  • Construction and validation of the associated depression risk prediction model in patients with type Ⅱ diabetes mellitus

    ObjectiveTo explore the risk factors for accompanying depression in patients with community type Ⅱ diabetes and to construct their risk prediction model. MethodsA total of 269 patients with type Ⅱ diabetes accompanied with depression and 217 patients with simple type Ⅱ diabetes from three community health service centers in two streets of Pingshan District, Shenzhen from October 2021 to April 2022 were included. The risk factors were analyzed and screened out, and a logistic regression risk prediction model was constructed. The goodness of fit and prediction ability of the model were tested by the Hosmer-Lemeshow test and the receiver operating characteristic (ROC) curve. Finally, the model was verified. ResultsLogistic regression analysis showed that smoking, diabetes complications, physical function, psychological dimension, medical coping for face, and medical coping for avoidance were independent risk factors for depressive disorder in patients with type Ⅱ diabetes. Modeling group Hosmer-Lemeshow test P=0.345, the area under the ROC curve was 0.987, sensitivity was 95.2% and specificity was 98.6%. The area under the ROC curve was 0.945, sensitivity was 89.8%, specificity was 84.8%, and accuracy was 86.8%, showing the model predictive value. ConclusionThe risk prediction model of type Ⅱ diabetes patients with depressive disorder constructed in this study has good predictive and discriminating ability.

  • Meta-analysis of risk factors for epilepsy in children

    Objective To systematically review and analyze the risk factors of epilepsy in children, to explore the related etiology of epilepsy in children, and to provide evidence-based strategies for reducing the occurrence of epilepsy in children. Methods PubMed, Embase, VIP, CNKI, Web of science, Cohrane, and CBM were searched for relevant studies on risk factors for childhood epilepsy since the establishment of the database, and the study type was selected as a case-control study. After screening and quality evaluation, the literatures that met the requirements were finally selected for inclusion in the study, and the extracted target data were statistically analyzed by RevMan5.3 software. Results A total of 9 literatures were included for Meta-analysis of the risk factors of epilepsy in children. A total of 3792 cases were studied, including 1 922 cases in the control group and 1 870 cases in the case group. Meta-analysis results showed that adverse perinatal period, family history of epilepsy, febrile seizures, and central nervous system infection were closely related to the occurrence of epilepsy in children, and the OR values were OR=3.46, 95%CI (2.51, 4.79), OR=4.77, 95%CI (3.83, 5.95), OR=7.81, 95%CI ( 5.64, 10.80), OR=3.00, 95%CI (1.44, 6.26), P values were all less than 0.05. Conclusions Adverse perinatal period, family history of epilepsy, febrile seizures, and central nervous system infection are the current risk factors for childhood epilepsy.

  • Analysis of Risk Factors for Ventilator-Associated Pneumonia in Mechanically Ventilated Patients in Respiratory Intensive Care Unit

    【Abstract】 Objective To analyze the risk factors for ventilator-associated pneumonia ( VAP) in respiratory intensive care unit ( RICU) , as well as the impact on mortality. Methods A retrospective cohort study was conducted in 105 patients who had received mechanical ventilation in RICUbetweenMay 2008 andJanuary 2010. The duration of intubation, vital signs, primary disease of respiratory failure and complications,blood biochemistry, blood routine tests, arterial blood gas analysis, APACHEⅡ score,medications, nutritional status, bronchoalveolar lavage ( BAL) , protected specimen brush ( PSB) quantitative culture, chest X-rayexamination were recorded and analyzed. Results The incidence rate of VAP was 32. 4% . Mortality in the VAP patients were significantly higher than those without VAP( 58. 8% vs. 28. 2% , P = 0. 007) . The duration of intubation, hypotension induced by intubation, cerebrovascular disease, and hypoalbuminemiawererisk factors for VAP in RICU. Conclusions Mortality of the patients with VAP increased obviously. The risk factors for VAP in RICU were the duration of intubation, hypotension after intubation, cerebrovascular disease, and hypoalbuminemia.

