  • OBSERVATION OF EFFECT OF LASER PHOTOCOAGULATION BEFORE INTRAOCULAR SILICONE OIL REMOVAL ON GUARDING AGAINST RECURRENCE OF RETINAL DETACHMENT

    PURPOSE:To observe the effect of laser/photocoagulation on prevention of recurrence of retinal datachment following removal of intraocular silicone oil. METHODS:Laser photocoagulation was performed in a series of 24 cases (24 eyes)which had been formerly operated on with vitreoretinal surgery and intraocular silicone oil tamponade for retinal datachment,2 weeks to 3 months before intraocular silicone oil removal. Argon green or krypton red laser photocoagulation were done in whole or half cycle scattered laser burns behind the sclera! ridge. RESULT:Among the 24 eyes ,after the intraocular silicone oil had been removed, 22 (91.7%)eyes had their retinas retained in normal position. CONCLUSION :Laser photocoagulation before removal of intraocular silicone oil might be helpful in avoiding the recurrence of retinal detachment. (Chin J Ocul Fundus Dis,1997,13: 197-198)

  • Promoting Healthy Sichuan construction by emphasizing disease prevention

    This paper reviews the implementation of disease prevention and control policy of Sichuan province in three historic stages, and analyses the situation, challenges and tasks about disease prevention and controlling in future, based on the new requirements about hygiene and medicine. Three strategies should be taken: 1) to improve the capacity and quality of health service; 2) to optimize the structure of institutions and talent teams; 3) to focus on the Healthy Sichuan construction, the treatment-prevention synergy, and the poverty health care.

  • Research progress of coronary artery occlusion associated with transcatheter aortic valve implantation

    Transcatheter aortic valve implantation (TAVI) has become the main treatment for elderly patients with middle and high risk aortic stenosis. However, coronary artery occlusion (CAO) related to TAVI is a very serious complication, which often leads to poor prognosis. Therefore, active preoperative prevention is particularly important. Preoperative computed tomography evaluation, bioprosthetic or native aortic scallop intentional laceration and chimney stent implantation technology can prevent TAVI-related coronary orifice obstruction. Ensuring commissural alignment during operation can reduce the occurrence of coronary occlusion, but its long-term prognosis needs further study. In addition, percutaneous coronary intervention is the main treatment, but there are problems such as difficult coronary access after TAVI. This article summarized the research progress in the mechanism, prevention and treatment of CAO related to TAVI.

  • Evidence-based Practice of Pit and Fissure Sealants

    We searched randomized controlled trials, meta-analysis and systematic reviews from OVID-EBM Reviews which included ACP Journal Club, The Cochrane Library, and MEDLINE(1991 to 2005 ) to evaluate clinical effectiveness of pit and fissure sealants for caries. The resultsshowed that pit and fissure sealants were recommended to prevent caries of the occlusal surface. The effectiveness varied between the two types of sealants, in general, flowable resin composite had a more satisfactory retention than glass ionomer composite. Acid etch was helpful for less microleakage and more satisfactory retention. Mechanical air-abrasion with acid etch may have the best border seal, However, we were not sure of the effectiveness of Er:YAG laser, technique of dental drill preparation and splicing. More high quality clinical trials on pit and fissure sealants are still needed.

  • Comparison of epidemiological characteristics and prevention of breast cancer in China and the United States

    ObjectiveBy comparing the epidemiological characteristics of the incidence and death of female breast cancer in China and the United States, the differences in prevention and screening strategies between China and the United States were analyzed to explore the prevention and control measures of female breast cancer in China. MethodsBased on the relevant data released by the Global Burden of Disease in 2020, the National Cancer Center of China, and the China Health Statistical Yearbook, the new cases and deaths of breast cancer in Chinese and American women in 2023 were estimated respectively, and the incidence, mortality and time trend of breast cancer in Chinese and American women were analyzed. ResultsIn China, 376 789 new cases of female breast cancer and 116 791 deaths were expected in 2023. In the United States, approximately 297 790 women were expected to be newly diagnosed with breast cancer in 2023, representing approximately 15.2% of new cancer cases. About 43 170 women died from breast cancer, accounting for about 7.1% of all cancer deaths. The incidence of breast cancer in women in the United States during the period 1975–2020 gradually increased and then stabilized; In contrast, the incidence of breast cancer among Chinese women increased year by year during the period 1990–2020. In recent years, the incidence and mortality rates of breast cancer in Chinese women had increased more than those in the United States, and there was a large difference between urban and rural areas in China. ConclusionsBoth China and the United States face a large burden of female breast cancer, and the characteristics of female breast cancer in China are similar to those in the United States. To reduce the burden of breast cancer in Chinese women, further efforts should be made in various aspects, such as strengthening breast cancer education, raising public health awareness, improving diet structure, cultivating healthy lifestyle, increasing screening efforts, and improving medical level.

