  • Research progress on relation between dietary factors and risk of pancreatic cancer

    ObjectiveTo summarize the influence of dietary factors on the risk of pancreatic cancer and its possible mechanism. MethodThe literatures relevant to studies of the influence of dietary factors on the risk of pancreatic cancer were collected and reviewed. ResultsThe total intakes of carbohydrate, fatty acid, protein, and vitamin affected the risk of pancreatic cancer, and the different substances belonging to the same nutrients had different effects on the risk of pancreatic cancer. In addition to nutrients, the popular beverages and different dietary patterns in recent years also affected the risk of pancreatic cancer through certain mechanisms. ConclusionDietary factors can affect risk of pancreatic cancer through a variety of mechanisms, and it might decrease risk of pancreatic cancer by intervening in dietary factors in daily life for healthy people.

  • 婴幼儿检查前水合氯醛灌肠效果影响因素分析

    【摘要】 目的 总结婴幼儿检查前应用水合氯醛灌肠镇静效果的影响因素及对策。 方法 2008年11月-2010年11月对450例进行MRI、CT、多导心电图等检查前的患儿应用10%水合氯醛灌肠。 结果 308例(68.4%)在20 min内进入睡眠,85例(18.9%)患儿能在20~30 min内进入睡眠镇静,57例(12.7%)无效。 结论 水合氯醛保留灌肠的效果受时间、体位等诸多因素影响,采取必要的措施就能提高灌肠的有效率。

  • Analysis of self-perceived burden status and its influencing factors in patients undergoing spine surgery

    Objective To investigate the status of self-perceived burden (SPB) in patients undergoing spine surgery and to explore its influencing factors, in ordering to provide a basis for formulating corresponding nursing interventions. Methods A cross-sectional survey was conducted on patients undergoing spine surgery in Department of Orthopedic Surgery, West China Hospital of Sichuan University between May and August 2024. The patient general information questionnaire, the patient SPB Scale, the Barthel Index, the Medical Coping Modes Questionnaire, and the Social Support Rating Scale were used to investigate the SPB status and its influencing factors in patients undergoing spine surgery. Results A total of 230 patients were included. There were 113 cases in the non-SPB group and 117 cases in the SPB group. There were statistically significant differences in age, marital status, occupation, payment method, the number of family, disease diagnosis, Barthel Index score, caregiver identity, and caregiver gender between the two groups of patients (P<0.05). Among 117 patients with SPB, 83 (36.09%) had mild SPB, 27 (11.74%) had moderate SPB, and 7 (3.04%) had severe SPB. The average SPB scores for mild, moderate, and severe patients were (24.06±2.92), (33.07±2.87), and (44.86±4.56) points, respectively. The results of binary logistic stepwise regression analysis showed that the patient’s marital status, disease diagnosis, and caregiver gender were independent influencing factors for SPB in patients undergoing spine surgery (P<0.05). Conclusion The SPB of patients undergoing spine surgery is at a mild to moderate level, which is affected by factors such as marital status, disease diagnosis, and caregiver gender.

  • Influencing factor analysis of malignancy rate of pulmonary nodules based on pathological outcomes and optimization of integrated management strategies

    Objective To analyze the benign-malignant outcomes of pulmonary nodules in surgical patients and their influencing factors, and provide evidence and ideas for optimizing and improving the integrated management model of pulmonary nodules. Methods From October to December 2023, a convenience sampling method was used to select patients who underwent lung surgery at West China Hospital, Sichuan University between July 2022 and June 2023 for this study. The malignancy rate of postoperative pathological results of pulmonary nodules and its influencing factors were analyzed using univariate analysis and multiple logistic regression. Results A total of 4600 surgical patients with pulmonary nodules were included, with a malignancy rate of 88.65% (4078/4600) and a benign rate of 11.35% (522/4600). Univariate analysis showed significant differences in malignancy rates among different genders, ages, methods of pulmonary nodule detection, and smoking histories (P<0.05); however, no significant difference was found regarding place of birth or family history of lung cancer (P>0.05). Multiple logistic regression analysis indicated that females [odds ratio (OR)=1.533, 95% confidence interval (CI) (1.271, 1.850)], older age groups [61-75 vs. ≤30 years: OR=1.640, 95%CI (1.021, 2.634); >75 vs. ≤30 years: OR=2.690, 95%CI (1.062, 6.814)], and pulmonary nodules detected during physical examinations [OR=1.286, 95%CI (1.064, 1.554)] were high-risk factors for malignancy, with statistical significance (P<0.05). Conclusion In the integrated management of pulmonary nodules, it is crucial not to overlook females or older patients, as they may be more significant influencing factors than smoking; furthermore, lung examinations are effective means of early detection of malignant lung tumors and are worth promoting and popularizing.

