Meibomian gland dysfunction (MGD) is a common ocular surface disease, and the pathogenesis of MGD is closely related to the alteration of meibomian gland morphology and (or) function. At present, the treatment strategy for MGD consists of physical therapy, artificial lubricants, topical and systemic antibiotics and anti-inflammatory agents, topical steroids and Ω-3 dietary supplementation. Recently, the thermal pulsation system (LipiFlow) has been used clinically, which allows heat to be applied to the palpebral surfaces of the upper and lower eyelids directly over the meibomian glands, while simultaneously applying pulsatile pressure to the outer eyelid surfaces. Due to its specific design, the cornea and eyeball can successfully avoid the potential damage from overheating and improper expression, which will greatly improve the safety during the clinical application and patients’ compliance. Studies have showed that the effectiveness of LipiFlow when treating MGD is at least as well as twice-daily lid warming and massage for 3 months, which displays a broad application prospect. At present, the application of LipiFlow in China has not been fully developed, and only some hospitals have experimental applications. This paper summarizes the research progress of LipiFlow treatment in MGD.
ObjectiveTo introduce the method and preliminary experience of robot-assisted bilateral internal mammary arteries (BIMA) harvesting for off-pump coronary artery bypass grafting (OPCAB) with 5 grafts via left anterolateral minithoracotomy.MethodsBIMA were harvested using the da Vinci robotic surgical system, and the right internal mammary artery (RIMA) was pulled out of the thoracic cavity through right second intercostal space. Intercepting the distal part of the RIMA for the BIMA composite Lima-Rima Y graft and anastomosing the great saphenous vein with remaining RIMA end to end. The Y graft anastomosed with left anterior descending (LAD) branch and diagonal branches (DIAG), artery-vein graft sequentially anastomosed with blunt round branch, left ventricular posterior branch and posterior descending branch.ResultsThe operation succeeded without hemodynamic instability and intra aortic balloon pump (IABP) implantation or cardiopulmonary bypass. The blood flow of Y graft was 24 mL/min, and the blood flow of artery-vein graft was 30 mL/min. Ventilator assistance time was 35 hours, ICU staying time was 62 hours, and postoperative myocardial enzymes increased temporarily. Postoperative coronary CTA showed that all the grafts were patency, and cardiac ultrasound indicated that the heart function was normal. The patient cured and discharged from hospital 7 days after operation.ConclusionRobot-assisted bilateral internal mammary artery harvesting for OPCAB with 5 grafts via left anterolateral minithoracotomy is feasible, which can achieve complete revascularization.
ObjectiveTo explore the significance of ultrasound combined with magnetic resonance imaging (MRI) in the diagnosis of pernicious placenta previa (PPP).MethodsA total of 101 patients with suspected PPP admitted to Chengdu Women’s and Children’s Central Hospital from January 2016 to February 2019 were retrospectively analyzed. The surgical results were used as the gold standard, and the diagnostic value of ultrasound, MRI, ultrasound combined with MRI was compared.ResultsSurgery results showed that among the 101 patients with suspected PPP, 79 patients had positive PPP diagnosis and 22 patients had negative PPP diagnosis. There was no significant difference in the diagnostic sensitivity, negative predictive value, or diagnostic coincidence rate of PPP among ultrasound, MRI, and ultrasound combined with MRI (P>0.05). The diagnostic specificity of the series combination of ultrasound and MRI (86.36%) was higher than that of ultrasound (59.10%), MRI (59.10%), and the parallel combination of ultrasound and MRI (31.32%), and the differences were statistically significant (P<0.05). The diagnostic coincidence rates of abnormal placental attachment of MRI (96.20%) and the parallel combination of ultrasound and MRI (96.20%) were higher than that of ultrasound (87.34%) and the series combination of ultrasound and MRI (87.34%) (P<0.05).ConclusionsUltrasound combined with MRI has a high diagnostic value for PPP. If necessary, ultrasound combined with MRI can be considered to improve the diagnostic efficiency.
