  • Association between red cell distribution width and prognosis of acute ischemic stroke patients receiving intravenous thrombolysis: a meta-analysis

    ObjectiveTo systematically evaluate the association between red cell distribution width (RDW) and prognosis of patients with acute ischemic stroke (AIS) undergoing intravenous thrombolytic therapy.MethodsWe searched the PubMed, Embase, Cochrane Library, Web of Science, Chinese Biomedical Literature Service System, China National Knowledge Infrastructure, Wanfang database, and CQVIP database to identify eligible studies evaluating the relation between RDW and prognosis in AIS patients receiving intravenous thrombolysis from the establishment of databases to May 2021. The Newcastle-Ottawa Scale was used to evaluate the literature quality, and RevMan 5.3 software was used to process the data. The relationship between RDW and prognosis in AIS patients treated with intravenous thrombolytic therapy was analyzed using odds ratio (OR) and 95% confidence interval (CI).ResultsFive studies with a total of 1 269 participants were included into this meta-analysis. All the studies were retrospective case-control studies. Because of statistical heterogeneity (I2=59%, P=0.04), random-effects model was chosen. Meta-analysis result showed that among AIS patients who received intravenous thrombolysis, those with elevated RDW before thrombolysis were more likely to have poor prognosis than those without elevated RDW [OR=1.50, 95%CI (1.14, 1.98), P=0.004].ConclusionElevated RDW is associated with adverse outcome of AIS patients treated with thrombolysis, and may be a risk factor for poor prognosis in patients treated with thrombolytic therapy.

  • Stress Analysis of Cervical Spine C4-C6 Under Frontal Impact Load

    In order to check the neck response and injury during motor vehicle accidents, we developed a detailed finite element model for human cervical spine C4-C6. This model consisted of cortical bone, cancellous bone, annulus, nucleus, ligaments and articular facet, and it also set up contact in the contacting parts for simulating the movement perfectly under frontal impact. This model could be used for stress and strain distribution after the frontal impact load was applied on this model. During the process of frontal impact, the most displacement simulated data were in the interval range of experimental data. The experimental results showed that this model for the human cervical spine C4-C6 simulated the movement under the frontal impact with fidelity, and reflected the impact dynamics response on the whole.

  • ANALYSIS OF hBMSCs SPATIAL DISTRIBUTION AND GENE EXPRESSION IN BIOCORAL SCAFFOLD WITH DIFFERENT SEEDING METHODS

    Objective To compare the effect of two different methods of cell seeding on spatial distribution and gene expression of hBMSCs in biocoral scaffold in vitro cultures. Methods The composite of hBMSCs and biocoral scaffold was prepared by traditional seeding (group A) and fibrin glue seeding (group B). The seeding efficiency was measured after 30 minutes of incubation in group B and after 3 hours in group A. At 2, 7, 14 and 21 days after culture, the samples were harvestedand the serial longitudinal sections were cut for each embedded composite. The sections were stained with DAPI and were measured using fluorescence microscope with apotome under serial optical sections. The cell number in every 10 × objective field was automatically measured by AxioVision image analysis software and levels (from seeding surface to bottom L1-L5) or columns (from centre to margin) for comparing cell distribution were set up. The specific osteogenic genes [osteonectin (ON), core binding factor α1 (Cbfα1), osteocalcin (OC)] expression was measured by RT-PCR. Results The seeding efficiency was significantly higher in group B (88.32% ± 4.2%) than in group A (66.51% ± 12.33%, P lt; 0.01). At 2 days after culture, the cell number from L1 to L4 decreased gradully in two groups (P lt; 0.05); in the cell number of different columns, there was no significant difference in group A (Pgt; 0.05) whereas significant difference in group B (P lt; 0.05); there was no significant difference in gene expression between two groups (P gt; 0.05). At 7 days after culture, the cell number was less than that at 2 days in group A and there was significant difference among levels (P lt; 0.05). The cell number and osteogenic gene expression increased sharply and there appeared uniform cell distribution in group B (P gt; 0.05). The gene expression of ON and Cbfα1 in group B was higher than that in group A (Plt; 0.05). At 14 days after culture, the cell number in levels or columns in group A decreased sharply and was less than that at 7 days (P lt; 0.05); whereas the cell number was similar to that at 7 days in group B (P gt; 0.05). The OC gene expression reached the highest level in group B at 14 days. The gene expression was higher in group B than in group A (P lt; 0.05). At 21 days after culture, there was significant difference in the cell number among levels and in the gene expression between group A and group B (P lt; 0.05); there was no significant difference in the cell number among columns in two groups (Pgt; 0.05). In addition, the cell number of most levels and columns in group B was more than that in group A at 7, 14 and 21 days after culture (P lt; 0.05). Conclusion More uniform cell distribution with rapid prol iferation and osteogenic differentiation is available in different levels or columns of scaffold by fibrin glue seeding than by traditional seeding.