  • The Health Status of Elderly Peptic Ulcer Patients and Its Influencing Factors in Haikou City

    ObjectiveTo investigate the health status of elderly peptic ulcer patients in Haikou city and its influencing factors. MethodsSeventy-eight elderly peptic ulcer patients treated in the Department of Gastroenterology, Haikou Hospital between February 2012 and December 2014 were chosen to be the observation group, and at the same time, another 78 healthy elderly people examined in the same hospital were designated as the control group. The general information and health status of the study subjects were investigated for correlation analysis. ResultsThe health status, physical function, body pain, life quality and mental health scores in the observation group were significantly lower than those in the control group (P<0.05). Univariate analysis showed that age, smoking, high-salt diet, body mass index (BMI) were significantly associated with the incidence of peptic ulcer (P<0.05). Multivariate logistic regression analysis showed that age, smoking, high-salt diet, BMI were all independent risk factors for peptic ulcer occurrence (P<0.05). ConclusionThe health status of elderly peptic ulcer patients in Haikou city is relatively low. Age, smoking, high-salt diet and BMI are the major independent risk factors for the occurrence of peptic ulcer. We need to actively strengthen symptomatic prevention and intervention.

  • Etiology and Risk Factors of Late-Onset Hospital-Acquired Pneumonia in Respiratory Intensive Care Unit

    Objective To analyze the etiology, risk factors, and prognosis of late-onset hospitalacquired pneumonia ( L-HAP) in respiratory ICU. Methods In this retrospective case control study, 30 L-HAP patients and 30 patients without HAP in respiratory ICU were enrolled to investigate the features and risk factors of L-HAP. Stratification was made according to the onset time of L-HAP. The etiology and pathogen distribution at each stage were described and analyzed. Results Univariate analysis revealed thatunconsciousness, aspiration, mechanical ventilation, hypoalbuminemia, and long-term use of proton pump inhibitor were significantly associated with L-HAP. Logistic regression analysis revealed that mechanical ventilation( OR = 8. 7) and hypoalbuminemia ( OR = 20. 4) were independent risk factors for L-HAP. The L-HAP patients had longer stay in hospital, long-termantibiotic use, and higher mortality compared with the patients without HAP. For the patients whose L-HAP onset time within 6-14 days, the dominated pathogens were Acinetobacter baumannii and Klebsiella pneumonia. For those within 15-28 days, the dominated pathogens were Pseudomonas aeruginosa, Acinetobacter baumanni, and Staphylococcus aureus. For those beyond 29 days, the dominated pathogens were Pseudomonas aeruginosa and Stenotrophomonas maltophilia. Conclusions Mechanical ventilation and hypoalbuminemia are independent risk factors for L-HAP. The pathogen features of L-HAP are quite different at different inhospital stage.

  • Assessment of Inhospital Mortality Risk Factors in the Patients Undergoing Offpump Coronary Artery Bypass Grafting

    Abstract: Objective To explore the inhospital mortalityrelated risk factors in the patients undergoing offpump coronary artery bypass grafting (OPCAB). Methods We retrospectively analyzed the clinical data of 215 patients undergoing OPCAB in our hospital from November 2007 to November 2008. There were 171 males and 44 females aged between 40 and 85 years old. Among them, there were 47 patients older than 70 years old. All of them were coronary artery disease (CAD) patients with triple vessel disease. We adopted univarialble analysis and logistic multivariable regression analysis to screen the risk factors for the mortality of OPCAB. Results Six patients died in hospital after OPCAB with a mortality rate of 2.79% (6/215). No renal dysfunction or respiratory failure occurred. The rate of reoperation for bleeding was 4.65% (10/215) and all the 10 patients having undergone reoperation were alive. A total of 209 patients were all alive after 1year follow-up. The results of logistic multivariable regression analysis showed that New York Heart Association (NYHA) Ⅲ and Ⅳ heart function (OR=42.116,95% CI 3.319 to 534.465,P=0.004) and mechanical ventilation duration (OR=1.007,95%CI 1.001 to 1.013,P=0.028) were independent risk factors for inhospital mortality of OPCAB. Conclusion OPCAB is an effective and safe treatment for CAD with triple vessel disease. NYHA Ⅲ and Ⅳ heart function and mechanical ventilation time after OPCAB are the risk factors for OPCAB inhospital mortality, yet, needs further study with large sample.