Objective To compare vein graft patency after endoscopic great saphenous vein harvesting (EVH) and conventional open saphenous veinharvesting (OVH) in coronary artery bypass grafting (CABG), and to identify risk factors for vein graft stenosis. Methods The great saphenous vein was harvested using an EVH method in 60 patients, 34 males and 26 females, who underwent CABG in the General Hospital of PLA between May 2006 and May 2009. The mean patient age was 66.6±9.2 years in the EVH group.The OVH group had 60 patients (40 males and 20 females with a mean age of 65.7±10.6 years), chosen to match the clinical characteristics of the EVH group. 64multislice computed tomography (64MSCT) was used to evaluate vein graft patency at six months and two years postoperatively. We also collected and analyzeddata on possible risk factors for vein graft stenosis. Results The harvesting time was longer in the EVH group than in the OVH group (52.5±13.3 minutes vs. 36.1±18.0 minutes, t=2.13, P<0.05). The EVH group requireda greater number of repairs to veins than did the OVH group (2.2±13 vs. 0.9±0.6,t=2.60, P<0.05). There were no statistically significant differences invein length, number of vein grafts, or vein graft flow between the two groups. There was also no significant difference in vein graft patency between the EVH and OVH groups at six months postoperatively (96.2% vs. 94.5%) or at two years postoperatively (90.2% vs. 91.5%). The average of vein graft blood flow was a riskfactor for vein graft stenosis(t=2.61, P=001). Conclusion The EVH vein graft had a good patency rate at six months and two years after the surgery.
ObjectiveTo summarize the mechanism of CD147 in breast cancer invasion and metastasis, treatment, and drug resistance so as to provide reference for clinical decision-making.MethodThe relevant literatures about studies of CD147 in breast cancer in recent years were reviewed.ResultsCD147 was widely distributed in vivo and highly expressed in malignant tumor tissues. CD147 promoted matrix metalloproteinases and vascular endothelial growth factor productions and tumor microenvironment generation by extracellular matrix in breast cancer through different mechanisms. It degraded extracellular matrix and stimulated neovascularization to promote tumor invasion and metastasis. Related studies had shown that CD147 was highly expressed in the breast cancer tissues and which was associated with tumor grade and prognosis in patients with breast cancer, and it was a biological marker for diagnosis of breast cancer. However, a large of drugs targeted for CD147 and its involved pathways didn’t well benefit patient with breast cancer due to the failure of clinical trials and chemotherapy resistance.ConclusionsCD147 plays a key role in development, invasion and metastasis, diagnosis and treatment, and drug resistance of breast cancer, as well as guiding the treatment and prognosis of patients. However, benefits are poor, and relevant molecular mechanisms of action are limited.
Objective To explore the interaction of postmastectomy radiotherapy (PMRT) and breast reconstruction, and elucidate how to choose the type and timing of breast reconstruction. Method Literatures about PMRT and breast reconstruction were reviewed. Results PMRT might increase the incidence of complications and impair the cosmetic satisfaction of breast reconstruction. Breast reconstruction might also compromise the effect of PMRT. Conclusions In patients who will receive or have already received PMRT, the optimal approach is delayed autologous tissue reconstruction after PMRT. If PMRT appears likely but may not be required at the time of mastectomy,delayed-immediate reconstruction may be considered, or immediate autologous tissue reconstruction may be considered in case of patients awareness of the increased complications and impaired cosmetic outcomes from PMRT.
