  • ADVANCE ON REPAIR OF GROWTH PLATE INJURY

    OBJECTIVE: To sum up the studying course and latter development of repair of injury of growth plate. METHODS: Recent original articles about repair of injury of growth plate were extensively reviewed, focused on the progresses in understanding repair of injury of growth plate and comparison of several major reparative methods. RESULTS: Repair of injury of growth plate is a great difficulty in experimental study and clinical treatment of pediatric orthopedics. Graft of free growth plate and cartilage were unfavorably used because of lack of blood supplement. Although graft of vascularized growth plate solved circulation problem, both two kinds of grafts were involved in limitation of donor and immunologic reaction. Non-cartilaginous tissue and material could only prevent formation of bony bridge in small defect of growth plate and lacked ability of regenerative repair. Transfer of tissue engineered cartilage might be the best choice for repair of injury of growth plate. CONCLUSION: Considering source of transplanted material, reparative effect and adverse reaction, repair of injury of growth plate with tissue engineered cartilage deserves further investigation.

  • Evaluation of the Design and Statistical Methods of Clinical Studies Published in Chinese Journal of Conservative Dentistry of 2002

    Objective To evaluate the quality of clinical studies on dentistry from the Chinese Journals. Methods Clinical studies in Chinese Journal of Conservative Dentistry of 2002 were searched. The quality of the clinical studies on assessment of treatments’ efficacy was evaluated. Results Among 204 related studies from 12 issues, there were 93 (45.58%) restrospective intervention studies, 6 randomized controlled blinded trials (2.94%), 42 randomized trials without blindness (20.58%), 20 controlled trials without randomization (9.80%) and 25 clinical observational studies (12.25%). The statistical analysis showed that 20 studies were with inappropriate methods. Conclusions It is necessary to improve the design and statistical analysis of clinical studies on stomatology in China to produce high-quality research evidence.

  • The Prevalence of Metabolic Syndrome and Correlation of Waist-to-hip Ratio and Body Mass Index with Metabolic Syndrome in Certain University

    Objective We aimed to describe the prevalence of metabolic syndrome, its epidemiological characteristics, and to analyse the relationship of waist-to-hip ratio (WHR) and body mass index (BMI) with metabolic syndrome (MS) among staff at Southeast University. Methods The data from the overall physical examination of 1979 staff were analyzed.Results The crude prevalence of MS were 21.7%,26.4% and 14.2% in the whole population, men and women respectively. The standardized rates were 14.7%,19.0% and 9.4%. The prevalence of MS in men was significantly higher than that in women(Plt;0.05). Both abdominal obesity and visceral obesity were positively correlated with the prevalence of MS(r=0.295, 0.248, P=0.000). Conclusion The prevalence of MS among staff of Southeast University has shown a significant increase in 2006. WHR and BMI are both correlated with the prevalence of MS.

  • Awareness of thrombolytic therapy for acute ischemic stroke in inpatients with a history of stroke and with a high risk of stroke

    Objective To explore the awareness of thrombolytic therapy for acute ischemic stroke in inpatients with a history of stroke and with a high risk of stroke. Methods From January to August 2012, using self-designed questionnaire, trained neurologists conducted the face to face investigation in 500 inpatients with a high risk of stroke, including those with a history of stroke in Department of Neurology in the Second Affiliated Hospital of Chongqing Medical University. Results A total of 467 valid questionnaires were recovered. Only 16.1% (75/467) patients were aware of thrombolytic therapy for acute stroke, of whom 50.7% (38/75) knew the time window of thrombolytic therapy. Awareness of thrombolytic therapy was higher in patients aged 56-70 years, with a higher level of education and income, and in those who knew at least 3 stroke warning signs and those with a history of stroke. While awareness of the time window of thrombolytic therapy was higher in those unmarried or widowed and with a history of stroke. Multiple logistic regression analysis showed that awareness of thrombolytic therapy was independently associated with age, education level, knowledge of stroke warning signs and a history of stroke; awareness of the time window was associated with marital status and a history of stroke (P<0.05). Conclusions Inpatients with a history of stroke and with a high risk of stroke in the Department of Neurology have poor awareness of thrombolytic therapy for acute ischemic stroke. It is necessary to improve the level of patients’ knowledge about thrombolytic therapy for acute stroke by health education.