  • Analysis of risk factors of intraoperative blood transfusion during liver transplantation

    ObjectiveTo explore risk factors of blood transfusion during liver transplantation and construct its prediction model. MethodsThe patients underwent liver transplantation who met the inclusion and exclusion criteria of this study from March 2020 to December 2020 in the Beijing Youan Hospital of Capital Medical University were retrospectively collected. The univariate and logistic multivariate analysis were used to evaluate the risk factors of blood transfusion during liver transplantation and construct the prediction model for intraoperative blood transfusion. ResultsA total of 151 eligible liver transplantation patients were collected in this study, including 51 non-transfusion patients and 100 transfusion patients. The univariate analysis results showed that the differences of primary diagnosis, preoperative hemoglobin (Hb), platelet count, prothrombin time, international normalized ratio, Child-Turcotte-Pugh score, and end-stage liver disease (MELD) score were statistically different between them (P<0.05). The above variables selected by the univariate analysis were selected by stepwise method, then the preoperative Hb and MELD score were selected into the multivariate logistic regression analysis, the results showed that the preoperative Hb≤113 g/L and MELD score >14 increased the risk of blood transfusion during liver transplantation [Hb: OR=6.652, 95%CI (2.282, 19.392), P<0.001; MELD score: OR=16.037, 95%CI (6.336, 40.592), P<0.001]. The logistic regression model predicted the area under receiver operating characteristic curve was 0.873 [95%CI (0.808, 0.919), P<0.001], the sensitivity and specificity were 91.0% and 67.5%, respectively, Youden index was 0.674, the accuracy was 86.1%. ConclusionsResults of this study suggest that preoperative Hb ≤113 g/L and MELD score>14 increase risk of blood transfusion during liver transplantation. Logistic regression model constructed according to preoperative Hb and MELD score has a better sensitivity and specificity of intraoperative blood transfusion.

  • Analysis of the occurrence and influencing factors of fatigue in asthma patients

    ObjectiveTo investigate the fatigue of asthma patients, and to analyze its influencing factors, and provide a reference for clinical intervention.MethodsThe convenience sampling method was adopted to select asthma patients who were in clinic of the First Affiliated Hospital of Guangxi Medical University from November 2018 to March 2019. The patients’ lung function were measured. And questionnaires were conducted, including general data questionnaire, Chinese version of Checklist Individual Strength-Fatigue, Asthma Control Test, Chinese version of Self-rating Depression Scale. Relevant data were collected for multiple stepwise linear regression analysis.ResultsFinally, 120 patients were enrolled. The results of multiple stepwise linear regression analysis showed that age, education level, place of residence, time period of frequent asthma symptoms, degree of small airway obstruction, Asthma Control Test score and degree of depression were the influencing factors of fatigue in asthma patients (P≤0.05). Multivariate linear stepwise regression analysis showed that degree of small airway obstruction, degree of depression and time period of frequent asthma symptoms were the main influencing factors of fatigue in asthma patients, which could explain 51.8% of the variance of fatigue (ΔR2=0.518).ConclusionsThe incidence of fatigue in asthma patients is at a relatively high level. Medical staff should pay attention to the symptoms of fatigue in asthma patients. For asthma patients, it is recommended to strengthen standardized diagnosis and treatment, reduce the onset of symptoms at night and eliminate small airway obstruction. Psychological intervention methods are needed to improve patients’ depression, reduce fatigue symptoms, and improve quality of life.

  • Research progress on risk prediction model of anastomotic leakage after gastric cancer resection

    ObjectiveTo systematically summarize the research progress in risk prediction models for postoperative anastomotic leakage in gastric cancer, and to explore the advantages and limitations of models constructed using traditional statistical methods and machine learning, thereby providing a theoretical basis for clinical precision prediction and early intervention. MethodBy analyzing domestic and international literature, the construction strategies of logistic regression, least absolute shrinkage and selection operator (LASSO) regression, and machine learning models (support vector machine, random forest, deep learning) were systematically reviewed, and their predictive performance and clinical applicability were compared. ResultsThe traditional logistic regression and LASSO regression models performed excellently in terms of interpretability and in small-sample scenarios but were limited by linear assumptions. The machine learning models significantly enhanced predictive capabilities for complex data through non-linear modeling and automatic feature extraction, but required larger data scales and had higher demands for interpretability. ConclusionsDifferent prediction models have their own advantages and limitations; in practical clinical applications, they should be flexibly selected or complementarily applied based on specific scenarios. Current anastomotic leakage prediction models are evolving from single factor analysis to multi-modal dynamic integration. Future efforts should combine artificial intelligence and multi-center prospective clinical studies to validate, so advancing the development of precise and individualized anastomotic leakage predictive tools for patients after gastric cancer resection.

  • RISK FACTORS OF PANCREATIC AND PERIPANCREATIC SEPTIC NECROSIS IN ACUTE PANCREATITIS

    A review of patients with acute pancreatitis treated in this hospital in recent 10 years was made.To determine the risk factors of septic necrosis in and around the pancreas,32 cases with septic necrosis which were proved in surgical operation and 44 cases without septic necrosis(as control)were included in this study.The possible factors were comparatively analysed.The results showed that septic necrosis in and around the pancreas obviously related to the diagnostic or therapeutic punctures,early surgical drainage and paralytic ileus(OR 302-548,P<005),but there were no associations with age,etiology,shock,respiratory failure and total parenteral nutrition(OR 078-126,P>005).The authers suggest that either pancreatic,peripancreatic puncture or early surgical drainage should be limited and any medication which makes paralytic ileus deteriorated such as atropine should be avoided in the treatment of acute pancreatitis.

It is related to the course of diabetes, blood glucose, blood pressure, and blood lipids, among which the course of disease and hyperglycemia are recognized main risk factors. In addition, other factors which include heredity, gender, age, obesity, pregnancy, insulin use, can also affect the occurrence and development of DR, but there is no unified conclusion about its correlation. A comprehensive understanding of the risk factors that affect DR can provide new ideas for the prevention, diagnosis, treatment, and intervention of DR.