ObjectiveTo analyze the clinical characteristics of coronavirus disease 2019 (COVID-19), identify the high-risk factors which promoted the disease progression into severe or critical cases, and provide clinical guidance.MethodsNinety-six cases of clinically diagnosed coronavirus disease 2019 were collected and analyzed from General Hospital of the Yangtse River Shipping from January 20 to February 14, 2020. According to clinical classification, those cases were divided into two groups: mild group, including common and mild cases, and severe group including severe and critical cases.ResultsThere were more than half of patients ages>60 years old and severe or critical illness (61.5%, 56.3%), and obese patients accounts for 45.8%, the complication of hypertension accounts for 52.1%. The average time from onset to hospitalization was (6.1±2.6) days, and the average hospitalization days were (15.7±6.3) days. Compared with the mild group, the number of male patients in the severe group was more (57.4% vs. 35.7%, P<0.05), the numbers of elderly patients were larger (77.8% vs. 40.5%, P<0.01), and more obese peoples (55.6% vs. 33.3%, P<0.05). Cough symptom was more common in clinical presentation in the severe group (81.5% vs. 61.9%, P< 0.05). Patients with the complication of hypertension or diabetes occupied a higher proportion in the severe group, which were 64.8% vs. 35.7% and 35.2% vs. 14.3% (P< 0.05), respectively. And the rate of hypoxemia in admission of the severe group was significantly higher (79.6% vs. 9.5%, P< 0.01).ConclusionsThe cases of ages>60 years old and severe or critical illness in coronavirus disease 2019 occupy a higher proportion, and the complication of hypertension accounts for high proportion. Men, ages>60 years old, obese, coughing symptoms, with hypertension and / or diabetes, hypoxemia upon admission are high risk factors for progression into severe and critical cases.
ObjectiveTo explore the value of 3.0 T MRI functional imaging in differential diagnosis of radiation brain injury and recurrence of glioblastoma multiforme.MethodsFrom March 2017 to January 2018, 31 patients diagnosed with brain glioblastoma multiforme in Peking University International Hospital were collected continuously, including 14 cases of tumor recurrence and 17 cases of radiation-induced brain injury. All the patients routinely underwent conventional MRI head scan, three-dimension arterial spin labeling (3D-ASL), dynamic susceptibility contrastperfusion weighted imaging (DSC-PWI), and enhanced MRI scan sequence; related parameters were recorded and compared.ResultsCerebral blood flow (CBF) value of abnormal enhanced area in the recurrence group was significantly higher than that in the brain injury group with 3D-ASL scan (t=3.016, P=0.005), and no difference was found in edema area between the two groups (P>0.05). In the recurrence group, CBF value of abnormal enhanced area was significantly higher than that of the normal area (t=2.628, P=0.014); however, there was no significant difference in the CBF value between the abnormal enhancement foci and the normal areas in the radiation brain injury group (P>0.05). Relative cerebral blood volume (rCBV) ratio (t=2.894, P=0.007) and relative cerebral blood volume (rCBF) ratio (t=2.694, P=0.012) of abnormal enhanced area, as well as rCBV ratio (t=2.622, P=0.013) and rCBF ratio (t=2.775, P=0.010) of edema area in the recurrence group were significantly higher than those in the brain injury group with DSC-PWI scan. No differences were found in relative mean transit time (rMTT) ratio and relative time to peak (rTTP) ratio between the two groups (P>0.05). In the brain injury groupr, CBV ratio (t=2.921, P=0.008) and rCBF ratio (t=3.100, P=0.004) of abnormal enhanced area were significantly higher than those of the edema area, and no difference was found in rMTT ratio or rTTP ratio (P>0.05). In the recurrence group, no difference was found in all focal parameters between abnormal enhanced area and edema area (P>0.05). In diagnosis value analysis, the areas under the curve of CBF in 3D-ASL scan, and rCBF ratio, rCBV ratio in DSC-PWI scan were 0.752, 0.675, and 0.645, respectively; the cut-off values were 34.59, 1.48, and 1.67, respectively; the sensitivities were 79.2%, 61.5%, and 58.3%, respectively; and the specificities were 44.4%, 32.8%, and 22.4%, respectively.ConculsionThe diagnostic value of functional MRI imaging in distinguishing glioblastoma multiforme recurrence and radiation-induced brain injury is high recommendated; further research and clinical application should be needed.