  • Factor analysis of secondary intraocular hypertension after intravitreal dexamethasone implantation in patients with diabetic macular edema

    Objective To observe and analyze the risk factors of secondary intraocular hypertension in diabetic macular edema (DME) patients after treatment with dexamethasone vitreous cavity implant (DEX). MethodsA retrospective observational study. A total of 352 patients with type 2 diabetes mellitus (T2DM) secondary macular edema diagnosed by ophthalmic examination and treated with DEX in Department of Ophthalmology of Harbin 242 Hospital from January 2016 to March 2022 were included in the study. Among them, 221 were males and 131 were females, with the mean age of (55.56±8.09) years. There were 194 patients with disseminated macular edema, 158 patients with cystoid macular edema. All patients underwent vitreous cavity implantation of DEX. Intraocular pressure (IOP) was measured once a month for 3 months after treatment, with IOP over than 25 mm Hg (1 mm Hg=0.133 kPa) or higher than 10 mm Hg from baseline as secondary intraocular hypertension. The relevant clinical data were collected, and the risk factors of secondary intraocular hypertension in DME patients after DEX treatment were analyzed by binary logistic regression. ResultsAmong 352 patients, 116 patients (32.95%, 116/352) were in the intraocular hypertension. Among them, 29 patients (25.00%, 29/116), 69 patients (59.48%, 69/116) and 18 patients (15.52%, 18/116) occurred intraocular hypertension at 1, 2 and 3 months after treatment, respectively. Compared with the normal IOP group, the IOP in the intraocular hypertension group increased significantly at 1, 2 and 3 months after treatment, with statistical significance (t=10.771, 21.116, 13.761; P<0.001). Compared with normal IOP group, the patients in the intraocular hypertension group had younger age (t=6.967), longer duration of diabetes (t=5.950), longer axial length (AL) (t=14.989), higher proportion of DME grade 3 (Z=6.284), higher proportion of DEX implantation in pars plana (χ2=23.275), and higher HbA1c level (t=10.764), the differences were statistically significant (P<0.05). Logistic regression analysis showed that longer AL [odds ratio (OR)=1.428, 95% confidence interval (CI) 1.054-1.934], DEX implantation in pars plana (OR=1.358, 95%CI 1.063-1.735), and higher HbA1c (OR=1.702, 95%CI 1.225-2.366) were the risk factors for secondary intraocular hypertension in DME patients after DEX treatment (P<0.05), older age was a protective factor (OR=0.548, 95%CI 0.380-0.789, P<0.05). ConclusionsLong AL, DEX implantation in pars plana and high HbA1c are the risk factors for secondary intraocular hypertension after DEX treatment in DME patients, older age is a protective factor.

  • Prevalence and risk factors of diabetic retinopathy in Guangdong provincial civil servants

    Objective To learn the prevalence and risk factors of diabetic retinopathy (DR) of provincial civil servants (more than 40 years old) in Guangdong, China. Methods Total of 3844 Guangdong provincial civil servants over 40 years old were enrolled in this study. All participants diagnosed with diabetes underwent a complete ophthalmologic examination, including stereoscopic fundus photography and optical coherence tomography. Photographs were graded in a masked manner to assess the presence and severity of DR and diabetic macular edema (DME). Results Of 3844 civil servants, DM was confirmed in 155 patients, the prevalence was 4.03%. Of 155 DM patients, DR was in 11 patients, the DR prevalence was 7.10%. There were statistically differences on the fasting blood glucose (FBG), total cholesterol, low density lipoprotein and whether or not to use insulin between DR group and non DR group (t=3.158, 2.200, 2.050;chi;2=5.128; P<0.05). High-FBG values, high blood lipids, INS applications had accelerated the occurrence and development of DR. The high FBG and the insulin using were the independent risk factors of DR. Conclusions DR prevalence is 7.10% in Guangdong provincial civil servants more than 40 years old. The high FBG and the insulin using are the independent risk factors of DR.

