  • Risk Factors of Morning Blood Pressure Surge among Elderly Patients with Primary Hypertension in China: A Meta-Analysis

    ObjectiveTo systematically review the risk factors in elderly patients with primary hypertension with morning blood pressure surge in China, so asto provide references for clinical treatment and prevention of complications. MethodsSuch databases as PubMed, EMbase, The Cochrane Library (Issue 11, 2013), CNKI, VIP and WanFang Data were electronically searched for the case-control studies about morning blood pressure surge (MBPS) among elderly patients with primary hypertension in China from January 2006 to June 2014 were collected. Literature was screened according to inclusion and exclusion criteria, data were extracted and the methodological quality of the included studies was assessed, and then meta-analysis was conducted using RevMan 5.2 software. ResultsA total of 16 studies involving 2 007 cases were finally included, of which 956 cases were detected with MBPS. The results of meta-analysis showed that significant differences were found in glucose levels (MD=0.42, 95%CI 0.04 to 0.81, P=0.03), urinary microalbumin levels (MD=23.85, 95%CI 6.64 to 41.07, P=0.007), incidences of cerebrovascular events (OR=1.96, 95%CI 1.25 to 3.08, P=0.004), carotid atherosclerosis (OR=5.13, 95%CI 1.70 to 15.45, P=0.004) and left ventricular hypertrophy (OR=2.49, 95%CI 1.70 to 3.64, P < 0.000 01), left ventricular mass (MD=12.89, 95%CI 3.94 to 21.84, P=0.005), and carotid artery intima-media thickness (MD=0.08, 95%CI 0.02 to 0.14, P=0.009); while no significant difference was found in gender (OR=1.12, 95%CI 0.84 to 1.49, P=0.44), total cholesterol levels (MD=0.01, 95%CI-0.11 to 0.12, P=0.92), and creatinine levels (MD=1.77, 95%CI-1.16 to 4.70, P=0.24) between patients with or without MBPS. ConclusionCurrent evidence shows that glucose levels, early kidney damage, emergent cerebrovascular events and the reconstruction of the artery and the left ventricle are risk factors of abnormal MBPS in China. However, the above conclusion needs to be verified by further conducting high quality prospective studies.

  • Correlation of the Adiponectin Gene +45 in Exon 2 Polymorphisms and Type 2 Diabetes Mellitus in Chinese Population: A Meta-Analysis

    Objective To evaluate the relationship between the single nucleotide polymorphisms (SNPs) of the adiponectin gene +45 in exon 2 and type 2 diabetes mellitus (T2DM) in Chinese population via meta-analysis. Methods Databases including PubMed, Ovid, CBM, VIP, CNKI, and WanFang Data were searched from inception to June 2012, and the references of articles were also retrieved to collect case-control studies about the correlation of SNPs of the adiponectin gene +45 in exon 2 and T2DM in Chinese population. According to the self-designed inclusion and exclusion criteria, two reviewers screened articles, extracted data, and assessed the quality of the included studies independently. Then meta-analysis was performed STATA 11.0, with stability evaluated by both stratified analysis and sensitivity analysis. Moreover, Begg’s funnel plot and Egger’s method were used to assess the published bias of articles. Results 21 articles involving 22 studies were included (3272 T2DM cases and 2597 controls). There were significant differences between the two groups in dominant, recessive and addictive genetic models, and the pooled ORs (95% CI) were 1.36 (1.04, 1.78), 2.07 (1.55, 2.75), and 2.44 (1.59, 3.75), respectively. Conclusion The genetic single nucleotide polymorphisms of the adiponectin +45 in exon 2 is associated with type 2 diabetes in Chinese population. G allele of APM1 is a risk factor for type 2 diabetes, no matter in dominant, recessive or addictive genetic models.