  • Relationship between circulating endothelial cell number,plasma endothelin level and diabetic retinopathy

    Objective To investigate the effect of the damage and functional change of vascular endothelial cells (VEC) on diabetic retinopathy(DR). Methods Circulating endothelial cell (CEC) number and plasma endothelin(ET) level were measured in 18 normal control subjects and 55 patients with diabetes mellitus (DM) consisting of 20 cases of DM with out retinopathy,20 cases of DM with-background diabetic retinopathy and 15 cases of DM with proliferative diabetic retinopathy. Results CEC number and plasma ET level in DM were significantly higher than those in normal subjects(Plt;0.001)respectively.With the progression of DR,CEC number was significantly elevated and plasma ET level was gradually elevated.There was significant positive correlation between CEC number and plasma ET level (r=0.738,Plt;0.001,n=55). Conclusion VEC damage and elevated plasma ET level induced by VEC damage may play an important role in the development and progression of DR. (Chin J Ocul Fundus Dis,2000,16:166-168)

  • Construction of Tumor Vaccine Fused from Dendritic Cells and Walker-256 Cells and Its Effect on Implanted Liver Cancer in Rats

    ObjectiveTo explore the antitumor effect of tumor vaccine fused from dendritic cells (DC) and Walker-256 cancer cells on implanted liver cancer in rats and the related mechanism of inhibition for tumor angiogenesis. MethodsWalker-256 cancer cells and mature DC were fused by 50% polyethylene glycol method for preparation of DC-Walker-256 fusion vaccines. Implanted liver cancer models were established through operations on healthy male SD rats at the age of 6-8 weeks. All the rats were divided into four groups, and rats in each group were injected subcutanely with fusion vaccine (group), mixed cultured cells (group), simple DC (group), and PBS (blank control group), respectively. On 28 d after making model, the rats were put to death, the tumor was observed and pathological essays were prepared. All rats’ spleens were collected and prepared into lymphocyte to detect antigenic specificity cytotoxic T lymphocyte (CTL) by enzymelinked immunosorbent spot (ELISPOT) method. The expressions of VEGF, ANG-1, ANG-2, and MVD were detected by immunohistochemistry. ResultsThe numbers of rats survived in the fusion vaccine group, mixed culture cells group, simple DC group, and blank control group was 8, 5, 6, and 3, respectively. The rats in the other three groups except for fusion vaccine group were manifested as inaction, anorexia, and gloomy fur in some degree as well as ascites. The tumorigenesis was found in all survival rats except for two in the fusion vaccine group. The weight of liver tumors of rats in the fusion vaccine group 〔(32.4±9.2) g〕 was significantly lighter than that in the mixed culture cells group 〔(67.3±5.1) g, P=0.031〕, simple DC group 〔(75.0±8.3) g, P=0.019〕, and blank control group 〔(86.6±10.5) g, P=0.008〕, respectively. The number of tumorspecific CTL of rats in the fusion vaccine group was also significantly higher than that in the other three groups (P=0.019, P=0.025, and P=0.001, respectively). The MVD of tumor tissue in the fusion vaccine group was (24.12±2.32) vessels/HP, which was significantly lower than that in the mixed culture cells group 〔(40.34±1.29) vessels/HP, P=0.025〕, simple DC group 〔(42.36±3.16) vessels/HP, P=0.035〕, and blank control group 〔(56.48±5.16) vessels/HP, P=0.006〕, respectively. The MVD of tumor tissue in the mixed cultured cells group and simple DC group was similar (P=0.165), however, which was significantly lower than that in the blank control group (P=0.040 and P=0.043). The positive rate of VEGFA protein expression was 23.2% in the fusion vaccine group, which was significantly lower than that in the mixed culture cells group (42.5%, P=0.031), simple DC group (61.3%, P=0.019), and blank control group (89.6%, P=0.003), respectively. The positive rate of VEGF-A protein expression in the mixed cultured cells and simple DC groups was similar (P=0.089), however, which was significantly lower than that in the blank control group (P=0.027 and P=0.038). The positive rate of ANG-1 protein expression in the fusion vaccine group (43.2%) was not different from that in the mixed culture cells group (46.3%, P=0.292), simple DC group (51.3%, P=0.183), or blank control group (49.6%, P=0.179), respectively, and the difference of pairwise comparison in latter three groups was not significant (P=0.242, P=0.347, and P=0.182). The positive rate of ANG2 protein expression was 19.2% in the fusion vaccine group, which was significantly lower than that in the mixed culture cells group (62.3%, P=0.007), simple DC group (67.3%, P=0.005), and blank control group (71.6%, P=0.004), respectively, however, the difference of pairwise comparison in latter three groups was not significant (P=0.634, P=0.483, and P=0.379). ConclusionFused vaccine can induce CD8+ CTL aiming at tumor cells and establish the effective antitumor immunity in vivo and also downregulate the level of VEGF and ANG-2 to suppress tumor angiogenesis and thereby achieve the purpose of curing tumor.

