  • Relationship between optic disc hemorrhage and localized retinal nerve fiber layer defect in normal-tension glaucoma

    ObjectiveTo investigate the relationship between optic disc hemorrhage and localized retinal never fiber layer defects (RNFLDs) in norma l tension glaucoma.MethodsIn 83 patients with normal-tension glaucoma, the cumulative frequency and quadrantal distribution of optic disc hemorrhages were retrospectively analyzed. The neighboring relation between optic disc hemorrhages and RNFLDs in a same quadrant and the changes of correspondin gretinal never fiber layer (RNFL) after the occurrence of optic disc hemorrhages were observed by tridimensional photochromy of ocular fundus.Results(1) The occurrences and distribution of optic disc hemorrhages: 29of83(34.94%) patients (33 eyes) had totally 58 occurrences, including 39 in infer iotemporal area, 14 in superiotemporal area, and 5 in other area. (2) The relati onship of neighborhood between optic disc hemorrhages and RNFLDs: in the availab le tridimensional photochrome, 23 occurrences in 15 patients (16 eyes) were foun d with cuneiform RNFLDs in the same quadrant, in which 22 was near the border of cuneiform RNFLDs. (3) The changes of corresponding retinal never fiber layer (R NFL) after the occurrence of optic disc hemorrhages: the photochromes of 24 occurrences in 20 patients (21 eyes) were kept well in the initial and the 2-year follow-up periods, while the changes of RNFL were found in each region correspon ding to the 19 occurrences (in inferiotemporal or superiotemporal area) in the initial photochrome, including 7 cuneiform defects with various sizes, and 12 developed localized RNFLDs next to the initial hemorrhages in the optic disc. No obvious localized RNFL corresponding to the other 5 occurrences (1 in inferiotempo ral, 1 in superiotemporal, and 3 in other areas) were found in the follow up period.ConclusionOptic disc hemorrhages in normal-tension glaucoma occur mostly in inferiotemporal area, and secondly in superiotemporal area of optic disc, and the appearance of optic disc hemorrhages may suggest that the localized RNFLDS would develop in the associated regions.(Chin J Ocul Fundus Dis,2004,20:339-342)

  • Burden of intracerebral hemorrhage and its risk factors in China: findings from the Global Burden of Disease Study 2021

    Objective To analyze the trends in the burden of intracerebral hemorrhage (ICH) and its related risk factors in China from 1990 to 2021, providing evidence for targeted prevention and control. Methods Based on public data from the Global Burden of Disease Study 2021, four epidemiological indicators, including incidence, prevalence, deaths, and disability-adjusted life-years (DALYs) were selected to describe the burden of ICH in China in 2021. Change rates and estimated annual percentage changes (EAPC) were calculated to evaluate the trends in disease burden from 1990 to 2021. The Das Gupta method was used to decompose the effects of population growth, population aging and epidemiological changes on the burden during this period. Finally, the attributable burden of risk factors related to ICH was analyzed. Results In 2021, the age-standardized incidence, prevalence, mortality, and DALY rates of ICH in China were 61.2/100000, 222.1/100000, 68.8/100000, and 1351.6/100000, respectively, all higher than global estimates. From 1990 to 2021, although these rates showed a declining trend (EAPC: −2.24, −1.26, −2.38, and −2.47, respectively), the absolute disease burden, including the absolute number of incidence cases, prevalence cases, deaths, and DALYs, continued to rise, with an increase ranging from 20.57% to 51.59%. In addition, the burden of ICH in China varied by age and sex, with older adults and males experiencing a higher burden. Decomposition analysis indicated that population aging and growth were the primary drivers of the increasing ICH burden in China, while epidemiological changes mitigated this trend. Metabolic factors were the predominant attributable risk factors for ICH. High systolic blood pressure, ambient particulate matter pollution, and diet high in sodium were important risk factors common to both genders. The DALY burden attributable to smoking and alcohol use was higher in men, whereas impaired kidney function and secondhand smoke had a greater impact on women. Conclusions The burden of ICH in China has continued to increase from 1990 to 2021, and it may further escalate in the context of population aging. Risk factor control remains a key priority for prevention. Future strategies should incorporate age- and sex-specific interventions to reduce the ICH burden in China.