  • Meta-analysis of Effect on HIV/AIDS Intervention in Floating Population

    Objective To evaluate the effect of a health education for preventing HIV/AIDS in floating population. Methods A computerized literature search was carried out in PubMed, CBM (Chinese Biomedical Database), CNKI (Chinese National Knowledge Infrastructure), Wanfang (Chinese) and VIP (Chinese) databases to collect articles published between 1996 and 2006 concerning the effect of a HIV/AIDS education intervention in floating population. We also checked the reference lists of relevant articles. The study type was self-control intervention study. Meta-analyses were performed to assess 3 outcomes of the intervention, i.e. knowledge about HIV transmission, means of prevention and attitudes towards HIV/AIDS patients. Fixed and random effect models were employed to combine results after a heterogeneity test, with rate difference (RD) used as the indicator of intervention effect. Results The analysis showed that the RD for knowing the sexual transmission route of HIV, the RD for knowing the effect of condoms for HIV prevention, and the RD for changing attitudes towards HIV/AIDS patients, i.e. treating them as ordinary people, were increased by 16% (0.10, 0.22), 22% (0.17, 0.28) and 19% (0.13, 0.25), respectively. Conclusion Health education for preventing HIV/AIDS is effective in changing knowledge and attitudes in floating population.

  • Expression of T Cell Costimulatory Molecule and Variance of T Cell Subpopulations in Patients with Gastric Cancer andColorectal Cancer

    Objective  To investigate the role of expression of T cell costimulatory molecule CD28 and variance of T cell subpopulations in the development and prognosis of gastric cancer and colorectal cancer. Methods The peripheral blood lymphocytes were tested for T cell subpopulations and T cell costimulatory molecule CD28 by flow cytometry in 38 patients with gastric cancer, 42 patient s with colorectal cancer , and 21 healthy peoples as control group . Results  Expressions of T cell costimulatory molecule CD28 in patients with gastric cancer and colorectal cancer were (25. 80 ±10. 56) % and (28. 95 ±9. 29) % , and significantly higher than that of control group 〔(0. 82 ±0. 98) % , Plt; 0. 01〕. Expression percentage of total T cell (CD3 + ) in patient s with gastric cancer and colorectal cancer were significantly lower than that of control group 〔(53. 61 ±13. 84) % and (55. 96 ±10. 68) % vs (72. 07 ±7. 83) % , Plt; 0. 01〕. Expression percentage of CD4 + T cell (CD4 + CD3 + ) in patients with gastric cancer and colorectal cancer were significantly lower than that of control group 〔( 29. 84 ±9. 71) % and ( 33. 75 ±9. 04) % vs (38. 79 ±5. 08) %; Plt; 0. 01 , Plt; 0. 05〕; Expression percentage of CTL cell (CD8 + CD28 + CD3 + ) in patient s with gastric cancer and colorectal cancer were significantly higher than that of control group 〔( 1. 57 ±1. 99) % and (1. 93 ±2. 61) % vs (0. 02 ±0. 04) %; P lt; 0. 01〕; Expression percentage of CD8 + inhibitory T cell (CD8 + CD28 -CD 3 + ) and CD4 / CD8 ratio in patient s with gastric cancer were significantly lower than that of control group 〔(16. 06 ±6. 94) % vs (20. 56 ±6. 54) % , Plt; 0. 05 ; (1. 10 ±0. 51) % vs (1. 36 ±0. 31) % , P lt; 0. 05〕; Expression of regulatory T cell (CD4 + CD25 + CD3 + ) of patients with colorectal cancer was (19. 74 ±6. 89) % , which was significantly higher than that of control group 〔(13. 72 ±3. 08) % , Plt; 0. 01〕. No difference of expression was found in peripheral T cell subpopulations of postoperative patients with gastric cancer and colorectal cancer after one week ( Pgt; 0. 05) . Conclusion  T cell number is fall ,T cell costimulatory molecule CD28 useless expression is increase in patient s with gastric cancer and colorectal cancer. CD4 + T cell subpopulation is significantly decreased in patient s with gast ric cancer. The regulatory T cell of patient s with colorectal cancer is significantly increased.

  • Database-assisted study: geographical distribution of colorectal cancer in regional medical center—a report under real world data combined with Tableau map