  • EFFECT OF RIVAROXABAN ON RISK OF BLEEDING AFTER TOTAL KNEE ARTHROPLASTY

    Objective To investigate the effect of rivaroxaban on the risk of bleeding after total knee arthroplasty (TKA). Methods A total of 119 cases undergoing primary TKA because of knee osteoarthritis between June 2009 and May 2011, were randomly divided into the rivaroxaban group (59 cases) and the control group (60 cases). There was no significant difference in gender, age, height, weight, side, disease duration, and grade of osteoarthritis between 2 groups (P gt; 0.05). Thepreoperative preparation and operative procedure of 2 groups were concordant. At 1-14 days after TKA, rivaroxaban 10 mg/d were taken orally in the rivaroxaban group, and placebo were given in the control group. The blood routine examination was performed before operation and at 2 days postoperatively; the total blood loss and hemoglobin (HGB) decrease were calculated according to the formula; the blood loss, postoperative wound drainage, and wound exudate after extubation were recorded to calculate the dominant amount of blood loss; and the bleeding events were recorded within 35 days postoperatively. Results The total blood loss and HGB decrease were (1 198.34 ± 222.06) mL and (33.29 ± 4.99) g/L in the rivaroxaban group and were (1 124.43 ± 261.01) mL and (31.57 ± 6.17) g/L in the control group, showing no significant difference (P gt; 0.05); the postoperative dominant blood loss in the rivaroxaban group [(456.22 ± 133.12) mL] was significantly higher than that in the control group [(354.53 ± 96.71) mL] (t=4.773, P=0.000). The bleeding events occurred in 3 cases (5.1%) of the rivaroxaban group and in 1 case (1.7%) of the control group, showing no significant difference (χ2=1.070, P=0.301). Conclusion Rivaroxaban has some effects on the risk of bleeding after TKA. In general, rivaroxaban is safe.

  • Experience on Diagnosis and Treatment of 69 Cases of Primary Retroperitoneal Tumors

    Objective To summarize the experience in diagnosis and treatment of primary retroperitoneal tumor (PRT). Methods Clinical data of 69 patients with PRT from June 1998 to June 2008 were analyzed retrospectively. Results Bellyache, abdominal distention, and abdominal mass were common symptoms in the patients with PRT. Ultrasound, CT, and MRI examination were effective. The major histopathological classification was soft tissue tumor, germinoma, lymphatic hematopoietic system tumors, and other rare tumors. Complete resection of tumors was performed in 42 cases, combined organs resection in 10 cases, partial resection of tumors in 11 cases, and only biopsy in 6 cases. There were 10 cases of intraoperative vessel and organ injury, which were treated by repair or vessels suturing and combined organ resection. Postoperative complications occurred in 8 cases, which were cured by conservative treatment. One patient died of hemorrhage acute stress ulcer combined multiple organ failure. The survival rates of 1, 3, and 5 years in patients underwent complete resection of tumors were 71%, 64%, and 46%, respectively. Of 11 patients underwent partial resection of tumors, 8 cases died within one year, and 3 cases died within 3 years after operation. All malignant tumor patients treated by biopsy died within one year after operation.Conclusion Synthetically using imaging examination may diagnose definitely, and to resect tumors as much as possible will improve patients’ survival.

  • The Application of K6 and SCL-8AD among Inpatients in A General Hospital

    目的 初步测试中文版情绪障碍快速筛查量表K6、SCL-8AD及其分量表(SCL-8、SCL-ANX4及SCL-DEP6)在综合医院内科和精神科住院患者中的适用性。 方法 2011年11月-12月对四川大学华西医院内科(心脏内科、消化内科、风湿免疫科、内分泌科、神经内科)和精神科的住院患者共373例应用由K6和SCL-8AD量表构成的问卷进行自评,分析总问卷、各量表及分量表的信度及相互之间的相关性;在各科患者间进行评分结果差异性比较,并分别将内科及精神科评分结果与住院日进行相关分析。 结果 各量表及全问卷的Cronbach’s α系数和分半系数>0.75(内科:0.802~0.952,精神科:0.774~0.944);其中,K6及SCL-8的Cronbach’s α系数和分半系数在内科及精神科均≥0.817。总问卷、各量表及分量表相互之间均存在较高相关性(r≥0.676);其中,K6及SCL-8与问卷总分均有极高相关性(r≥0.959)。问卷总分、各量表及分量表评分内科组均低于精神科组。 结论 条目少、操作简便的中文版K6和SCL-8量表内部一致性高、其评定结果能较全面反应综合医院内科及精神科住院患者中不同程度的焦虑及抑郁情绪,值得对其进行更深入研究,以便推广应用于非精神科患者情绪障碍的快速筛查。