  • Laser treatment for premacular hemorrhage

    Objective To evaluate the effective method for treatment of premacular hemorrhage. Methods In 36 cases (36 eyes) of premacular hemorrhage (2PD),25 cases were treated with medicine, and 11 cases were treated with laser surgery and medicine. Results In 25 patients treated with medicine, the average time of vision recovery was 24 days and the average time of clearance of vitreous hemorrhage was 40 days; in the other 11 patients treated with laser surgery and medicine,10 were treated successfully, and in these 10 patients, the average time of vision recovery were 5 days and the average time of clearance of premacular hemorrhage were 11 days in 10 cases which were operated successfully with laser in 11 cases. Conclusions Laser surgery of inducing preretinal hemorrhage to vitreous body is an effective method for treatment of premacular hemorrhage to relieve the impaired central vision.   • Evidence-based nursing care on prevention of post-pancreaticoduodenectomy hemorrhage

    Objective To explore how to integrate the various sources of information in designing an evidence-based nursing care plan for preventing gastrointestinal hemorrhage (GIH) after pancreaticoduodenectomy (PD). Method Papers and references about prevention of GIH after PD were searched between September and October 2015, and an evidence-based nursing care plan was drawn up and implemented from November 2015 to January 2016. Results A total of 79 papers were found and of which 17 were aviliable. Thirty-nine patients were cared on the basis of the effective project, of whom one was dignosed with GIH on the 3rd postoperative day and the rate of post-PD hemorrhage was 2.6%. All patients were diacharged on the 6th or 7th postoperative day. Conclusion Exploring evidences under the guidance of scientific method and applying them to clinical nursing can prevent post-PD hemorrhage and improve life quality of patients.

  • Comparison of fundus fluorescein angiography and indocyanine green angiography for imaging of subretinal hemorrhage in age-related macular degeneration

    ObjectiveTo compare the effects of fundus fluorescein angiography (FFA) and indocyanine green angiography (ICGA) on classifications and locations of choroidal neovascularization (CNV) located under subretinal hemorrhage in age-related macular degeneration, and on the areameasuring of subretinal hemorrhage.MethodsThe medical record of 177 patients with subretinal hemorrhage associated with AMD confirmed by photochrome of ocular fundus, FFA and ICGA examinations were retrospectively reviewed. The locations and classifications of CNV on the images of FFA and ICGA in the 177 patients and the area of subretinal hemorrhage of 30 patients randomly selected from the 177 patients were analyzed and measured. ResultsOn the images of FFA, the locations and classifications of CNV could be defined in 160 patients (90.4%), and CNV was considered eligible for laser therapy in 24 patients (13.6%). On the images of ICGA, the locations and classifications of CNV could be defined in 175 patients (98.9%),and CNV was considered eligible for laser therapy in 51 patients (28.8%). There was no statistical difference of the locations(Z=-0.383,P=0.701) and classifications ( χ2=2.993,P=0.810) of CNV on the images of FFA and ICGA. The areas of blocked fluorescence measured on the images of FFA (x-=26.610 mm2 and M=13.548 mm2) were larger than those of ICGA (x-=24.714 mm2,M=12.875 mm2) with statistical differences (Z=-3.000,P=0.003) between FFA and ICGA. ConclusionsICGA is beneficial for imaging CNV located under subretinal hemorrhage, and may increase the number of the patients who are considered eligible for laser treatment. The effect of measurement of the area of subretinal hemorrhage in AMD evaluated by FFA is better. (Chin J Ocul Fundus Dis,2003,19:149-151)

  • THE INDOCYANINE GREEN ANGIOGRAPHY AND AGE-RELATED MACULAR DEGENERATION WITH MACULAR HEMORRHAGE

    PURPOSE:To search for the occult choroidal neovascularization(CNV)of age-related macular degeneration (AMD)with macular hemorrhage using indocyanine green angiography(ICGA). METHODS:FFA and ICGA were performed in a series of 22 cases(24 eyes)of AMD with macular hemorrhage,and the findings of both angiograms were compared each other. RESULTS :ICGA was found to be superior than FFA in evaluating the occult CNV of AMD with hemorrhage owing to the following outstanding findings in this series,i.e,in judging the presence,position,number and range of the occult CNV. CONCLUSION:ICGA is an important technique in diagnosing the subretinal occult CNV in AMD with macular hemorrhage,and useful in selecting therapeutic measures including photocoagulation and surgical treatment.   • TRANS-CATHETER EMBOLIZATION IN THE TREATMENT OF MASSIVE BILIARY HEMORRHAGE