  • Nonsteroidal Anti-inflammatory Drugs for Prevention of Colorectal Neoplasms: A Systematic Review

    Objective To assess the effect of nonsteroidal anti-inflammatory drugs (NSAIDs) for the prevention of colorectal neoplasia. Methods A systematic review of all relevant randomized controlled trials and quasi-randomized controlled trials of NSAIDs for prevention of colorectal neoplasms was performed by using The Cochrane Collaboration recommended methods. Results Nine trials were included and assessed. There was sufficient evidence for aspirin to prevent the development of colorectal adenomas compared with placebo in three trials of high quality and large sample size with relative risk (RR) 0.81, 95% confidence interval (CI) 0.72 to 0.91 and P=0.000 5 . No adequate evidence supported aspirin in the prevention of development of colorectal cancer (RR 0.97, 95% CI 0.79 to 1.20, P= 0.79). However, there was no evidence to support sulindac and celecoxib curing or preventing colorectal adenomas or familial adenomatous polyposis (RR 0.71, 95% CI 0.49 to 1.03, P= 0.07 and RR 0.90, 95% CI 0.76 to 1.07, P=0.23). No evidence on the dose of NSAIDs was used for prevention of colorectal adenomas at present. No significant difference was seen in the number of adverse events between patients taking NSAIDs and those taking placebo (P=0.9). Conclusions Aspirin may prevent the development of colorectal adenomas and may avoid polypectomy for 1 in every 10 to 18 persons but we don’t know whether aspirin can be substituted for endoscopically removed colorectal polyps. However, the true clinical benefit for prevention of colorectal neoplasia of NSAIDs should be considered.

  • Prevention and treatment of iatrogenic medial collateral ligament injuries in total knee arthroplasty

    ObjectiveTo summarize the prevention and treatment of iatrogenic medial collateral ligament (MCL) injuries in total knee arthroplasty (TKA).MethodsThe relevant literature about iatrogenic MCL injuries in TKA was summarized, and the symptoms, causes, preventions, and treatments were analyzed.ResultsPreventions on the iatrogenic MCL injuries in TKA is significantly promoted. With the occurrence of MCL injuries, the femoral avulsion can be fixed with the screw and washer or the suture anchors; the tibial avulsion can be treated with the suture anchors fixation, bone staples fixation, or conservative treatment; the mid-substance laceration can be repaired directly; the autologous quadriceps tendon, semitendinosus tendon, or artificial ligament can be used for the patients with poor tissue conditions or obvious residual gap between the ligament ends; the use of implant with greater constraint can be the last alternative method.ConclusionNo consensus has been reached to the management of iatrogenic MCL injuries in TKA. Different solutions and strategies can be integrated and adopted flexibly by surgeons according to the specific situation.

  • Interpretation of hospital infection management key points for Implementation Rules for the Accreditation Standards for Tertiary Comprehensive Hospitals in Sichuan Province (2023 Edition)

    Based on the national 2020 and 2022 versions of Accreditation Standards for Tertiary Hospital, the Hospitals Accreditation Office of the Sichuan Provincial Health Commission organized to develop Implementation Rules for the Accreditation Standards for Tertiary Comprehensive Hospitals in Sichuan Province (2023 Edition). In order to guide the evaluated hospitals to comprehensively understand and master the content of hospital infection prevention and control (IPC), this article interprets the main evaluation points and scoring methods of hospital infection management in the detailed rules, emphasizes on organizational management, system implementation, monitoring/supervision, and connotation improvement for IPC. The purpose is to make the evaluated hospitals attach importance to the standardized implementation of daily work of IPC, focus on the routine, objective, and quantitative approach to accreditation work, and continuously achieve the effect of quality improvement in IPC.

  • Clinical analysis of 282 patients with infectious endophthalmitis

    Objective To analyze the causes and pathogens of infectious endophthalmitis in our hospital. Methods The clinical data and laboratory findings of 282 inpatients with infectious endophthalmitis were retrospectively reviewed. There were 206 males(73.05%)and 76 females(26.95%)with a mean age of (36.0plusmn;11.2) years (range from 2 to 79 years). Except 14 patients who lost their eyeballs as of serious conditions, pathogen examinations of vitreous specimen had been performed for all remaining 268 cases. The clinical and laboratory data were statistically analyzed by Chi-square and Kappa consistency tests, when P<0.05 the difference should be considered as statistically significant. Results The 4 major causes of infectious endophthalmitis of those 282 patients included ocular trauma (177 cases, 62.77%), infectious keratitis (61 cases, 21.63%), eye surgery (32 cases, 11.35%), and endogenous endophthalmitis (12 cases, 4.25%). 19.21% of the patients in ocular trauma group was 2-13 years old, 55.74% of the patients in infectious keratitis group was 14 -50 years old, and 60.61% of the patients in eye surgery group was 51-79 years old. The major pathogens were fungus and staphylococci each accounted for 30.63% of all cases. Most of the pathogenic bacteria were sensitive to ciprofloxacin (81.81%) and gentamicin (77.92%). Conclusions The major reason of infectious endophthalmitis was ocular trauma, and the major pathogens were fungus and staphylococci. Those findings may help us to treat infectious endophthalmitis patients appropriately.
