  • Research progress on the mechanism and predictors of hemorrhagic transformation in acute ischemic stroke

    Acute ischemic stroke is the most common type of stroke. Hemorrhagic transformation is one of its serious complications, which may lead to severe neurological deterioration and poor prognosis. The occurrence of hemorrhagic transformation is mainly related to the inflammatory mechanism after infarction, blood-brain barrier injury, ischemia-reperfusion injury and abnormal coagulation function. Identification of early predictors of hemorrhagic transformation can help reduce its incidence and severity. However, the mechanism of hemorrhagic transformation is complex, and there is currently no unified standard for its prediction. This article aims to review the related mechanisms and early predictors of hemorrhagic transformation after stroke, in order to provide a reference for early identification and prevention.

  • STUDY ON MECHNISM OF THE CIRCULATION RECONSTRUCTING OF SKIN FLAP AFTER EARLY-REPEATED SHORT ISCHEMIA TRAINING

    Objective To investigate the effects of repeated shortischemia training on flap survival area, vascular endothelial growth factor and the microvascularsel density. Methods Seventy-two rabbits were divided into:the experimental group(n=64), the skin flaps were constructed in two sides of back, one side flap were given ischemia training for 15 minutes and 8 times one day at the pedicles from the 1st to 8th day after operation (group A), the other side flap was served as a control (group B), the corresponding site was only marked as a blank control group (group C).Then, 8 pedicles of group A and group Bwere isolated every day. The surviving area of all skin flaps were calculated on the5th day after isolating operation. The vascular endothelial growth factor(VEGF)and microvessel density(MVD) of the 3 groups were checked with immunohistologochemical staining. Results After the operation, all animalswere survival with normal vitality.The survival flap area of group A were significant more than that of group B after 3 days(Plt;0.05).The expressions of VEGF and MVD of group A and group B were higher than group C. The expression of VEGF of group A was significantly higher than that of group B(Plt;0.01). The counting of MVD of group A was also significantly higher than that of group B(Plt;0.05). There was positive correlation between flap survival area and MVD in group A. The relation of time point was n and n 2 respectively,correlation coefficient was 0.850. As well as MVD and VEGF were positive correlation,correlation coefficient was 0.801. Conclusion Early repeated shortischemia training can increase flap survival area, the mechanism maybe involve the increased expression of VEGF, which can increased skin flap microvascular density.

  • Get to know about pain again: new interpretations, new classifications and new treatments

    The scientific understanding of pain has stayed on a traditional classic “pain pathway” for many years. However, with the continuous progress of clinical diagnosis, this pathway obviously cannot explain all pain phenomena, and the understanding and interpretation of pain needs to be updated. Based on the influence of comprehensive medicine based on philosophy and the continuous summary and research of clinical cases of somatic symptoms, this paper takes the biological defense mechanism as the basic starting point, analyzes the position of pain in the biological defense mechanism, and reinterprets and defines pain from the new point of view of comprehensive medicine with the new perspective of biological defense mechanism. In addition, this paper also presents the new treatment principles of pain according to the occurrence mechanism through the perspective of biological defense and psychosomatic mechanism, and puts forward new principles of treatment and the view that “defense” is the fundamental purpose of all biology, aiming to provide a reference for the reinterpretation of more diseases and the research on the nature of biological behavior.

  • Research progress of adipose-derived stem cells in promoting the repair of peripheral nerve injury

    ObjectiveTo summarize the research progress of adipose-derived stem cells (ADSCs) in promoting the repair of peripheral nerve injury.MethodsThe related literature at home and abroad in recent years was widely reviewed, the mechanism of ADSCs promoting the repair of peripheral nerve injury was introduced, and its basic research progress was analyzed and summarized. Finally, the clinical transformation application of ADSCs in the treatment of peripheral nerve injury was introduced, the existing problems were pointed out, and the new treatment regimen was prospected.ResultsADSCs have the function of differentiation and paracrine, and their secreted neurotrophic factors, antiapoptosis, and antioxidant factors can promote the repair of peripheral nerve injury.ConclusionADSCs are rich in content and easy to obtain, which has a definite effectiveness on the repair of peripheral nerve injury with potential clinical prospect.

  • Research progress on the relationship between cuproptosis and breast cancer

    ObjectiveTo summarize the latest advances in copper and cuproptosis in the field of breast cancer, and to provide a reference for clinical treatment decisions. MethodThe literatures related to copper and cuproptosis in recent years were read and summarized, and the research progress on the role of copper in breast cancer, the application of cuproptosis in the diagnosis and treatment of breast cancer were reviewed. ResultsCuproptosiswas a novel form of programmed cell death, which occurred via direct binding of copper to lipoylated components of the tricarboxylic acid (TCA) cycle, this resulted in lipoylated protein aggregation and subsequent iron-sulfur cluster protein loss, leading to proteotoxic stress and ultimately cell death. Cuproptosis induced proliferation and migration of breast cancer cell , mediated personalized immunotherapy, and participated in endocrine and chemotherapeutic drug resistance. ConclusionExploring the mechanism of cuproptosis provides potential applications for subsequent immunotherapy, endocrine therapy, and chemotherapy for breast cancer, leading to new effective strategies for patients.