    【Abstract】Objective To investigate the diagnostic and therapeutic value of selective angiography and embolization treatment in massive biliary hemorrhage in emergency. Methods Eleven cases of massive biliary bleeding were selectively angiographied and embolized with thread segment and gelatin sponge grain by Seldinger technique in emergency. Results Massive biliary bleeding in this series included bleeding after biliary surgery (7), post-laparoscopic cholecystectomy bleeding (1), bleeding from traumatic injury to liver (1), and rupture of huge hepatic cancer (2). There were 9 cases with abnormal angiographic findings including contrast medium leakage (9), tumor vessels and tumor tissue staining (2) and signs of pseudo-aneurysm formation (4). All the bleedings were stopped after emergency intervention but 2 cases with recurrent massive bleeding after 48 hours. Conclusion Emergency trans-catheter embolization treatment is an effective and safe method for biliary massive hemorrhage. It is also important to keep the biliary tract unobstructed.

  • Efficacy and Safety of Sodium Citicoline Tablets in the Treatment of Acute Cerebral Hemorrhage: A Randomized Controlled Trial

    Objective To evaluate the efficacy and safety of sodium citicoline tablets in the treatment of acute cerebral hemorrhage within 72 hours from the onset. Methods A randomized , double -blind, double-dummy, active control clinical study was performed. Patients who met the inclusion criteria were randomized into two groups. The treatment group (18 cases) received sodium citicoline tablets (0.2 g tid) and placebo capsule (0. 2 g tid), while the control group (18 cases) received sodium citicoline capsule (0.2 g tid) and placebo tablets (0. 2 g tid). The duration of treatment was 21 days for the two groups. National Institutes of Health Stroke Scale (NIHSS) and Barthel Index (BI) were used to evaluate the recovery of neurological functions. Results NIHSS and BI scores increased significantly in both groups after treatment (P 〈0. 01 ). There was no statistical difference of the improvement between the two groups (P 〉0. 05). No adverse drug reaction or significant change in laboratory norms was found in either group. Conclusions Sodium citicoline tablets is effective and relatively safe in the treatment of acute cerebral hemorrhage. The efficacy and safety of sodium citicoline tablets in the treatment of acute cerebral hemorrhage is similar to that of sodium citicoline capsule.

  • Experimental Detection Study on Cerebral Hemorrhage in Rabbits Based on Magnetic Induction Phase Shift Spectroscopy Under the Feature Band

    This study was aimed to improve the sensitivity of magnetic induction phase shift detection system for cerebral hemorrhage. In the study, a cerebral hemorrhage model with 13 rabbits was established by injection of autologous blood and the cerebral hemorrhage was detected by utilizing magnetic induction phase shift spectroscopy (MIPSS) detection method under the feature band. Sixty five groups of phase shift spectroscopy data were obtained. According to the characteristics of cerebral hemorrhage phase shift spectroscopy under the feature band, an effective method, B-F distribution, to diagnose the severity of cerebral hemorrhage was designed. The results showed that using MIPSS detection method under feature band, the phase shift obviously growed with increase of injection volume of autologous blood, and the phase shift induced by a 3-mL injection reached-7.750 3°±1.420 4°. B-F distribution could effectively diagnose the severity of cerebral hemorrhage. It can be concluded that the sensitivity of the cerebral hemorrhage magnetic induction detection system is improved by one order of magnitude with the MIPSS detection method under the feature band.

  • Improving the understanding of stroke with hereditary cerebral small vessel disease

    Stroke with hereditary cerebral small vessel diseases is a rare disease. Its clinical manifestations include early-onset ischemic lacunar or hemorrhagic stroke with high disability. Its typical imaging markers include lacunes, white matter hyperintensities, microbleeds, intracerebral hemorrhages located in deep or lobe of brain, crotical microinfarcts, and enlarged perivascular spaces. As the clinical and neuroimaging signs and symptoms of hereditary cerebral small vessel diseases often overlap with sporadic cerebral small vessel diseases, it is hard to diagnose. This article summarizes the clinical features, importance of obtaining valuable family history, genetic diagnosis, and management of stroke with hereditary cerebral small vessel disease to improve its accuracy diagnosis.