Abstract: Objective To explore clinical outcomes of nitroglycerin perfusion via the aortic root before heart re-beating for patients undergoing heart valve replacement. Methods A total of 254 patients with valvular heart diseases underwent heart valve replacement in Beijing Anzhen Hospital from January 2008 to December 2011. According to whether or not they received nitroglycerin perfusion via the aortic root before heart re-beating during cardiopulmonary bypass (CPB), all the 254 patients were randomly divided into two groups, the nitroglycerin group and the control group. There were 134 patients in the nitroglycerin group including 57 males and 77 females with their average age of 58.47±12.34 years. They received aortic root perfusion of 5 mg nitroglycerin diluted into 20 ml after full de-airing of the heart, and repeated aspiration was performed 8 to 10 times before heart re-beating during CPB. Aortic cross-clamp was released after the heart showed signs of automatic re-beating, and the aortic root was squeezed in high frequency by hand to make sure that the aorta was squeezed to close each time. In the control group, there were 120 patients including 64 males and 56 females with their average age of 61.85±11.97 years. Patients in the control group received routine procedures of CPB except nitroglycerin perfusion before heart re-beating. Automatic heart re-beating rate, CPB supporting time after heart re-beating, postoperative usage of inotropic drugs, mechanical ventilation time, perioperative morbidity and length of hospital stay were compared between the 2 groups, and patients were followed up and further evaluated. Results There was no in-hospital death. CPB supporting time after heart re-beating(12.32±9.76 min vs. 23.43±10.33 min, t=-1.233, P=0.000), dopamine usage[3.45±2.33 μg/ (kg?min) vs. 5.67±3.12 μg/ (kg?min) , t=-0.943 , P=0.034] and epinephrine usage [0.026±0.013 μg/ (kg?min) vs. 0.056±0.021 μg/ (kg?min) , t=-0.822 , P=0.027] of the nitroglycerin group were significantly shorter or lower than those of the control group. Automatic heart re-beating rate of the nitroglycerin group was significantly higher than that of the control group [97.01% vs. 66.67 % , χ 2 =-2.115 , P=0.000]. A total of 121 patients in the nitroglycerin group were followed up for 2-36 months, and 13 patients were lost during follow-up. There was no death during follow-up. Conclusion For patients undergoing heart valve replacement, some myocardial protection strategies including nitroglycerin perfusion via the aortic root before heart re-beating, use of norepinephrine, and aortic root squeezing in high frequency by hand can significantly increase the automatic heart re-beating rate, shorten the CPB supporting time after heart re-beating, and improve postoperative clinical outcomes.
ObjectiveTo determine the expression change of activating transcription factor 5 (ATF5) in human rectal cancer tissue, and analyze the correlation between ATF5 expression and the clinicopathologic parameters of rectal cancer. MethodsNinetytwo paired samples of rectal cancer tissue and more than 5 cm distant normal rectal tissue were obtained from inpatients between March 2009 and October 2009 in this hospital. ATF5 mRNA and protein expressions were detected by quantitative real-time RT-PCR and immunohistochemical staining. ResultsThirty-three (35.9%) cases of rectal cancer showed ATF5 mRNA overexpression; however, the expression level of ATF5 mRNA in the rectal cancer tissue was not statistically different from that in the normal rectal tissue (P=0.363). There was no evidence for the relationship between the ATF5 mRNA expression and the patients’ age, gender, histological type, tumor differentiation degree, invasive depth, lymph node metastasis, distance metastasis, or TNM stage. In contrast, the positive expression rate of ATF5 protein in the rectal cancer tissue was significantly higher than that in the normal rectal tissue (P=0.000). Moreover, the ATF5 protein expression was correlated with the tumor differentiation degree (P=0.013), but not with other clinicopathologic features (Pgt;0.05). ConclusionsThe results suggest that ATF5 protein may be related to the carcinogenesis and differentiation of human rectal cancer. However, further researches are required to prove the correlation.