    ObjectiveTo analyze the geographical distribution of patients with colorectal cancer by screening the current Database from Colorectal Cancer (DACCA) version in West China Hospital.MethodsThe selected DACCA database version of this data analysis was updated on September 5, 2019, and the two data of the " date of operation” and " address” were selected as the main research items. The characteristics of each selected data item were analyzed, and then the selected data were used as a joint feature analysis.ResultsAccording to the condition of selection by " address”, 7 096 valid data rows from the whole nation were obtained, 6 551 valid data rows from Sichuan province were obtained, and 2 954 valid data rows from Chengdu city were obtained. The geographic information provided by the DACCA database showed that, with the year changing, the provincial distribution area of patients was mainly the southwest region with middle-east of Sichuan province as center, mainly including the parts of Chongqing, Yunnan, and Guizhou; The distribution area of the municipal level in Sichuan province was mainly the east region with axis of the " Mianyang-Chengdu-Ya’an”, and Chengdu was the core; The regional distribution of patients in the Chengdu was mainly within the third ring load with Wuhou District, the Jinniu District, and the Qingyang District as the core area.ConclusionThegeographical information provided by DACCA database shows the geographical distribution characteristics of patients in the past 20 years, reflecting the basic characteristics and changes of the service area of West China Hospital, and can provide a basis for medical policy makers in screening, diagnosing and treating of colorectal cancer, and key management areas of following-up.

  • Database research part Ⅰ: colorectal cancer from reginal medical center and population characteristics

    ObjectiveScreening the Database from Colorectal Cancer (DACCA) based on West China Hospial data by " Operation Date”, we purposed to analyze the population characteristics of colorectal cancer patients in regional medical center within recent Database Version.MethodsThe DACCA Version was updated in December 12th, 2018. Personal data (including sex, age, blood type, height, weight, and BMI), location data (including provinces, cities, and subordinate areas in Chengdu), occupation and education data, and main diagnosis data were included in the items. Characteristic analysis was performed on each selected data item.ResultsAccording to screening, 9 633 analytical data rows were obtained. Based on the database information, there were 24 consecutive years from 1995 to 2018 into every year. We set 2005 to 2006 as the time node for the database construction. The contribution to database before 2005 (including) was 1 358, while after 2005 (not including) were 8 275. The contribution rate (contribution numbers/years) after 2005 was higher than before 2005 [1 358/11 vs. 8 275/13, 95% CI was (–625.337, –400.831), P<0.001]. According to gender distribution, total male data were 4 669, female were 3 340, non-checked were 1 624. According to age distribution, age were from 13 to 104 [(59±13) years]. Linear prediction was used to predict the age distribution with the " year” as the time axis. The results showed the stable linear prediction (\begin{document}$\hat y$\end{document}=0.016 1x+26.54, R2=3.42×105, P=0.601 108). According to height, height were from 138 cm to 192 cm [(161±7)cm], linear prediction results showed that the linear variation with height changes by value (\begin{document}$\hat y$\end{document}=0.110 5 x–60.911, R2=0.002 6, P=0.000 272). According to weight, weight were from 27.5 kg to 80.5 kg [(59.38±10.27) kg], linear prediction results showed that the linear variation with height changes by value (\begin{document}$\hat y$\end{document}=0.296 5x–537.24, R2=0.010 625, P=2.37×1014). Available 6 884 data showed the difference between serving areas by West China Hospital and official definition of western region. A total of 9 209 data obtained by analyzing main diagnosis, showed that the main site of disease was rectum (68.64%). Sigmoid was the main location of colon cancer (68.64%), and anal-rectal cancer was main of rectal cancer (27.06%).ConclusionPopulation characteristics from DACCA database could initially reflect the trend of increasing weight and BMI of colorectal cancer patients, and also reflect the regional distribution characteristics based on geographic information. They would be the clues for further database research.

  • EXPERIMENTAL STUDY ON MICROWAVE REGULATING IMMUNOLOGICAL DISORDER IN CHRONIC LYMPHEDEMA PATIENTS

    Twenty patients with chronic lymphedema had been treated by microwave heating. T-lympocyte subpopulation and HLA-DR phenotype of peripheral blood in patientswith lymphedema were examined by using dual colour flow cytometry before and after treatment. We found that CD4 (T helpe/inducer) in chronic lymphedema decreased significantly (Plt;0.01), HLA-DR increased significantly (P lt;0.05). After the microwave treatment, the CD4, CD4/CD8 ratio increased significantly; HLA-DR, HLA-DR+CD+8 lymphocyte reduced. It was clear that microwave could regulate the immunological disorder of lymphedema patients.