  • Database-assisted study: geographical distribution of colorectal cancer in regional medical center—a report under real world data combined with Tableau map

    ObjectiveTo analyze the geographical distribution of patients with colorectal cancer by screening the current Database from Colorectal Cancer (DACCA) version in West China Hospital.MethodsThe selected DACCA database version of this data analysis was updated on September 5, 2019, and the two data of the " date of operation” and " address” were selected as the main research items. The characteristics of each selected data item were analyzed, and then the selected data were used as a joint feature analysis.ResultsAccording to the condition of selection by " address”, 7 096 valid data rows from the whole nation were obtained, 6 551 valid data rows from Sichuan province were obtained, and 2 954 valid data rows from Chengdu city were obtained. The geographic information provided by the DACCA database showed that, with the year changing, the provincial distribution area of patients was mainly the southwest region with middle-east of Sichuan province as center, mainly including the parts of Chongqing, Yunnan, and Guizhou; The distribution area of the municipal level in Sichuan province was mainly the east region with axis of the " Mianyang-Chengdu-Ya’an”, and Chengdu was the core; The regional distribution of patients in the Chengdu was mainly within the third ring load with Wuhou District, the Jinniu District, and the Qingyang District as the core area.ConclusionThegeographical information provided by DACCA database shows the geographical distribution characteristics of patients in the past 20 years, reflecting the basic characteristics and changes of the service area of West China Hospital, and can provide a basis for medical policy makers in screening, diagnosing and treating of colorectal cancer, and key management areas of following-up.

  • Correlation analysis of red blood cell distribution width and disease severity of acute exacerbation of bronchiectasis in the elderly patients

    ObjectiveTo investigate the relationship between the red blood cell distribution width (RDW) and the severity of acute exacerbation of bronchiectasis in elderly patients.MethodsThe clinical data of 216 elderly patients with acute exacerbation of bronchiectasis admitted from January 2015 to October of 2018 were analyzed retrospectively. The severity of acute exacerbation of bronchiectasis in the elderly was evaluated by bronchiectasis severity index (BSI) score. Meanwhile, 50 elderly people receiving qualified medical examination were collected as a healthy control group in the same period. The distributions of BSI score, RDW, procalcitonin (PCT), neutrophil percentage (NEU%) and C-reactive protein (CRP) were described in the patients with different risk degree. The severity of acute exacerbation of bronchiectasis in the elderly was evaluated by BSI. The patients were divided into three groups by BSI score: a low risk group, a middle risk group, and a high risk group. The indexes were described including the distribution of stratified BSI score, RDW, PCT, NEU%, CRP at different risk levels. The correlation of each index was analyzed by Spearman correlation. The threshold value of RDW was calculated by general linear regression, and the influencing factors of BSI score were analyzed by multivariate linear regression analysis.ResultsThe higher the risk stratification, the higher the BSI score, RDW, PCT, NEU% and CRP were. RDW was positively correlated with PCT, NEU% and CRP (r values were 0.425, 0.311, 0.177, respectively, P<0.05). BSI score was positively correlated with RDW, PCT, NEU%, and CRP (r values were 0.425, 0.394, 0.650, 0.578, respectively, P<0.05). RDW was positively correlated with PSI score (r=0.425, P<0.05). The thresholds of RDW were 11.45% and 14.03%. Multiple linear regression showed that RDW, PCT, NEU% and CRP were all influential factors of BSI score and explained 52.3% of the total mutation rate.ConclusionRDW is related to the severity of acute exacerbation of bronchiectasis in the elderly, and can predict the severity of acute exacerbation of bronchiectasis in the elderly.

