi;2=0.209, P=0.647). Conclusion After PPV with TA assistance and ILM peeling, the rates of retinal reattachment and macular hole closure in eyes with moderate highly myopic MHRD are higher than that in eyes with extreme highly myopic MHRD, but there is no difference in visual acuity.