  • Health-Related Quality of Life for Disease Population in China based on EQ-5D: A Systematic Review

    ObjectiveTo systematically review the health-related quality of life (QOL) based on EQ-5D instrument for disease population in China. MethodsWe searched databases including PubMed, The Cochrane Library (Issue 5, 2015), EMbase, CNKI, VIP and WanFang Data from inception to August 2015, to identify studies involving healthrelated QOL about different diseases in China. A descriptive analysis was used to analyze health utility by disease and the proportion of patients who having problem in five dimensions, as well as comparing health-related QOL in different diseases. ResultsA total of 26 studies were included, involving 15 kinds of diseases. The health utility in different studies about the same disease was different. Diabetes ranged from 0.79 to 0.94, liver disease infected by HBV was from 0 to 0.80, hypertension was from 0.78 to 0.93, coronary heart disease was from 0.75 to 0.90, and chronic obstructive pulmonary disease was from 0.64 to 0.73. After ranking the health utility of fifteen kinds of diseases, the utility of kashin-beck disease and rheumatoid arthritis were 0.45 and 0.56, respectively, and the utility of children cerebral palsy was 0.44, which was the lowest. "Pain/discomfort" was the dimension with highest proportion of patients who having problem for most diseases while atrial fibrillation and children cerebral palsy were the "Self-care" and "Usual Activities". ConclusionEQ-5D is widely used in health-related QOL studies among different diseases in China. Many factors including sample source, severity of disease, complication and value set have influence on the measurement results of health utility.

  • Distribution of human papilloma virus subtypes detected by flow fluorescence hybridization in physical examination women in West China Hospital of Sichuan University

    Objective Using molecular biology method to detect and genotype human papilloma virus (HPV) in women taking physical examination in West China Hospital, Sichuan University, to explore the infection status and genotype distribution of HPV in normal women in Chengdu area, and to provide basis for early effective prevention and control of cervical cancer and domestic research and development of HPV vaccine. Methods Flow fluorescent hybridization technique was used to detect and genotype HPV-DNA in 25 148 healthy women taking physical examination in West China Hospital, Sichuan University between May 1st, 2018 and May 31st, 2019. The overall positive HPV infection rate, HPV genotype distribution, and characteristics of HPV infections were analyzed and calculated, and the HPV infection rates of different age groups were calculated and compared by chi-square test using SPSS 17.0 software. Results The overall positive rate of HPV infection was 12.19% (3 066/25 148). The high-risk HPV genotypes infection rate was 8.69% (2 186/25 148), and the top five subtypes with the highest infection rates were HPV52, HPV53, HPV58, HPV16, and HPV39. The low-risk HPV genotypes infection rate was 4.66% (1 171/25 148), and the top five subtypes with the highest infection rates were HPV61, HPV81, HPV43, HPV44, and HPV6. Single subtype infections were the main infections with a proportion of 81.74% (2 506/3 066), and the most common multiple infections were double infections which accounted for 13.96% (428/3 066). In different age groups, the HPV infection rate of group 60-69 was the highest (12.87%), while that of group 70-89 was the lowest (10.88%), but the difference among different age groups was not statistically significant (χ2=4.035, P=0.544). Conclusion According to the results of this study in women taking physical examination in West China Hospital, Sichuan University, we suggest adding HPV52, HPV53, and HPV58 which have the highest infection rate in high-risk HPV subtypes to the evaluation of domestic HPV vaccine screening and the cervical cancer prevention and control system.

  • DETECTATION OF CD3,CD4,CD8 AND CD44 CONTENT OF FINE NEEDLE ASPIRATION SPECIMENTS IN THYROID CANCER PATIENTS

    To search for the relationship between immune state and tumor metastases, CD3,CD4,CD8 and CD44 contents in 13 speciments of fine needle aspiration (FNA) from thyroid cancer patients were detected by the flowcytometry (FCM) and comparison between thyroid cancer and benign tumor was made. The result showed :in thyroid cancer group, CD+3,CD+4 cells and the ratio of CD+4/CD+8 reduced significantly (P<0.01),and CD+8 cell increased significantly (P<0.01), in metastases group,this change was much significantly. CD44 expressed significantly higher in cancer group than that of the benign thyroid neoplasms, and the change was related to the tumor metastases. The results indicate that CD44 can be as a marker of tumor and indicator of metastases. The disturbance of immune system results in active expression of CD44 by tumor cells, so, lead to metastases. It is helpful to the diagnosis of thyroid cancer, assessment of metastases and management in surgery.