  • The value of platelet volume indices in early period as prognostic indicators of sepsis

    ObjectiveTo investigate the prognostic value of platelet volume indices [mean platelet volume (MPV) and platelet distribution width (PDW)] in early period for patients with sepsis .MethodsThis retrospective study was conducted in a general ICU at a University Hospital from January 2015 to January 2017. Patients with sepsis were divided into a survival group and a death group according to 28-day prognosis. The demographic and clinical data were recorded and analyzed, including platelet indices, Charlson comorbidity index, Glasgow Coma Scale (GCS) score and Acute Physiology and Chronic Health Evaluation Ⅱ (APACHEⅡ) score. Multivariable COX proportional hazard ratio model was used to evaluate the 28-day mortality risk, and receiver operating characteristic (ROC) curve were used to evaluate the value of platelet volume indices.ResultsA total of 209 patients were enrolled in this study. Mean arterial pressure, pH, platelet count, serum creatinine, bilirubin, troponin I, prothrombin time, GCS score and platelet volume indices showed significant differences between the survival group (n=137) and the death group (n=72) (all P<0.05). Multivariable COX regression analysis revealed that the PDW was independently associated with 28-day mortality in sepsis (HR=1.068, 95%CI 1.002-1.139, P=0.043). The area under the ROC curve of PDW was 0.762 (95%CI 0.694 - 0.831), with a sensitivity of 86% and a specificity of 63% at a cut-off value of 18.65%.ConclusionPDW can be used as a powerful prognostic indicator in patients with sepsis following 28-day.

  • The Role of Red Cell Distribution Width in Coronary Artery Diseases Patients Complicated with Obstructive Sleep Apnea-Hypopnea Syndrome

    Objective To investigate the role of red cell distribution width ( RDW) in coronary artery diseases patients complicated with obstructive sleep apnea-hypopnea syndrome ( OSAHS) . Methods 134 coronary artery diseases patients who had at least one-vessel disease confirmed by coronary angiography were investigated by polysomnography for OSAHS. The patients were classified according to theapneahypopnea index(AHI) . The level of RDW, triglyceride, cholesterol, high density lipoprotein, low density lipoprotein, hemoglobin, white blood cells and hematocritwere measured. The receiver operating characteristic curve was drawn to predict the moderate-severe OSAHS in coronary artery diseases patients according to RDW value. Results When 134 coronary artery diseases patients were classified into a control group and an OSAHS group according to the AHI, the level of RDW in two groups was not significantly different [ ( 13.44 ±1.30) % vs. ( 13.12 ±0.92) % , P gt; 0.05] . When 134 coronary artery diseases patients were classified into a control and mild OSAHS group and a moderate-severe OSAHS group according to the AHI, the level of RDW in two groups was significantly different [ ( 13.07 ±0.94) vs. ( 14.02 ±1.41) % , P lt; 0.05] . And no difference was found in hemoglobin, triglyceride, cholesterol, high density lipoprotein, low density lipoprotein, platelet, and hematocrit between two groups. The ROC curve analysis revealed that the area under ROC curve was 0.748 ( 0.523-0.972) , and the best cut-off for moderate-severe OSAHS was 13.95% with sensitivity of 71.43% and specificity of 82.98% . Conclusion RDW may be a useful and simple tool to predict moderate-severe OSAHS in coronary artery diseases patients.