  • Analysis of risk factors of severe retinopathy of prematurity

    ObjectiveTo analyze the risk factors of severe retinopathy of prematurity (ROP) and provide consultable evidence for the rational establishment of screening standard.MethodsThe clinical data of 168 prematureinfants (gestational age less than 37 weeks) who was diagnosed in our department from Dec 2002 to Apr 2004 was analyzed retrospectively. Gender, birth count (BC), gestational age (GA), birth weight (BW), duration of oxygen therapy and vascularization devlopment of posterior and peripheral retina examined by binocular indirect ophthalmoscope after mydriasis were recorded. The results were recorded by the international classification of ROP (ICROP), and stage 1, 2 and 3 were mild ROP while threshold disease, stage 4 and 5 were severe ROP. Logistic regression was appliedto analyze the relationship of ROP and gender, BC, GA, BW, and oxygen therapy. ResultsSevere ROP was found in 91 eyes (27.1%) of 47 infants (28.0%) in 336 eyes of 168 premature infants, including threshold disease in 20 eyes (6.0%) and disease at stage 4 in 11 eyes (3.3%) in which the diseases at stage 4A was foundin 2 eyes (0.6%) and stage 4B in 9 eyes (2.7%). There were 60 eyes (17.8%) at stage 5. In all of the factors, GA, BW and oxygen therapy were found to have a significant impact on severe ROP (P=0.000, 0.000 and 0.015,α=0.05) while gender and BC were not (P=0.640 and 0.084, α=0.05). Statistic analysis of subgroupshowed that the risk of severe ROP in premature infants would increase significantly when GA≤30 weeks, BW≤1500 g or oxygen therapy gt;4 days. Conclusions Severe ROP relates to GA, BW and oxygen therapy instead of gender and BC. The risk of occurrence of severe ROP in premature infants increases significantly when GA≤30 weeks, BW≤1500g or oxygen therapy gt;4 days, so it is recommended to screen such premature infants carefully. (Chin J Ocul Fundus Dis,2005,21:271-274)

  • Clinical analysis of the relationship between retinopathy of prematurity and bronchopulmonary dysplasia

    ObjectiveTo investigate the clinical risk factors of preterm infants and its severity in premature infants with bronchopulmonary dysplasia (BPD) with retinopathy of prematurity (ROP).MethodsRetrospective clinical study was performed. A total of 126 preterm infants with BPD in the Neonatal Department of the Affiliated Hospital of Qingdao University from January 2016 to December 2018 were enrolled in the study. Among them, 69 were males and 57 were females, whose gestational age<32 weeks and birth weight<1500 g. BPD grades Ⅰ, Ⅱ, and Ⅲ were 63, 40, and 23 cases respectively. According to the presence or absence of ROP, children were divided into ROP group and non-ROP group, with 48 (38.1%) and 78 (61.9%) cases respectively. The differences of clinical data between the two groups were compared and analyzed. Quantitative data comparison between groups was performed by t test, and count data comparison was performed by χ2 test. The risk factors of ROP in BPD premature infants were analyzed by multi-factor logistics regression. The correlation between BPD severity and ROP severity was tested by Spearman rank correlation test.ResultsCompared with the non-ROP group, the ROP group had a smaller gestational age (t=5.988), lower birth weight (t=7.371), higher the application rate of oxygen concentration>30% (duration of service>24 h), high rate (χ2=17.244) and longer noninvasive ventilation time (t=-7.139), the differences were statistically significant (P<0.05). In the logistic regression model, the noninvasive ventilation time was the risk factor for ROP in preterm infants with BPD (OR≈1.054, P<0.05), while gestational age and birth weight were importantly protective factors for ROP in preterm infants with BPD (OR≈0.938, 0.996; P<0.05). The results of the correlation analysis found that the severity of BPD was significantly positively correlated with the severity of ROP. As the severity of BPD increased, the severity of ROP increased, and the difference was statistically significant (rs=0.306, P<0.035).ConclusionsFetal gestational age, low birth weight, hyperoxia, and long-term non-invasive mechanical ventilation are the main risk factors for ROP in preterm infants with BPD. The severity of BPD is positively correlated with the incidence and severity of ROP.