  • Research progress in osteogenesis and osteogenic mechanism of heparan sulfate

    Objective To discuss the role of heparan sulfate (HS) in bone formation and bone remodeling and summarize the research progress in the osteogenic mechanism of HS. Methods The domestic and abroad related literature about HS acting on osteoblast cell line in vitro, HS and HS composite scaffold materials acting on the ani-mal bone defect models, and the effect of HS proteoglycans on bone development were summarized and analyzed. Results Many growth factors involved in fracture healing especially heparin-binding growth factors, such as fibroblast growth factors, bone morphogenetic protein, and transforming growth factor β, are connected noncovalently with long HS chains. HS proteoglycans protect these proteins from protease degradation and are directly involved in the regulation of growth factors signaling and bone cell function. HS can promote the differentiation of stem cells into osteoblasts and enhance the differentiation of osteoblasts. In bone matrix, HS plays a significant role in promoting the formation, maintaining the stability, and accelerating the mineralization. Conclusion The osteogenesis of HS is pronounced. HS is likely to become the clinical treatment measures of fracture nonunion or delayed union, and is expected to provide more choices for bone tissue engineering with identification of its long-term safety.

  • Changes of gut microbiota after bariatric surgery and the mechanisms of improving metabolism

    Objective To review the changes of gut microbiota after bariatric surgery and the related mechanisms of improving metabolism. Method Domestic and international literatures in recent ten years on the changes of gut microbiota in bariatric surgery and the mechanisms of improving metabolism were collated and summarized. Result The common bariatric procedures performed to date were vertical sleeve gastrectomy (VSG) and laparoscopic Roux-en-Y gastric bypass (RYGB). The changes of gut microbiota vary in different surgical procedures, which were related to the changes of diet habits, gastrointestinal anatomy, gastrointestinal hormone levels and metabolic complications. The gut microbiota might improve the body metabolism by regulating the levels of short chain fatty acids, branched chain amino acids and bacterial endotoxin in the intestinal lumen. Conclusions Significant changes are found in gut microbiota after bariatric surgery, which may be involved in the improvement of body metabolism by regulating the level of bacterial endotoxin and microbial metabolite. However, more in-depth mechanisms need to be further clarified.

  • Case report: the identification and treatment of primary severe acute respiratory syndrome (SARS) patients in Sichuan province

    Objective Can we successfully make diagnosis, treatment and prevention in patients with SARS in the economically undeveloped region? Methods we retrospectively reviewed the whole process of diagnosis, treatment and prevention in three patients with SARS. Results All the three patients recovered with no socomial infection in medical staff and no second-generation patients. Conclusion In the economically undeveloped region, if one attached importance to epidemic crisis inwardly and put all the steps into effect really, and if one took four early steps, started the contingent mechanism up urgently, worked can onically and treated patients actively when the epidemic crisis broke out, the outbreak could be overcome successfully with little cost.

  • Exploring and analyzing the improvement mechanism of U-Net and its application in medical image segmentation

    Remarkable results have been realized by the U-Net network in the task of medical image segmentation. In recent years, many scholars have been researching the network and expanding its structure, such as improvement of encoder and decoder and improvement of skip connection. Based on the optimization of U-Net structure and its medical image segmentation techniques, this paper elucidates in the following: First, the paper elaborates on the application of U-Net in the field of medical image segmentation; Then, the paper summarizes the seven improvement mechanism of U-Net: dense connection mechanism, residual connection mechanism, multi-scale mechanism, ensemble mechanism, dilated mechanism, attention mechanism, and transformer mechanism; Finally, the paper states the ideas and methods on the U-Net structure improvement in a bid to provide a reference for later researches, which plays a significant part in advancing U-Net.

  • Research progress on mechanism of CD147 in promoting progression of breast cancer and related diagnosis and treatment

    ObjectiveTo summarize the mechanism of CD147 in breast cancer invasion and metastasis, treatment, and drug resistance so as to provide reference for clinical decision-making.MethodThe relevant literatures about studies of CD147 in breast cancer in recent years were reviewed.ResultsCD147 was widely distributed in vivo and highly expressed in malignant tumor tissues. CD147 promoted matrix metalloproteinases and vascular endothelial growth factor productions and tumor microenvironment generation by extracellular matrix in breast cancer through different mechanisms. It degraded extracellular matrix and stimulated neovascularization to promote tumor invasion and metastasis. Related studies had shown that CD147 was highly expressed in the breast cancer tissues and which was associated with tumor grade and prognosis in patients with breast cancer, and it was a biological marker for diagnosis of breast cancer. However, a large of drugs targeted for CD147 and its involved pathways didn’t well benefit patient with breast cancer due to the failure of clinical trials and chemotherapy resistance.ConclusionsCD147 plays a key role in development, invasion and metastasis, diagnosis and treatment, and drug resistance of breast cancer, as well as guiding the treatment and prognosis of patients. However, benefits are poor, and relevant molecular mechanisms of action are limited.