  • Associations of preoperative red cell distribution width with mortality and morbidity in patients underwent liver transplantation: a retrospectively observational study

    ObjectiveTo investigate the associations of preoperative red cell distribution width (RDW) with mortality and morbidity in patients underwent liver transplantation. MethodsThis investigation was a retrospective study, the patients underwent liver transplantation met the inclusion criteria from June 2017 to May 2020 in the West China Hospital of Sichuan University were enrolled. The patients were divided into RDW≤14.5% group and RDW>14.5% group according to the normal RDW critical value (14.5%). The propensity score matching (PSM) was used to adjust the baseline characteristics. The primary outcome was 1-year mortality. The secondary outcomes included 1-year survival, 30-day mortality, incidence of early allograft dysfunction, acute kidney injury, renal replacement therapy, and pulmonary complications, as well as ICU stay and postoperative hospital stay. ResultsA total of 303 patients who met the analysis conditions were included. After PSM, 57 patients in each group were matched. There were no significant differences between the two groups in the baseline data such as the gender, age, body mass index (BMI), initial diagnosis, MELD score, Child-Pugh grade of the recipients, and the gender, age, and BMI of the donors (P>0.05). The 1-year [22.8% (13/57) versus 5.3% (3/57), χ2=7.27, P=0.007] and 30-day [15.8% (9/57) versus 3.5% (2/57), χ2=4.93, P=0.026] mortality of the patients with RDW >14.5% were higher than that of the patients with RDW ≤14.5% . The Kaplan-Meier survival curve showed that the 1-year survival of the patients with RDW ≤14.5% after liver transplantation was better than that of the patients with RDW >14.5% [hazard ratio=4.75, 95%CI (1.78, 12.67), P=0.007], but there were no significant differences between the two groups in the incidence of early graft dysfunction, acute renal injury, renal replacement therapy, and pulmonary complications, as well as postoperative hospital stay and ICU stay (P>0.05). ConclusionPreliminary results of this study indicate that preoperative RDW of patients underwent allogeneic liver transplantation is associated with1-year mortality, 30-day mortality, and 1-year survival.

  • Keshan Disease in Chongqing City during 2008-2012: Clinical Characteristics and Geographical Distribution

    ObjectiveTo analyze the clinical characteristics and geographical distribution of Keshan disease in Chongqing city for prevention and disease control. MethodsWe collected the clinical data of patients with Keshan disease from 2008 to 2012 in Liangping, Shizhu, Fengdu and Dianjiang counties as well as Wanzhou district of Chongqing city including the medical history, physical examination, results of laboratory tests to analyze the clinical characteristics and geographical distribution. ResultsFifty-eight patients were included from Liangping (n=21), Shizhu (n=25), Fengdu (n=11) and Dianjiang (n=1). The number of patients with potential and chronic Keshan disease was 16 and 42, respectively. The average age of patients was 54.91±15.53 years. The proportion above age 60 was 32.76% and below age 10 was 3.45%. The patients had main clinical signs as heart enlargement (36.76%), low-weak first heart sound (22.41%), systolic murmur (10.34%), arrhythmia (8.62%), etc. Abnormal ECG detection rate was 98.28%, with common types followed by sinus rhythm (37.93%), complete right bundle branch block (25.86%), ST-T changes (24.14%), left ventricular hypertrophy (15.52%), atrial fibrillation (13.79%), occasional ventricular premature (10.34%), T changes (10.34%), sinus bradycardia (8.62%), and incomplete right bundle branch block (6.90%). X-ray results showed that heart enlargement accounted for 82.76%. The ratios of mild, moderate and significant expansion of the heart were 46.55%, 27.59%, and 8.62%, respectively. ConclusionIn recent years, most patients with Keshan disease in Chongqing are chronic type at older age. The main clinical symptom is heart enlargement with high abnormal ECG detection rate.
